The Hair Loss Industry Is Broken | Evidence Quality Masterclass

  • In the hair loss industry, the phrase “science-backed” no longer means anything.
    Marketers have uncovered a repeatable formula to manipulate clinical trials - all by taking advantage of the broad legal definition of "hair loss" and "hair growth".
    First, they design their studies in such a way to all but guarantees good results - and for any pill, supplement, topical, device, or shampoo they want to sell. They do this by:
    -Starting a study in September to produce false-positive hair regrowth due to hair cycle seasonality
    -Jam-packing people who just quit minoxidil into a placebo group, so they shed lots of hair
    -Picking study participants with temporary forms of hair loss that will self-resolve without a pill
    -Advertising a "clinically studied" product, then bait-and-switching to sell an unstudied product
    -Grouping vellus and terminal hairs together to force statistically significant hair count results
    ...and more.
    The end-result: consumers buy these products, their "real-world" results almost never match the study's "clinical results", and then they blame themselves. In actuality, the blame should be shifted to the companies who knowingly engage in these practices - at the expense of consumers and to the benefit of their bottom line.
    In this video, we’ll expose these practices.
    We’ll feature dozens of examples from big brands - companies with study designs so deceptive that their "clinical findings" should not - in our opinion - be trusted.
    Then, we’ll reveal a step-by-step framework you can employ to protect you from deceptive marketing practices: a framework for evaluating evidence quality.

      Currently I am 24 and balding , most probably at norwood 2, consulted with a dermatologist, he said I have MPB ,most likely will prescribe minoxidil, but I am hesitant to use it, need suggestions from your side how to regow and maintain hair naturally

      I get the skepticism. I retained my hairline but lost density in the presentation of female pattern hair loss - which is more globalized diffuse thinning. That makes it look different from standard cases of AGA.

      Im a 18 year old boy and i have been facing hairloss like for a year now my pattern is like thinning on top and side and back area like lines in which there is less hair i have lost density and it happened when i was previous year in huge stress and then pain happened in my scalp and then doctor told me thhat its androgenic but i dont think althougj i have family heritage but im so young and can you pls gibe your opinion on it my pstgern is like as i remeber first lines were forming in my top scalp and then hair started to losse density and now my density is low can it be reversed? I am taking finanstride for 2 moths 0.5mg and ive seen that my hair have become thick and hairloss stopped like in first week taking pill kindly give your suggestion 🙏 what can be the cause of it ive been facing it for one year and also can your recomend some treatments as im taking only 0.5mg ffin and multivitamin and not minoxifdil will ill get my hair back

    The internet can be a scary place sometimes. Even media types like forums advertise expensive anti-hair-loss products. People suffering from hair-loss are very receptive to 'new' types of solutions, because of desperation. Important not to get mislead.

    Can I make two video requests please? The first is a video on diffuse unpatterned alopecia (DUPA), there is a lot of confusion in the community regarding what causes this type of hair loss and how to treat it. Further many more men than originally thought have this type of hairloss than originally thought. With an important point being that until men with dupa stabilise their hairloss they can’t get a hair transplant. The second request is a video covering your thoughts on CosmeRNA.       Thanks! I really appreciate that. I'm sorry we haven't gotten to the video, but it's still in the works! We ended up turning that project into a manuscript about topical / mesotherapy pharmaceutical 5-alpha reductase inhibitors, and if the manuscript passes peer review, we're going to publish it + make a video about the key findings - similar to what we did with our microneedling paper. I know I owe you this video, and I promise we'll get to it!

      Nothing amazing but my hair are still getting thicker, after 16 years of balding. I am a little Nw2 with thick hair. Should be completely bald.

    I use topical finasteride with minoxidil and I have definitely got thicker and new hair growth. I am also testing my testosterone with 3-monthly blood tests and all levels are normal.

    Thanks for the video! I wanted to request to please make a video on hair loss caused by hard water.
    I have been facing the hair loss when I went to college and wanted to ask how it affect (I learned that it is because of blood supply)
    Also, what are the fixes one can use practically? Like, I cannot install an expensive water softener system, but there exist some tap water softeners at cheaper prices that are portable, but are they even effective? that's also what I wish to know.
    To other I wish to request to please like the comment since there is little to no material on hard water caused hair loss on internet and this thing is not temporary, the rate of hair loss is affected to even 10 years sometimes so if you are travelling to a hard water region we are screwed for years then!

    Rob,
    I would like to ask you if you are continuing the research on massage, and are you perhaps elevating it to a higher level than self-assessment, so that you can more confidently claim that it has an effect? I'm asking only because it takes up a lot of time, really a lot of time is spent on it, but if it's effective, then every minute spent on it is worth it. On the other hand, if it's not effective, then spending 40 minutes daily for 2-3 years to stop hair loss through massage, but doing so unnecessarily, would be harsh.

    First of all- thank you Rob for showing the way to transparency for everyone. These conclusions are powerful tools everybody can use for free.
    Second- I think there is a lot to benefit, if you would make a video about the science and data surrounding the two really unmentioned interventions for hair loss-
    massages, and scalp exercises.
    I'm referring to what you researched as you said in the video, and to the exercises Tom Hagerty talks about in his blog. Knowing the anatomy of the scalp is crucial to understanding hair loss, and feels like there is a lot to gain from researching more of this stuff. Pretty sure everyone here doesn't even know the occipitalis muscle and how to move it, since it is never even mentioned in any of your videos.

    You and your team are amazing. You are making people smarter not only about hair loss but in general about any drug they may buy. I wish that everyone that is starting their hair loss journey finds this video first, I wish that anyone who may buy any drug watch this video. I have good hair but am nearing a little past a norwood 2 and I already have a prescription for oral Finasteride and already have topical Minoxidil. I will still be taking doses of vitamins and minerals just to make sure i do not have a underlining deficiency, but those are relatively inexpensive anyways. Your a gem and you have earned my subscription, trust and interest. Our society needs more people like you.

    Somewhat OT but an anecdote: For a few months I've been taking taurine (1,000 mg, 3x/day) and L-arginine and getting a better night's sleep. I do scalp massages kind of sporadically / whenever I think of it.
    I s w e a r I have more hair on the top of my head, down the part line. I look in the mirror and ask myself, "Is it my imagination or am I seeing less scalp?? No, it's not my imagination...this is nuts." My hair just appears fuller and is growing longer in the front corners and there's hardly any fall-out.
    IDK if it's the taurine, the L-arginine, the scalp massage, or the better sleep -- or all of the above, but I'm 53 and I've had thinner hair on the top of my head since my late 20s and now it's filling in. I'm amazed.

    I love this channel a lot. Rob, I have a question. The scalp massage session treatment you've talked about in earlier videos, those 20-25 minute sessions, what harm is done if I skip those once in a while. Because sometimes, I'm just in a great haste and I simple can't find 20 minutes on some mornings and nights. Will it not work at all?

    This video is gold.
    Have you ever thought about using your platform to run crowdfunding campaigns to finance innovative, open-source, and high-quality hair loss studies? You'd be the prime individual to do that.
    Many promising interventions are "physical"(as opposed to pharmaceutical, like microneedling), and therefore not really "patentable" by big pharma.
    Big players have therefore little interest in investing in them, since they wouldn't be guaranteed to own that treatment and get the returns for their investment.

      Thanks for watching! I'm definitely open to crowdfunding hair loss studies - provided there's enough preclinical support, early clinical support, and/or biological plausibility to suggest there's at least a decent chance that the intervention would achieve a great outcome. It'd be awesome to set up crowdfunding campaigns where supporters could also gain fractional ownership of that intervention's patent.

    Hey Rob! thanks for this video, as usual very helpful :). Recently I've been looking into Laser helmets but it is not easy to find their clinical studies nor their results. So besides the most expensive capillus model (my got is expensive!), have you looked into other brands and found some products with good clinical studies results?

    Very informative, I feel I now have an understanding of what all this study terminology means. That said, I can't help feeling like this leads back to square one - use fin and min and hope you don't get horrifying side effects, because nothing else will work.
    Probably gonna shave my head before I go that route.

      That's what I thought (shave my head) but as I noticed hair gradually thinning I picked up the courage and jumped on to finasterided, take 1mg 3x a week, mon, wed and fri. Been on it since April this year and have noticed hair thickening and noticeably less hair loss in the shower. Regret not taking it much earlier due to the potential side effects.

      @@rcl300018 yeah well. I'm sticking to the shave plan. Maybe if the side effects were genuinely super uncommon. Maybe if the side effects weren't potentially permanent. Not to mention that you first lose a ton of hair anyway just for starting the stuff, and can take like a year or more for improvement to show, if it does.
      Just not worth it imo.

    My hair loss started after 5-6 months of hair loss. The hair loss has largely stopped but now the hair on top of my head is thinning and the rest of my body is losing hair such as beard, underarms, eye brows.

    My hair loss is related to my thyroid or my thyroid medication.
    I finally feel good, have energy, no other symptoms, but it doesn't seem to matter, my hair is falling out.
    I can lower or raise my dose, change medications .... doesn't matter.
    No one addresses this hair problem, EVER.

      You could have an iodine deficiency, it causes thyroid illness. But no doctor tells you to take iodine supplements because they want to get money for the thyroid medication.
      You can test if you are iodine deficient.
      The thyroid needs iodine to work but the body aswell. Hair will fall out and get thinner if your body doesn't get enough iodine

      Dose NOT work, at all. Don't waste your time. It is NOT a laser. It is just a red light, nothing more. Even if it did work, the light does not penatrate your hair. You'd have to be completely bald for any light to reach your scalp.

    Hey i am 23 suffering from male patterns baldness for 2 3 years i never tried minoxidil or finastride.i use only demaroller and oil but the results are very minor in contrast the hair receds gradually. In your opinion is there any way to prevent hair loss if yes then what should i do.

    I am suffering from hair loss for the past 15 odd years. It's now at norwood 4.5 to norwood 5 something. I've read about most of these scams and everytime I wanted to try one, I'd procrastinate due to fear of side effects. Could've done the usual fin, min, transplant. But no, fin has some debatable side effects, min has a heavy upkeep and stopping it may result in more shedding from what I've learnt and transplant, I dont wanna lose my donor area forever. So still waiting for a natural / non-invasive, no side-effect, permanent cure which may never happen for a few centuries by the looks of it. We live in such a greedy world. Incomprehensible.

    So look for interventional studies (rather than mechanistic or observational) that are randomized, double blind, and controlled, with subjects who are representative of the target population and products that match what was actually tested. Also look for results based on a phototrichogram rather than self reporting or photos. Forget hair loss - this is general information that applies much more broadly. Thanks.

      1. Thanks for the kind words. In the last 7 years, there has not only been an increase in hair loss funding overall, but a better appropriation of funds to research groups with innovative ideas / interventions -- some of which don't even target anti-androgenic mechanisms. I'm hopeful that we'll see the fruits of this labor -- in the form of new treatments -- in the next 5-10 years. The biggest challenge with a transparent fund is that you still have to decide who to fund with the money. In my eyes, some of the best minds in this space are already funded, and now it's time to wait and see how those preclinical / phase I / phase II studies go.
      2. Over the last year, we had so many projects going on with the site that I couldn't find enough hours in the day to make progress on them and create videos at the quality I wanted. So, I decided to pause RUclips uploads to focus on those projects. As of today, they're mostly finished (new manuscripts, new interactive guides, new community features, etc.). And that means that our channel is back! Thank you for watching and if you have any video recommendations, please let me know.

    Great informatic video, I have a question though, did you reach any data concerning the continues use of minoxidil for more than a year? as I've seen most of the studies are made for a year and some months , nothing more , which makes it in my opinion a not reliable option, as from my knowledge about reading and watching videos like yours, minoxidil doesn't focus on the problem which is DHT rapid production in scalp ( as a blocker), but it accelerate hair follicle to produce hair and remove terminal hair(or weak hair that will fall anyway) , which introduce shedding every certain period until minoxidil doesn't work at all because the follicle is over used and its health % is lowered due to this drug, hence why the date is mostly 1 year and not supported over a year ,... Too much words but is this claim accurate or am i going the wrong way here, because I started minoxidil 2019 and then got progressive hair loss after 3 months then stopped it then after 9 months i did 5 sessions PRP which helped a lot ( no idea if its from minoxidil or the 5 months PRP sessions )then after those 5 months my hair went to fall phase again , so i tried Rogaine again after 2 years but only 1 months due to doctors recommendation that I have Androgenic alopecia , so currently stuck with other solutions like flotene and vici and such , which are not doing much and considering using Rogaine but a bit afraid and losing hair very fast.

    Experienced heart palpitations from topical minoxidil, and I’m on break until further notice. Should I get back on and only apply once a day, or twice a day but half the dosage, or don’t get back on?

      I'm not a doctor and cannot offer medical advice. I do know people who managed to eliminate these side effects by titrating the daily dose - like you suggested. There are also other potential factors to consider. For instance, high-strength minoxidil concentrations likely have higher systemic absorption, and thereby a higher risk of side effects. If you're combining topical minoxidil with retinoic acid, microneedling, or any other ingredient/therapy that increases the permeability of the stratum corneum, that will also drive up systemic absorption. So, consider playing around with these variables (with the guidance of a qualified doctor). Finally, we do have a few members who switched from minoxidil to nanoxidil and noticed these side effects dissipate, but it also came at the expense of reduced treatment efficacy.

    What do you think about cold water rinses to stimulate the scalp? I’ve always suffered from hairloss and couple years back my hair grew back thicker and longer than ever. It was unbelievable. I just recently realized that when my hair grew back, I was doing daily cold water rinses. After I stopped doing these cold water rinses I lost all my hair again. Could there be a connection???
