Doesn't exactly fit the bill, but by NOT offing her in your first meeting, she had a few more years of dealing with the consequences of the horrible experiments she took part in plus indoctrination eating at her mind from the inside out until it literally blows up in her face. You can also "help" Khalisa al-Jilani's viewership by punching her lights out multiple times lmao, that kinda ruins her with what I can only imagine as brain trauma 😅

V helps Judy and some of her chooms that work at the club “Clouds” take control of the establishment from the Tyger Claws gang and give it to the workers in an attempt to create a better working environment, which goes awry after killing the Claws managing the bar when other members end up hitting back later on, zeroing many of Judy’s chooms and the dolls.
I laughed. It was so silly, and it felt like a nod to AC2 where you have the flying machine that magically works but in actuality it never could. The design he uses in RDR looks a lot like the one used in AC
The side quest called "Family Values" in Assassin's Creed Odyssey. Some kid locks himself in a cage after hearing a prophecy that his mom would die and his dad would scream in agony, with it being his fault. So after he asks you to help him prevent the prophecy from coming true, your actions instead inadvertently become part of the circumstances that makes sure it DOES happen.
Sellen: "I refuse to coddle or cast kind words." *Proceeds to coddle and praise us throughout her questline* Missed on getting Azur and Lusat armor cause my heart cant handle seeing her turn into a ball
RDR2, the balloon guy. Poor guy thinks you just going for a balloon ride. Which turns into scouting a prison, and then being shot at by gang members chasing a friend of yours. Though I will say, the "slow motion" cut scene kills in that balloon are hilarious.
So I'm just playing Baldur's Gate 3 for the first time. I went to help the gnome the goblins were bullying, and instead of stopping the windmill, I accidentally sped it up. Needless to say, I was shocked and felt terribly when I saw the little guy launched into orbit.
The poachers are actually tied to Diallos's questline, not to entering Jarburg. So it's possible to finish Alexander's and Jar Bairn's questlines without the poachers attacking and Jarburg stays peaceful the whole game.
Grand Theft Auto and Red Dead Redemption series, specifically the flying machine side quest featured im the video are EXACTLY why I do all side missions/quests in any game that offers them. I can still remember first seeing him nose dive off the cliff almost immediately lmao, what followed was me staring with my jaw open followed by a lot of laughs.

In Elden Ring, Sorceress Sellen gets turned into the ball, called a Graven Mage (also sometimes called Graven Mass) due to her study of the Primeval Current. Its suggested that she might have been the reason for the other Graven Mass/Mages you see around the place through the school of Graven Mages. Its implied this might be the reason she was kicked out of the academy to begin with. So you helping her gave her the ability to continue that study which ultimately caused her to get "exactly what she wanted". Whether she regrets it or not, maybe?

Yes Man warns you about having a military force just waltzing around, but they become a viable police force in the area.

In RDR2, I came across the encounter in Saint Denis with the robber that mugs the newlywed couple for the first time but got to the scene too slowly and the wife was shot as a result. The husband wailed on about how his life was over and I felt so bad for him that I loaded from a previous save and tried again, successfully. This time though, the wife coldly said "Hmph, at least there was a real man around to do something" and started walking away without him. Again, I felt terrible... I'd saved her life, yes, but I felt like my actions condemned that husband to a life of misery no matter what I did.

For some goofy reason, they're at odds with one another because of the founders who were apparently feuding and insisted that the families continue to bicker years later. However, two people, a young man from one hotel, and a girl from the other, are in love, think Romeo and Juliet. Thankfully, the pair don't perish in a tragic misunderstanding, but the end result of trying to help them may as well have been something as bitter as the famous play. In an initially unrelated side quest, you meet a man named Joaquin who has ambitions of becoming a businessman, and help him out with some pep talk and occasional favors. Eventually, the two quests converge and Joaquin offers to take the stocks from both hotels, putting them under his control. Except this leads to Joaquin firing both families, leaving them on the street. The romantically involved pair might be able to see each other freely now, but they're also without any means to get married or find a home. It's a shame, since the lovers really do seem to care about each other, and were suffering because of their families stupid feud, but they wind up paying the same price when it was only their families who should have been screwed over for prioritizing their success over everything else.
As soon as I am done in whiterun getting the main quest started on a new playthrough I immediately take the carriage to get the Mace of Molag Bhaal. It is my main focus after the very first parts of the game after getting the shout. The Mace rocks in the first part of the game, filling all your soul gems
In Gta IV I remember the mission to help Marnie Allen from her addiction to discover that in Gta V she works for a sect that takes advantage of people.
