  • Dr. Ebraheim’s educational animated video describing injury to the acromioclavicular joint.
    Shoulder separation can be defined into three useful and practical classifications:
    1-Sprain
    2-Partial tear
    3-Complete tear
    •clavicle up
    •posterior dislocation
    •very high
    •inferior dislocation
    Dislocation of the acromioclavicular joint is different from dislocation of the shoulder joint. A dislocation of the shoulder joint involves dislocation of the glenohumeral joint.
    Acromioclavicular joint separation
    The cross arm test is performed with direct palpation of the joint to access swelling & pain.
    Shoulder separation is almost always the result of a sudden, direct trauma to the top of the shoulder.
    If the clavicle is prominent, then the injury is severe.
    In mild cases, stress x-rays with 10 lb. of weight is helpful.
    x-rays show unstable significant injury.
    Treatment of AC joint separation is usually done with the use of a sling, ice therapy or tapping.
    Surgery is considered in unstable cases.
    Surgery called the Weaver-Dunn procedure reduces the clavicle to the acromion.

    Hi Dr , many thanks for this great explanation. i have been suffering from shoulder pain for last 15 yrs and had many treatmts but only to get a temporary relief, After looking at your great videos, i now suspect what might be the cause of the pain. something heavy fell on my shoulder clavicle long ago( at same time heard a sort of crack noise..no fracture identified. so i suspect its a AC inferior joint dislocation or tendon damaged, what test should help to confirm this plse?thx in adv

    I had a friend who did his AC over jumping a sand dune on his motorbike. Didn't tear it though, just stretched it ridiculously. He also had posterior dislocation of his humerus.

    Great vid. AC Joint separation II grade left untreated can be fixed after 4 years ? How much could cost a chirurgic operation ...approximately ? Il appreciate any suggestion . Thank you !

    i have the worst prob i have back pain where it is at an its been a huge bumb like when it first happen an this was about 6 to 5 months ago shouldnt my shoulder have healed by now an should i get surgery ever since i had this happen to me my uper back has been in pain an dont really know what to do at this point an i cant lift heavy things over my head it hurt or my shoulder goes out any advice please..

    yeah had it. terrible at first very painful and unable to move much at all. but it has healed but i am where i started. it was UNSUCCESSFUL!! my surgeon only repaired the bottom ligament which seems useless to me. i'd say just be very specific with your surgeon and make sure he's going to repair BOTH torn ligaments.

    Thanks for the Opportunity to learn from this media. I think you could improve the sound clarity and consistency by using a good pair of a phone Headset. otherwise the content is accurate i find and I would recommend it.

    It's been about 2 years since i had AC separation. And had several doctors to see my injury. Orthopedic, 2 Physical therapists (for rehab), Family doctor, Trauma and pain specialist DR. Mancao. Had a relief though, but still enduring pain right now, 2 of my doctor advised to build muscle specifically to the injured area. But my hypertension makes it difficult. My 2 theraphist don't want me to intake pain reliever drugs.

    What stretches do you use? I've had this for about 6 months and it started off healing really well but then it just stopped healing. I would be grateful for any advice :)

    I put an arm-bar on a guy in Jujitsu training. He struggled, using his shoulder to try to pull away. As he slowly was turned to his back, he fell and this shoulder separation happened. Luckily it was only the mildest form, called a sprain here. He says he'll be able to work out again soon.

    i fell on my shoulder and went to a after hours clinic. xray showed a significant shoulder separation with the clavicle facing up. i was given a sling and told to ice it. i keep having to push my clavicle down and forward into place because it keeps wanting to pop backwards. how do you know when its in the right place?
    I cants seem to be able to get an appointment with an orthopedist. Is there a better brace than the typical arm sling?

    Hi when I am in neutral position then... My clavicle is in the right spot. But if I do the test my clavicle comes out... What class would this be? Potentially

    QUESTION! I have some sort of AC joint problem I'm pretty sure its just a sprain not to serious but it happened when i was doing over head dumbell presses and now my shoulder is injured.. theres litterally 0 pain, no pain at all in the shoulder it just pops frequently and saggs out of place i dont know what to do

    I have been to several different doctors all of witch have given me a different diagnosis. In your professional opinion what would be a clue to wear the injury was if I the patient were to lay on my left side on a table and use my right arm and fully extended from my body as if to grab a cup and experienced pain in or around my right ac joint? Any help would be greatly appreciated due to the fact I have had this problem for four to five months now.

    if you have a small tear in your AC, do you need to get surgery or can you do a home-treatment? i can do everyday things, i can even exercise (although it does seem risky to do that), but i hear popping (if i do pushups) and i do feel my AC joint too.

    I just suffered a Grade 4/5 AC seperation. I have no connecting ligaments between my acroniom and clavicle. I'm considering the Weaver Dun or the LARS ligament. Which treatment will give me the best chance at a full recovery? I wish to weight lifting, including pressing exercises. Any other treatments i should look into?

    I hurt mine last year working out. Now I hurt the left one working out. Pretty sure its due to benching. The good news is that exercising with resistant bands helps the pain go away, but hurts like hell while doing it, and after it. Tuck your elbow in to your side and and move your arm left and right. It sounds very strange to do exercises while your shoulder hurts, but that's exactly what a doctor will tell you to do. Ice directly after and often and take anti inflammatory.

    If you sprain it what is the best treatment just to let it heal and not lift anything for awhile my doctor didn't do a X-ray or a MRI and I am in a great deal of pain still

    thanks Dr, problem is that i have seen at least 5doctors, had at least 5 Xrays, 3 MRI, 2 CT scans.they have not seen much apart from minor spine wear. that's what puzzling me. in the meantime , i have lost about 40% of my right arm capacity, and my muscles are slowly shrinking bcause uable to do much with it now..

    I had a type 2 separation and after three weeks have full mobility but little strength. Also I still have the bump, should I consider surgery or is the bump permanent?

    Thanks dr. Hawkins. I have seperation of the A/C joint and the clavicle is very prominent on my left side but I have NO pain and I can still do incline pushups :S Will I need surgery if it protrudes about 1cm out of my skin? I can put my arm across my body and its no tigher than the other arm. I'm gutted that I might have to stop my weight training, as I'm a skinny guy but I've made so much progress... to see it go down the drain to end up in a worse place is a kick in the teeth!

    Wow! I was told doctors debate on how to do this type of surgery, that is why some doctors advise not to at a grade 3... Thanks for the reply, I'm actually thinking hard about it now because I don't mind living with the hump, I mean mine is not that noticeable but the movements are uncomfortable... Thanks again...

    Have you gotten the surgery done? I got a grade 3 as well for about 2 6 weeks now and I was told by a couple of doctors that I don't need the surgery... Can you please reply? I want to know if I should or shouldn't get it.

    I see my specialist on Thursday after 2 weeks of having the injury. I can move my shoulder almost fully but I feel a "hole" where my AC joint was. When I first had the injury, there was a bump in that area. Is this normal?

    I've separated both of my shoulders. The left one was from a car accident and it was minor and I didn't really feel it at all. The right one was from a snowboarding accident and it was major. It hurt like hell and my arm was useless for weeks. My collar bone was sticking up a lot! I had surgery done. They shaved the end of the collar bone, put in a cadaver tendon, and two teflon screws.

    @nabilebraheim They all say its okay, there's no pain and its not loose. My doctor agrees with my friend (who's a physiotherapist) in saying its most likely a muscle imbalance, where one side is stronger than the other. I do agree that everyone with a boney bump like this should see a doctor, but I'm also saying that they shouldn't worry in the meantime if its not painful, because it mightn't be A/C shoulder separation, even if everything and everyone is pointing to that.

    Thanks for posting. We hope more emergency MDs will learn from physical therapists and use sports tape to stabilize the shoulder along with a sling. If you had a partial or complete tear Level 2-3, how long did it take you to fully recover?

    Thanks. I have dealt with shoulder pain for years and my doctor thinks this is what it is. You did a very good job explaining it. How long is the arm in a sling? That's my only concern as I won't be able to work but the pain is at a point now to where it's effecting my work and exercise.

    And the other problem is.. I don't know when my AC separation occurred. I don't remember any traumatic event that may cause it. But my Ortho told me it could be "progressive". Is it possible?

    Sir I met with an accident. 30 days ago. I had a acromioclavicular sublexion and scapula fracture. Doc told me not to lift weight for life with my injured arm and also I am unfit for army. I am devastated. Plz help sir. Is he right ??? Should I never lift weight ?? No push-ups pull ups ?? Please help sir.

    wow. i'm getting surgery in the next few weeks. my doctor is actually kind of against it. i can't live with the hump though. and i'm afraid of deforming myself even worse. i can't give up sports. no insurance? see a dr dude.
