Hey thanks for the video, I'm a basketball player and found out today after an ultra sound I've partially teared my peroneal longus at the bottom of my ankle How long till I'm at 100%
I had surgery yrs ago and it never hurt. Years later stress fractured my fibula it healed my leg has hurt every since. I have had an MRI and still have fluid. It hurt to get pedicures and always feels tender. The more walking I do the more further up my leg the pain goes. Eventually it feels like walking on a sprang ankle.
Had a tri malleolus with open reduction surgical repair for a broken ankle in 2015. Im 66 and not terribly active, but it seems that my ankle perineal tendonitis on the injured ankle side is getting more painful, and as I take daily walks, i find more frequently walking in the neighborhood is painful because of the uneven surface with neighborhood driveways. working in my garden sloped areas also cause pain for days or weeks. Im wearing good sneakers, and orthotics, which I am ruining at a frustrating rate because sneakers and garden mud! Also going to the bathroom at night is difficult, really hard to get up and lace up sneakers to walk to the toilet in the night. I hobble there with a painful ankle and I know that isnt good. What can I do??

Yep I'm wondering now if the "plantar fasciitis" I had fifteen years ago and is now manifesting as this is all related (of course it is...not enough stretching while overtraining and working physical jobs where I'm on my feet all day). Remember to stretch folks
I get pain as I sleep and wake up limping and the outside of my lower leg is so painful I cannot let my leg up on my other leg to get my socks on or shoes tied. So anyway I start walking and it is very painful, but after about a half mile or so the pain starts to go away and after about an hour I can walk normally without any limping. No bruise, no swelling, I am very careful not to twist my foot as that would make pain. It has been about 5 days and finally less pain in the morning. But the pain is extremely bad if I don’t walk, walking around makes it feel better. I think I searched for answers or days before coming across videos like this. This video was very helpful. Keep up the good work
Thank you for sharing. I’ve obviously got a defect because I’ve had this issue for 20 years. The pain and intense burning when jogging or walking has me screaming in agony during and for a good 15 minutes after exercise. In desperation (and in between screams) I found this video and I’m confident that this is what I’ve got - some kind of chronic tendonitis. It’s about time I did something about this. I can’t take this anymore!
The same happened to me.After 15 min.running the intense pain made me stop. Now I ve improved a bit but sometimes it comes again.I am a supinator runner,so I guess the shoes,terrain,etc...is causes the stress
@@a.c.2211 hello. There has been an update to this! So….I finally did do something about this and spoke to a GP through my work’s insurance. Short story: he has referred me to a vascular surgeon due to a concern with veins and / or arteries. I said I was suffering from some kind of chronic issue with the tendon. He thought that this was more vascular…..specifically, that there wasn’t enough blood or oxygen getting to the veins, muscles or arteries and this was causing the unbearable pain. He was particularly interested in the pain going after rest (not typical with tendonitis which would be constant - even with light walking….and I only get this pain when power walking or jogging). So….now utterly confused and I’ll need to book in with the vascular surgeon to see if there some kind of blockage going on in the legs 😬 will update! Ps I do hope you’re doing OK and aren’t suffering too much pain. It’s really nasty, isn’t it?!

This was extremely helpful, I have ballet students that are dealing with this tendinitis. It has to do with bad training /movement patterns, using extreme external rotation from the feet rather then hips, coupled with weak core and rotators. I will implement these suggestions and again, thank you.
Wow! What an incredible video! You are so knowledgeable and very precise in your description of these muscles, tendons,possible injuries and ways to fix and help heal the problem. I tried watching other videos and none of them covered half the things you did. Alot of them only mainly covered the Peroneus Brevis. You covered it all! My problem is definitely the Peroneus Longus. I'm very sore there and it's hard to walk especially after sitting for some time. I'm a truck driver for beer and beverage company and work very long hours 12-14hr days. I know there's many things that you mentioned that could be contributing to my pain. I'm one of those supposed tough guys or lol dumb guys that put off seeing a doctor until the absolute last second. I'm self diagnosing or guessing. I think the problems started happening a few months after this Corona Virus started. I've always been a very active guy and even after work still go to the gym and find time to take the kids for walks or hikes. Well, since pandemic gym and hikes stopped not nearly as active as I used to be, I put on some weight,working way more hours since we're really short on drivers,probably wearing worn shoes with not very good arch support, I have high arches by the way and believe I started to develop plantar fasciitus in my left foot about 3 weeks ago. I've been taking Ibuprofen for pain and icing my foot which has helped but with these longer hours working faster, harder and feeling a bit out of shape I feel like it's getting worse. Now, for the past 3 days I've been feeling lots of pain in the Peroneus Longus on both legs but especially my left leg. Wondering if my plantar fasciitus is contributing to my pain along with all the other things I'm doing wrong. Well, thanks to your video I am going to buy the massage stick, buy better shoes, buy a good pair of insoles probably the Powerstep Pinnacle Maxx, buy the Powerlix Ankle brace compression support sleeve and try to lose some weight! Hopefully doing all that plus stretching, icing and Ibuprofen hopefully I can get better and avoid seeing the doctor. Again, thank you so much for this incredible video. I had surgery on my longus and brevis tendons 2 weeks ago. My brevis tendon was trashed. He did a tenodesis of my brevis to my longus tendon. He did say that the longus was damaged as well, but doable. I now have my first ever cast at 62 years old, since the stitches were removed last week. I look forward to rehab, and to get back to work in my metetarsal boots.
I fell last year. 3 drs later I found my dream Podiatrist. He’s with New Age Ankle and Foot surgery. I’ve been suffering for a year with pain and one day I was in for my other foot and I explain what was going on. Well 1 week later I was in a mri machine, 4 ruptured ligaments/tendons. Peroneal tendon snapped talk about pain. Achilles tendinitis and I can barley walk.. but I graduated college hardly able to walk. Surgery next week.

Found this video and this helped me narrow it down. I'm experiencing peroneal brevis pain. Ironically I am a new snowboarder and this is 100% my root cause along with the lack of good orthotics. I've always experienced what I called a shin splint along the outter side of my leg (below the calf muscle and above the ankle) whenever I ran a lot, walked a lot or played lots of sports. Has always been an off and on issue. All of my shoes on my right side foot also end up rolling outward as they age. Snowboarding got me so sore because my binding on my right leg was set to 15 degrees for a few weeks. Then I noticed i was getting more and more sore and that area was always hurting. So I changed the binding down to 9 degrees and IMMEDIATELY could tell the difference. I know that I will need to get some insoles for sure and see a foot doc. But I'm so glad I found this video to connect all the dots.
Great video and information. More information than my doctor gave me. Heading back for another peroneal tendon repair surgery. I had one 10 years ago, I guess I trashed that repair with working on my feet all day. I wear good shoes and have had orthotics in my shoes for years. I have chronic brevis tendonitis in the left foot. In the evening when I'm relaxing that tendon likes to spasm for no reason, its really painful . The doctor is thinking of removing the bad and previous repaired tendon and replacing it with a cadaver tendon.

I've ridden horses for 30 years, for both occupation and as a hobby. Over the past 5 years, I've had 2, going on 3 surgeries for perneal tendon tears. They're extremely painful. My first one was the brevis, then after getting my brevis better, I tore my longus.
What kind of doctor did you see? I've been down for 4.5 months, just got my new mri results back today, and still no healing. I'm ready to just cut the foot off at this point, and there seems to be zero game plan other than just wait longer.

Excellent video and great explanation. I am a runner suffering from reoccurring tendinosis at the Insertion of the Peroneus Longus for the last 2 years which commenced after a long trail run. I have tried complete rest (twice) for months at a time, physio, magnetotherapy, strengthening, massages, frying the area with needles, changing shoe types and running techniques but unfortunately, the dull throbbing burning pain keeps coming back at the insertion, especially after or during hills and when sensitive it is even sore to the touch. At 2:10 on the video, that muscle that bulges to the right of the area you are talking about -- outside of the two areas you mention -- that is where my pain is. Going on 4 years.

I’m dealing with this right now. I was compensating for gout in my right foot. When I started feeling the awkwardness on my left foot I thought it might have been gout also. But this feels more like an injury than gout. This article explains exactly what I’m dealing with in my left foot. I have been walking with my foot not setting completely on the ground and walking with my left leg kicked out for a couple of months now. This peroneous longus and peroneous brevis tendonitis sucks almost as much as gout.

Thank goodness for this video. This is the BEST video I've found so far. I'm 7 weeks into my recovery from a rolled ankle. My mri report states this type of tendonitis in my ankle. I'm 2 weeks in with physical therapy. I'm back to being bruised up again on this tendon in my ankle due to the physical therapy. My therapist also told me I have high arches. And 4 years ago I developed Extensor tendonitis in the same foot. My right foot can't catch a break. I desperately need to know how to avoid any more tendonitis in my foot. Can you suggest any other videos for me?

Dead lifts and kettlebell swings have taken their toll. I dont have pain when walking but when i bend over i can feel pain in Peroneus Brevis. Its tender to touch. Going to try massage as you suggested.
How about adhesions along it such as trigger points muscle knots whatever you want to call them. And also I have cramps in my calves at night really bad.. for a job I walk on concrete a lot for 8 hours around 15,000 steps a day. Currently foam rolling I'm trying to friction them out. Please help with your opinion thank you

I had a tear, was in a boot per my orthopedic surgeon’s order, went to physical therapy, and no one ever mentioned orthopedic inserts as a tool to help - guess what I’ll be looking into! Thanks!
Good video and explanation of this problem. Thanks! Love the massage stick! It might have been good to spend a little time on the PARTICULAR biomechanics involved in many cases of peroneal tendonitis- the foot and heel motion caused by oversupination. Not all problems are caused by "stiff calves" (it would be great if they were). If you over-supinate (under-pronate) your foot is rolling outward as you step and you're pulling on your peroneal tendons repeatedly. Correct me if I'm wrong here. Two of the products you recommend are consequently probably USELESS for many cases of peroneal tendonitis- (those caused by oversupination)- they certainly did nothing for mine. The Powerstep Pinnacle inserts are fine all-purpose inserts but they do nothing for you if your peroneal tendon problem is caused by oversupination. You need something to elevate the outside (lateral) edge of the heel and foot- and the Powerstep inserts don't do that. You will need a lateral wedge insert of some kind. Likewise, the pull-on Powerlix ankle brace is certainly easy to use, and might help for other causes of ankle or foot pain, but it does nothing for me. A truely adjustable brace (I use the Tri-Lok) is a pain to use but it actually DOES help peroneal tendonitis. Peroneal tendonitis is a particular problem that needs particular remedies- not just all-purpose inserts or braces.
You are absolutely right and if an evaluation was performed in person there would be very specific evaluation and treatment. This is a very general video and is absolutely not a diagnosis nor advice for any single person. There are so many variables that making specific videos impossible. For example what if there is hip or knee arthritis, or a tight hamstring, or a slipped disc, or a nerve injury, or a truck driving pressing the leg against the door 16hrs a day. There are many causes for sure.
That being said these are basic devices whose only goal is to limit extreme ranges of motion. The effect can vary depending on the cause. For example if I hit someone in the peroneal tendon with a hammer a brace may actually put more pressure on it causing more pain for sure. A medical boot may cause more pain as well!
Hi doc, thank you for this very informative video. Can this be caused by ankle overpronation, or is over pronation caused by this? The sport i do requires me to push on the ball of my foot a lot. Afterwards i would feel a stabbing pain at a specific point in the peroneal brevis area. Would you say this is most likely peroneal brevis tendonitis? Thank you
I watched the full video. I got here since im experiencing pain in my peroneal brevis part (based on the video description of the area), when I try to run/jog even with enough dynamic warmup + walking warm up, this part of my leg hurts bad to very bad, then after around some time, or an hour sometime it subsides then im able to run without pain. Im now using a asics gey kayano 30, during my first 2 runs with this shoe, the pain did not occur, but after that it gets triggered until now. I don't recall snaping anythin on my leg, neither having an injury. Im able to do bulgarian split squats even tho im 95kg in proper form. im very frustrated now, i wanted to persue running, even if it hurts in the first part of my run, I still wait for it to subside and run the last part of my workout. Im now ordering a massage stick as you recommend in this video.
Mine only hurts along the 5th metatarsal area. Would this be the same injury just pain in a different spot? I don't see any information in regards to muscles/tendons in that area. All I can find is fractures which my doctor says I do not have.
I have tightness and pain in both Peroneal muscles when I start to walk or exercise. Once my legs are warmed up it goes away. But before it’s warmed up it’s super painful and tight. Had this problem since I was in grade school. Help!!
Would a weakness in one or both of these tendons account for frequent spasms in the outer calf? (I have very weak ankles and a history of severe plantar pain)
I had to hobble off the rugby pitch at practice tonight because of this. I was confused because I could still walk, but depending on position of my foot it would force me to limp. Definitely in the Brevis region so thank you for insole suggestion and I should probably pick up that roller too...

If have this pain, is it possible for my ankle plate (orif on femur) to be pressing on the tendon? Can't move the foot outward, sharp pain from ankle bone into the foot. I alternate between race up brace, compression sock and orthodics in walking shoes. Had ultrasound a few days ago and await results. Dud not anticipate this long recovery.

@@19kcrypto Sorry to hear you're suffering so much. The only help I've found has been chiropractic or osteopathic treatment, they can usually advise on exercises too. Some people find a tens machine helpful, but they never worked for me - I thought it was about as effective as poking yourself in the eye to distract from a toothache!😂 For the most part, try to learn to pace yourself - and keep positive!
i tore my MCL a few weeks ago and for a long time my calf was also sore preventing me to do rehab practices, my physio started massaging my calf where i said it hurts and it started popping at certain parts. i did my own research and saw that it was this tendon being spoke of in the video. So my question is, Why does it pop when massaging and also why has not physio nor doctor even noticed this problem after I've told them multiple times my calf is the actual problem with my leg
For me the pain occurs on both legs, only on the peroneal brevis. I don’t have any pain on my ankle or foot. But it really only hurts after I run or sprint. It never hurts while running or sprinting. Any ideas?

So I bought a pair of trail running shoes the Altra Olympus $265 and these shoes are actually what caused this peroneal tendonitis and gave me plantar fasciitis in both feet. I couldn't believe it. These shoe were recommended to me after doing a bunch of research. They pulled down on the top of my foot, they pull on my achilles tendon and offered zero support for stability. I needed an extra wide toe box and i love the tread, but these shoes unfortunately are going to the trash. So sad. it's been 6 months now living with peroneal tendonitis and now plantar fasciitis. I've done physio therapy, at home strengthening rolling, i bought all the stuff. then i started getting shockwave therapy. Then i had a pediatrist watch me walk and she noticed i walk on the outside of my foot while rolling my ankles. Now i need to buy another set of new shoes.
My pain resulted from an ankle sprain a year ago. The pain is right where the brevis attaches to the fifth metatarsal. It ranges from from being sore to feeling like an ice pick is being driven into the site… so pain from 2-6 on a average. It’s especially painfully if I’ve been sitting and laying in bed and then get up. I’m fit, at appropriate weight etc. I cannot wear any shoe with an arch so orthotics and braces are out. I’ve got a road cycling shoe that I’ve cut out the area around the cuboid and that’s tolerable riding my trainer. Oddly enough riding my MTB with flat pedals and shoes with no arch support causes the least pain. My foot surgeon (works with our Olympic Training Center has recommended dry needling and possibly platelet replacement protocol. He believes that surgery is a last resort and has a low success rate. It has taken three months to get a PT appointment so this stuff better work. I’ve lost too much riding and at 58 I’m not getting any younger.
This is a great video. I don't know if I missed something, but my pain is excruciating where you have the long line drawn towards your pinky toe. I’m suffering terribly at the moment with this. I’ve been slamming NSAIDS, and now on oral steroid, but this is just a terrible pain and in such a critical place that even walking normally hurts, so I limp. With that, my back, SI joint, and calf on the affected side are sore as well. It’s been 2 weeks since I believe I injured it, (jogging followed by jump rope) and just doesn’t seem to be letting up in any significant way. I saw an ortho 2 days ago who put me in a long boot (which makes me walk lopsided and makes my back/hip worse), and prescribed me Valterin cream. So far, I don’t see much light at the end of the tunnel. My xrays were negative for fx. I’m not sure exactly which or if both are affected, but I sure could use some advice or encouragement. Such a bummer bc now I can’t run or even power walk - biking luckily doesn’t hurt.
I have pain in the 5th metatarsal right where it inserts I noticed if I rub and massage the area it releaves a good bit of the discomfort would the treatments listed in this video help that location

I was wondering if training and being more barefoot, working on "short foot" exercises to increase plantar fascial resilience and self myofascial release everyday to get those compacted, not mobile structures moving a bit better could work before hand? I mean insoles or footwear with "arch support" wouldn't fix the cause but the symptoms. Thanks for your input.
hey doc. thx for this... I have a question. I have seen 2 podiatrists over the past several months for some foot problems. I am a 67-yr old athlete. I have never had significant foot problems throughout decades of playing baseball, tennis, pickleball, and long distance running. However, in the last 4 years I have played competitive pickleball. As you probably know, this requires a lot of starting, turning, stopping, and moving. In the last 2 years the first painful event was my big toe. A podiatrist in my area identified hallus limitus and did a procedure to 'fix' my condition. After recuperating, I began playing again. In a short period of time, that same pain re-surfaced. In addition, on one day of pickleball, I ran too hard for a backhand shot and -- I thought -- straine my right foot. The result was pain on the bottom middle of my right foot. I tried to 'shake it off', keep playing ... hoping it would get better. It didn't. It got worse until I had another stressful event on the pickleball court. That pain was centered on the right bottom of my right foot. I had it x-rayed and mri'ed. The MRI was not super clear but the 2nd podiatrist said that it was a tear in the peroneal longus tenon. He couldn't tell if it was partial or full. So I am currently in a boot. Before this, I had great shoes and a great orthotic. No pain along the bottom of my leg. but both big toe pain and pain on the bottom, right side of foot. Thoughts?

@@sl2608 As it turns out, the 'tear' in the tendon was actually a complete rupture. The tendon which is supposed to go down the leg, under the foot, and attach to the big toe was pulled apart; it did not heal; like a rubber band, it recoiled and 'died' in the bottom, right side of my foot. I am now running and playing w/o pain. The doc said that surgery was out of the question. At 69 (now) the real 'need' for that tendon has gone away. So... no surgery. Doing well. Thx for the question!

@@sl2608 No pain. It has been some time since my surgeon looked at it and gave the verdict, "no surgery". Maybe it was because of my age (69 at this writing). The tendon tore and relocated on the bottom, right side of my foot. It was not so much painful as it was -- simply uncomfortable. But over time the discomfort went away. I'm very glad to have avoided the 'surgery bullet.
So i have pain on the outside from a run. When I dont run it doesnt pain but when I start running it hurts. Therefore i think its a tear. I dont have the ability to see a podiatrist. If it is a tear can I do slow gentle jogs to strengthen it? Will it aaalways hurt at that point?
@DrTomBiernacki When I run or do certain crossfit exercises my feet start to ache on the outside. However, if I lift them up and shake them out for a few seconds the pain mostly subsides until I begin again and it slowly starts to build. Would this be peroneal tendonitis or would it be cuboid syndrome?

Omg thank you , I’ve been out of shape for so long that , walking for extended periods of time really hurts. And it’s because of the inflammation in the brevis. But I’m wondering if because i have an anterior pelvic tilt. Is that causing the issue?

Great video, any advice/suggestion for suspected custom orthotic causing peroneal tendinopathy? Is the device too controlling? Causing too much supination (I don't believe it is though) or would I benefit from additional lateral wedging/cuboid notch addition? Thanks
Its hard to say without evaluation, it could be many things and you can be tight or off balance in many areas. Generally we start with evaluating symmetry and starting with softer less aggressive insoles and generally move to more and more correction to assess what a patient can handle.
Hi did your tendinopathy clear up? My physio believes mine is currently caused by orthotics offsetting my foot and putting strain on the peroneals. Corrections to orthotics have not yielded results yet. Considering lateral wedging too
Insoles cause my problems. Also what other body part are you in a brace for life? This is not a long term solution. You even said you muscles work less in an insole. That’s a good thing? Goal should be to strengthen the foot and get away from things that weaken the foot.
My favorite thing is stabilizing the foot initially so that it can get relief, then #2 diagnosing why it is overworks, #1 accomplished with shoes, insoles and bracing, #2 usually a biomechanical exam, of course some imaging initially 😁
So with a shallow peroneal tendon groove, I’m getting subluxation without the popping, because there’s nothing for it to pop over? This is maybe why an ultrasound didn’t catch it? I’d really love to see an animation or one of your plastic models (no cadavers if at all possible) of the peroneal tendons going through their range of motion. I’m not clear on what I really am okay and not okay to do. My Dr just said no tip toes (so I guess ballet is out) and no running or jumping, ever again. But even just walking at a pace quick enough to work up a sweat triggers the pain. Having this pain for nearly 20 years now has caused so much weight gain because I can’t do anything anymore. I was in great shape when it started, so it’s another really irritating part of this. Thoughts? And do you have any additional visuals on how the peroneal tendons move? Thanks!
Hello, I've experienced a slight pain on the lateral side of my ankle since a few months , a little bit under the area of the peronal tendons . It feels ok while walking but standing still makes me feel discomfort. This happened after I did a lateral jump in stairs. I didn't twist my ankle or anything, but I could feel that little pain right at the reception and it didn't go away since. Any idea where that could come from? Could it be from the peroneals or instrinsic muscles? Best regards

I can walk without pain but my ankle is bothering me with pain during rest and sometimes dur to tightness there is limitation in doresflextion …I did physiotherapy since 6 months.. i feel pain in bone and down to ankle balls? Not specific to you but most of the time the pain is only aggravated while playing basketball. We find usually good shoes and good insoles while not playing works really well and you are less sore going into the game. These times of problems are usually aggravated in the 95% of the time you are outside of playing sports
We find orthotics and braces are not always practical during the sport, but most people go into the sport already sore and injured from all the other time they could have been protecting and healing it.

Hey thanks for the video, I'm a basketball player and found out today after an ultra sound I've partially teared my peroneal longus at the bottom of my ankle How long till I'm at 100%
I had surgery yrs ago and it never hurt. Years later stress fractured my fibula it healed my leg has hurt every since. I have had an MRI and still have fluid. It hurt to get pedicures and always feels tender. The more walking I do the more further up my leg the pain goes. Eventually it feels like walking on a sprang ankle.
Had a tri malleolus with open reduction surgical repair for a broken ankle in 2015. Im 66 and not terribly active, but it seems that my ankle perineal tendonitis on the injured ankle side is getting more painful, and as I take daily walks, i find more frequently walking in the neighborhood is painful because of the uneven surface with neighborhood driveways. working in my garden sloped areas also cause pain for days or weeks. Im wearing good sneakers, and orthotics, which I am ruining at a frustrating rate because sneakers and garden mud! Also going to the bathroom at night is difficult, really hard to get up and lace up sneakers to walk to the toilet in the night. I hobble there with a painful ankle and I know that isnt good. What can I do??
My MRI results came back with a longitudinal tear of the longus tendon as well as a tear in the spring tendon. Does this require surgery?

I get pain as I sleep and wake up limping and the outside of my lower leg is so painful I cannot let my leg up on my other leg to get my socks on or shoes tied. So anyway I start walking and it is very painful, but after about a half mile or so the pain starts to go away and after about an hour I can walk normally without any limping. No bruise, no swelling, I am very careful not to twist my foot as that would make pain. It has been about 5 days and finally less pain in the morning. But the pain is extremely bad if I don’t walk, walking around makes it feel better. I think I searched for answers or days before coming across videos like this. This video was very helpful. Thank you for sharing. I’ve obviously got a defect because I’ve had this issue for 20 years. The pain and intense burning when jogging or walking has me screaming in agony during and for a good 15 minutes after exercise. In desperation (and in between screams) I found this video and I’m confident that this is what I’ve got - some kind of chronic tendonitis. It’s about time I did something about this. I can’t take this anymore!

@@a.c.2211 hello. There has been an update to this! So….I finally did do something about this and spoke to a GP through my work’s insurance. Short story: he has referred me to a vascular surgeon due to a concern with veins and / or arteries. I said I was suffering from some kind of chronic issue with the tendon. He thought that this was more vascular…..specifically, that there wasn’t enough blood or oxygen getting to the veins, muscles or arteries and this was causing the unbearable pain. He was particularly interested in the pain going after rest (not typical with tendonitis which would be constant - even with light walking….and I only get this pain when power walking or jogging). So….now utterly confused and I’ll need to book in with the vascular surgeon to see if there some kind of blockage going on in the legs 😬 will update! Ps I do hope you’re doing OK and aren’t suffering too much pain. It’s really nasty, isn’t it?!

This was extremely helpful, I have ballet students that are dealing with this tendinitis. It has to do with bad training /movement patterns, using extreme external rotation from the feet rather then hips, coupled with weak core and rotators. I will implement these suggestions and again, thank you.

Wow! What an incredible video! You are so knowledgeable and very precise in your description of these muscles, tendons,possible injuries and ways to fix and help heal the problem. I tried watching other videos and none of them covered half the things you did. Alot of them only mainly covered the Peroneus Brevis. You covered it all! My problem is definitely the Peroneus Longus. I'm very sore there and it's hard to walk especially after sitting for some time. I'm a truck driver for beer and beverage company and work very long hours 12-14hr days. I know there's many things that you mentioned that could be contributing to my pain. I'm one of those supposed tough guys or lol dumb guys that put off seeing a doctor until the absolute last second. I'm self diagnosing or guessing. I think the problems started happening a few months after this Corona Virus started. I've always been a very active guy and even after work still go to the gym and find time to take the kids for walks or hikes. Well, since pandemic gym and hikes stopped not nearly as active as I used to be, I put on some weight,working way more hours since we're really short on drivers,probably wearing worn shoes with not very good arch support, I have high arches by the way and believe I started to develop plantar fasciitus in my left foot about 3 weeks ago. I've been taking Ibuprofen for pain and icing my foot which has helped but with these longer hours working faster, harder and feeling a bit out of shape I feel like it's getting worse. Now, for the past 3 days I've been feeling lots of pain in the Peroneus Longus on both legs but especially my left leg. Wondering if my plantar fasciitus is contributing to my pain along with all the other things I'm doing wrong. Well, thanks to your video I am going to buy the massage stick, buy better shoes, buy a good pair of insoles probably the Powerstep Pinnacle Maxx, buy the Powerlix Ankle brace compression support sleeve and try to lose some weight! Hopefully doing all that plus stretching, icing and Ibuprofen hopefully I can get better and avoid seeing the doctor. Again, thank you so much for this incredible video. I had surgery on my longus and brevis tendons 2 weeks ago. My brevis tendon was trashed. He did a tenodesis of my brevis to my longus tendon. He did say that the longus was damaged as well, but doable. I now have my first ever cast at 62 years old, since the stitches were removed last week. I look forward to rehab, and to get back to work in my metetarsal boots.
I fell last year. 3 drs later I found my dream Podiatrist. He’s with New Age Ankle and Foot surgery. I’ve been suffering for a year with pain and one day I was in for my other foot and I explain what was going on. Well 1 week later I was in a mri machine, 4 ruptured ligaments/tendons. Peroneal tendon snapped talk about pain. Achilles tendinitis and I can barley walk.. but I graduated college hardly able to walk. Surgery next week.

Found this video and this helped me narrow it down. I'm experiencing peroneal brevis pain. Ironically I am a new snowboarder and this is 100% my root cause along with the lack of good orthotics. I've always experienced what I called a shin splint along the outter side of my leg (below the calf muscle and above the ankle) whenever I ran a lot, walked a lot or played lots of sports. Has always been an off and on issue. All of my shoes on my right side foot also end up rolling outward as they age.
Snowboarding got me so sore because my binding on my right leg was set to 15 degrees for a few weeks. Then I noticed i was getting more and more sore and that area was always hurting. So I changed the binding down to 9 degrees and IMMEDIATELY could tell the difference.
I know that I will need to get some insoles for sure and see a foot doc. But I'm so glad I found this video to connect all the dots.
Great video and information. More information than my doctor gave me. Heading back for another peroneal tendon repair surgery. I had one 10 years ago, I guess I trashed that repair with working on my feet all day. I wear good shoes and have had orthotics in my shoes for years. I have chronic brevis tendonitis in the left foot. In the evening when I'm relaxing that tendon likes to spasm for no reason, its really painful . The doctor is thinking of removing the bad and previous repaired tendon and replacing it with a cadaver tendon.

I've ridden horses for 30 years, for both occupation and as a hobby. Over the past 5 years, I've had 2, going on 3 surgeries for perneal tendon tears. They're extremely painful. My first one was the brevis, then after getting my brevis better, I tore my longus.
What kind of doctor did you see? I've been down for 4.5 months, just got my new mri results back today, and still no healing. I'm ready to just cut the foot off at this point, and there seems to be zero game plan other than just wait longer.

Excellent video and great explanation. I am a runner suffering from reoccurring tendinosis at the Insertion of the Peroneus Longus for the last 2 years which commenced after a long trail run. I have tried complete rest (twice) for months at a time, physio, magnetotherapy, strengthening, massages, frying the area with needles, changing shoe types and running techniques but unfortunately, the dull throbbing burning pain keeps coming back at the insertion, especially after or during hills and when sensitive it is even sore to the touch. Any additional advice relating to relieving pressure or pain at the insertion would be appreciated! Thanks again for the great videos and great explanations

I’m dealing with this right now. I was compensating for gout in my right foot. When I started feeling the awkwardness on my left foot I thought it might have been gout also. But this feels more like an injury than gout. This article explains exactly what I’m dealing with in my left foot. I have been walking with my foot not setting completely on the ground and walking with my left leg kicked out for a couple of months now. Thank goodness for this video. This is the BEST video I've found so far. I'm 7 weeks into my recovery from a rolled ankle. My mri report states this type of tendonitis in my ankle. I'm 2 weeks in with physical therapy. I'm back to being bruised up again on this tendon in my ankle due to the physical therapy. My therapist also told me I have high arches. And 4 years ago I developed Extensor tendonitis in the same foot. My right foot can't catch a break. I desperately need to know how to avoid any more tendonitis in my foot. Dead lifts and kettlebell swings have taken their toll. I dont have pain when walking but when i bend over i can feel pain in Peroneus Brevis. Its tender to touch. Going to try massage as you suggested.

It might have been good to spend a little time on the PARTICULAR biomechanics involved in many cases of peroneal tendonitis- the foot and heel motion caused by oversupination. Not all problems are caused by "stiff calves" (it would be great if they were). If you over-supinate (under-pronate) your foot is rolling outward as you step and you're pulling on your peroneal tendons repeatedly. Correct me if I'm wrong here.
Two of the products you recommend are consequently probably USELESS for many cases of peroneal tendonitis- (those caused by oversupination)- they certainly did nothing for mine. The Powerstep Pinnacle inserts are fine all-purpose inserts but they do nothing for you if your peroneal tendon problem is caused by oversupination. You need something to elevate the outside (lateral) edge of the heel and foot- and the Powerstep inserts don't do that. You will need a lateral wedge insert of some kind.
Likewise, the pull-on Powerlix ankle brace is certainly easy to use, and might help for other causes of ankle or foot pain, but it does nothing for me. A truely adjustable brace (I use the Tri-Lok) is a pain to use but it actually DOES help peroneal tendonitis.
Peroneal tendonitis is a particular problem that needs particular remedies- not just all-purpose inserts or braces.
You are absolutely right and if an evaluation was performed in person there would be very specific evaluation and treatment. This is a very general video and is absolutely not a diagnosis nor advice for any single person. There are so many variables that making specific videos impossible. For example what if there is hip or knee arthritis, or a tight hamstring, or a slipped disc, or a nerve injury, or a truck driving pressing the leg against the door 16hrs a day. There are many causes for sure.

That being said these are basic devices whose only goal is to limit extreme ranges of motion. The effect can vary depending on the cause. For example if I hit someone in the peroneal tendon with a hammer a brace may actually put more pressure on it causing more pain for sure. A medical boot may cause more pain as well!

I watched the full video. I got here since im experiencing pain in my peroneal brevis part (based on the video description of the area), when I try to run/jog even with enough dynamic warmup + walking warm up, this part of my leg hurts bad to very bad, then after around some time, or an hour sometime it subsides then im able to run without pain. Im now using a asics gey kayano 30, during my first 2 runs with this shoe, the pain did not occur, but after that it gets triggered until now. I don't recall snaping anythin on my leg, neither having an injury. Im able to do bulgarian split squats even tho im 95kg in proper form. im very frustrated now, i wanted to persue running, even if it hurts in the first part of my run, I still wait for it to subside and run the last part of my workout. Im now ordering a massage stick as you recommend in this video.

Mine only hurts along the 5th metatarsal area. Would this be the same injury just pain in a different spot? I don't see any information in regards to muscles/tendons in that area. All I can find is fractures which my doctor says I do not have.
I have tightness and pain in both Peroneal muscles when I start to walk or exercise. Once my legs are warmed up it goes away. But before it’s warmed up it’s super painful and tight. Had this problem since I was in grade school. Help!!

The Pin Muscle Group. Peroneus Brevis can pull tuberosity of fifth metatarsal away in a foot rolling event. The fibula side of the ankle joint.

I had to hobble off the rugby pitch at practice tonight because of this. I was confused because I could still walk, but depending on position of my foot it would force me to limp. Definitely in the Brevis region so thank you for insole suggestion and I should probably pick up that roller too...

If have this pain, is it possible for my ankle plate (orif on femur) to be pressing on the tendon? Can't move the foot outward, sharp pain from ankle bone into the foot. I alternate between race up brace, compression sock and orthodics in walking shoes. Had ultrasound a few days ago and await results. Dud not anticipate this long recovery.

Started 26 years ago with a back injury that left me with a limp; 4 years ago developed sharp heel pain after standing baking for a party for several days; 2 years ago slipped down a step twisted my ankle - similar to the side of foot pain that's bugging me now.

@@19kcrypto Sorry to hear you're suffering so much. The only help I've found has been chiropractic or osteopathic treatment, they can usually advise on exercises too. Some people find a tens machine helpful, but they never worked for me - I thought it was about as effective as poking yourself in the eye to distract from a toothache!😂

i tore my MCL a few weeks ago and for a long time my calf was also sore preventing me to do rehab practices, my physio started massaging my calf where i said it hurts and it started popping at certain parts. i did my own research and saw that it was this tendon being spoke of in the video. So my question is, Why does it pop when massaging and also why has not physio nor doctor even noticed this problem after I've told them multiple times my calf is the actual problem with my leg
For me the pain occurs on both legs, only on the peroneal brevis. I don’t have any pain on my ankle or foot.
But it really only hurts after I run or sprint.
It never hurts while running or sprinting.
Any ideas?

Thank you for explaining there peroneus stuff. I began to fight my Corona overweight and found out that long evening walks were my thing. I guess my leg and foot were thinking: "hey what are you doing here?! Stop!" :) I now do excersizes and found myself some decent running shoes :)

So I bought a pair of trail running shoes the Altra Olympus $265 and these shoes are actually what caused this peroneal tendonitis and gave me plantar fasciitis in both feet. I couldn't believe it. These shoe were recommended to me after doing a bunch of research. They pulled down on the top of my foot, they pull on my achilles tendon and offered zero support for stability. I needed an extra wide toe box and i love the tread, but these shoes unfortunately are going to the trash. So sad. it's been 6 months now living with peroneal tendonitis and now plantar fasciitis. I've done physio therapy, at home strengthening rolling, i bought all the stuff. then i started getting shockwave therapy. Then i had a pediatrist watch me walk and she noticed i walk on the outside of my foot while rolling my ankles. Now i need to buy another set of new shoes.
My pain resulted from an ankle sprain a year ago. The pain is right where the brevis attaches to the fifth metatarsal. It ranges from from being sore to feeling like an ice pick is being driven into the site… so pain from 2-6 on a average. It’s especially painfully if I’ve been sitting and laying in bed and then get up.
I’m fit, at appropriate weight etc. I cannot wear any shoe with an arch so orthotics and braces are out. I’ve got a road cycling shoe that I’ve cut out the area around the cuboid and that’s tolerable riding my trainer. Oddly enough riding my MTB with flat pedals and shoes with no arch support causes the least pain.
My foot surgeon (works with our Olympic Training Center has recommended dry needling and possibly platelet replacement protocol. He believes that surgery is a last resort and has a low success rate. It has taken three months to get a PT appointment so this stuff better work. I’ve lost too much riding and at 58 I’m not getting any younger.
This is a great video. I don't know if I missed something, but my pain is excruciating where you have the long line drawn towards your pinky toe. It's on the side and it literally hurts me every second of the day. Can you please tell me what that is called? Or anyone else in the comments thank you guys 😊

I’m suffering terribly at the moment with this. I’ve been slamming NSAIDS, and now on oral steroid, but this is just a terrible pain and in such a critical place that even walking normally hurts, so I limp. With that, my back, SI joint, and calf on the affected side are sore as well. It’s been 2 weeks since I believe I injured it, (jogging followed by jump rope) and just doesn’t seem to be letting up in any significant way. I saw an ortho 2 days ago who put me in a long boot (which makes me walk lopsided and makes my back/hip worse), and prescribed me Valterin cream. So far, I don’t see much light at the end of the tunnel. My xrays were negative for fx. I’m not sure exactly which or if both are affected, but I sure could use some advice or encouragement. Such a bummer bc now I can’t run or even power walk - biking luckily doesn’t hurt.

I have pain in the 5th metatarsal right where it inserts I noticed if I rub and massage the area it releaves a good bit of the discomfort would the treatments listed in this video help that location
We would love to help you with your foot and ankle problem, but a complex problem like this really does require in depth evaluation and imaging to make a proper diagnosis. We wish we could do more for you!
We would be doing you a disservice with general advice for such an important issue. Your podiatrist or primary care doctor can likely help much more than we can in this instance.

I was wondering if training and being more barefoot, working on "short foot" exercises to increase plantar fascial resilience and self myofascial release everyday to get those compacted, not mobile structures moving a bit better could work before hand? I mean insoles or footwear with "arch support" wouldn't fix the cause but the symptoms. Thanks for your input.
hey doc. thx for this... I have a question. I have seen 2 podiatrists over the past several months for some foot problems. I am a 67-yr old athlete. I have never had significant foot problems throughout decades of playing baseball, tennis, pickleball, and long distance running. However, in the last 4 years I have played competitive pickleball. As you probably know, this requires a lot of starting, turning, stopping, and moving. In the last 2 years the first painful event was my big toe. A podiatrist in my area identified hallus limitus and did a procedure to 'fix' my condition. After recuperating, I began playing again. In a short period of time, that same pain re-surfaced. In addition, on one day of pickleball, I ran too hard for a backhand shot and -- I thought -- straine my right foot. The result was pain on the bottom middle of my right foot. I tried to 'shake it off', keep playing ... hoping it would get better. It didn't. It got worse until I had another stressful event on the pickleball court. That pain was centered on the right bottom of my right foot. I had it x-rayed and mri'ed. The MRI was not super clear but the 2nd podiatrist said that it was a tear in the peroneal longus tenon. He couldn't tell if it was partial or full. So I am currently in a boot. Before this, I had great shoes and a great orthotic. No pain along the bottom of my leg. but both big toe pain and pain on the bottom, right side of foot. Thoughts?
Will you be able to treat and heal from this without surgery? Hope so. Would be interested in knowing how you are dong now, a year after you posted your comment.
@@sl2608 As it turns out, the 'tear' in the tendon was actually a complete rupture. The tendon which is supposed to go down the leg, under the foot, and attach to the big toe was pulled apart; it did not heal; like a rubber band, it recoiled and 'died' in the bottom, right side of my foot. I am now running and playing w/o pain. The doc said that surgery was out of the question. At 69 (now) the real 'need' for that tendon has gone away. So... no surgery. Doing well. Thx for the question!

@@sl2608 No pain. It has been some time since my surgeon looked at it and gave the verdict, "no surgery". Maybe it was because of my age (69 at this writing). The tendon tore and relocated on the bottom, right side of my foot. It was not so much painful as it was -- simply uncomfortable. But over time the discomfort went away. I'm very glad to have avoided the 'surgery bullet.

So i have pain on the outside from a run. When I dont run it doesnt pain but when I start running it hurts. Therefore i think its a tear. I dont have the ability to see a podiatrist. If it is a tear can I do slow gentle jogs to strengthen it? Will it aaalways hurt at that point?
Peroneous longus inserts into the base of fifth or first metatarsal doctor?
Brevis is the 5th metarsal and longus is the 1st metatarsal base.
What does it mean if you massage the area and it starts to swell?
@DrTomBiernacki When I run or do certain crossfit exercises my feet start to ache on the outside. However, if I lift them up and shake them out for a few seconds the pain mostly subsides until I begin again and it slowly starts to build. Would this be peroneal tendonitis or would it be cuboid syndrome?

Probably tight/sore/stiff glutes/hamstring/calf leading to overpronation and overload. Usually, just pressure overload.

I have anterior tibialis pain in front of shins coming up to insertion at knee. Don’t know what to do about it.
Omg thank you , I’ve been out of shape for so long that , walking for extended periods of time really hurts. And it’s because of the inflammation in the brevis. But I’m wondering if because i have an anterior pelvic tilt. Is that causing the issue?

It is not likely that pelvic tilt has any connection with inflammation. However, it is impossible to know without in person examination. Great video, any advice/suggestion for suspected custom orthotic causing peroneal tendinopathy? Is the device too controlling? Causing too much supination (I don't believe it is though) or would I benefit from additional lateral wedging/cuboid notch addition? Thanks
Its hard to say without evaluation, it could be many things and you can be tight or off balance in many areas. Generally we start with evaluating symmetry and starting with softer less aggressive insoles and generally move to more and more correction to assess what a patient can handle.
Hi did your tendinopathy clear up? My physio believes mine is currently caused by orthotics offsetting my foot and putting strain on the peroneals. Corrections to orthotics have not yielded results yet. Considering lateral wedging too
Insoles cause my problems. Also what other body part are you in a brace for life? This is not a long term solution. You even said you muscles work less in an insole. That’s a good thing? Goal should be to strengthen the foot and get away from things that weaken the foot.
is this the same ting as a peroneal strain?
I have a 3 quarter custom orthotic. Do you recommend putting that orthotic on top of the regular insole or no?
3/4 is great but usually a full length provides more support. I say 3/4 gives medium support but a stronger full length can give even more.
For some people the 3/4 is enough, some need a full length that is more aggressive and even other may need a brace + a full length insole or more
@@DrTomBiernacki ok good to know. I wear these custom 3/4 orthotics in my soccer cleats as well. I put them on top of my regular soccer cleats inserts
Is there anything one can do when tendon outstretched (not inflamed)?
My favorite thing is stabilizing the foot initially so that it can get relief, then #2 diagnosing why it is overworks, #1 accomplished with shoes, insoles and bracing, #2 usually a biomechanical exam, of course some imaging initially 😁

So with a shallow peroneal tendon groove, I’m getting subluxation without the popping, because there’s nothing for it to pop over? This is maybe why an ultrasound didn’t catch it? I’d really love to see an animation or one of your plastic models (no cadavers if at all possible) of the peroneal tendons going through their range of motion. I’m not clear on what I really am okay and not okay to do. My Dr just said no tip toes (so I guess ballet is out) and no running or jumping, ever again. But even just walking at a pace quick enough to work up a sweat triggers the pain. Having this pain for nearly 20 years now has caused so much weight gain because I can’t do anything anymore. I was in great shape when it started, so it’s another really irritating part of this. Thoughts? And do you have any additional visuals on how the peroneal tendons move? Thanks!

Hello, I've experienced a slight pain on the lateral side of my ankle since a few months , a little bit under the area of the peronal tendons . It feels ok while walking but standing still makes me feel discomfort. This happened after I did a lateral jump in stairs. I didn't twist my ankle or anything, but I could feel that little pain right at the reception and it didn't go away since.
Any idea where that could come from? Could it be from the peroneals or instrinsic muscles? Best regards

I can walk without pain but my ankle is bothering me with pain during rest and sometimes dur to tightness there is limitation in doresflextion …I did physiotherapy since 6 months.. i feel pain in bone and down to ankle balls? What do you suggest me to do? Do i have peroneal tendenities?
I get this pain after dunks sometimes but its not bad, do you recommend wearing hightop shoes or wrapping the specific area?
Not specific to you but most of the time the pain is only aggravated while playing basketball. We find usually good shoes and good insoles while not playing works really well and you are less sore going into the game. These times of problems are usually aggravated in the 95% of the time you are outside of playing sports
We find orthotics and braces are not always practical during the sport, but most people go into the sport already sore and injured from all the other time they could have been protecting and healing it.
@@DrTomBiernacki it never hurts going into practice, it starts about 30 minutes into practice when i start to get warm, and on game days i never feel it because of Adrilyn
