    This is an important video. With even a few years of experience, any dentist who has been involved in orthodontic treatment is nearly certain to have had an encounter with "dreaded black triangles". REALISTIC EXPECTATIONS are essential. "Wishful thinking", on the other hand, is a prescription for regret. For the doctor, patient, and, in many cases including the patient's friends, exceptionally c.l.e.a.r. communication is critical to achieving good outcomes. Prior to the start of treatment, showing patients real clinical photos of "black triangles" sets the stage for good patient/doctor experiences.

      Our mentor, the one and only Gaffer, strikes again with words of wisdom! Dr. Samson is the first one we heard saying: If you see it before it's a diagnostic. Everything you see after is an excuse!

      I completely agree Dr. Samson. Any time there is overlap of anterior teeth during the initial exam, even in a 12 year old, I explain that it is a possibility. For older patients, I explain that it is a near certainty. And of course....these are the patients who always seem to have tall narrow incisors for whom IPR would be contraindicated.
      My consultations tend to be a little lengthier than most and have some “downers” like these explanations. But, I still remember the words of my Ortho dept chairman at Nova, Dr Malcolm Meister: “If you say it ahead of time, it is an explanation; if you say it afterwards, it is an excuse!” I don’t know if you met him when you visited Nova and brought your Big Tooth! I was there from 2002-4. Maybe he heard those words from you.

    I'm using invisaling , and now I see dark triangles in my teeth and I know is because of the shape of my teeth , I used normal braces for more 56 months I did the aliners but my problem was my dentists did not tell me I needed to use nigh aliners and now I have to use invisiling I hope my new doctor can help me because I don't want to see a dark triangle in my teeth. This was a bad surprise for me

    Black triangles really came on during my invisalgin treatment. I feel worse about my smile now than before Orthodontic treatment. What should I do? I brushed multiple times per day and really tried to keep my aligners clean. I feel sick.

      We unfortunately cannot evaluate your situation. We are sorry to hear that. We suggest you talk about it with your dentist/orthodontist. Sometimes these spaces appear during treatment and they disappear with time. But every case is different. There are ways to make them "disappear" as well, or camouflage them. We wish for the best in your treatment.
