For example, ek aircraft ke liye bhi har situation, including engine failure and hydraulic pressure loss ke liye bhi training and handbooks diye jaate hai, taaki pilots har ek emergency ko handle kar sake. Aur yahan ek system failure, jahan pe procedures bohot simple hai (clearance confirm karna, speed kam rakhna, aur har signal pe stop hoke check kar lena), uss ek system ko train aur memorize nahi kar paye hain in the full sense.

Loco pillots , guards and station master all are required to pass refresher training evry two to three years. And automatic singals are not new system it is age old system. Everyone has the knowledge about its working rules. But mistakes happen. The solution is- Railway should reduce the human intervention as much as possible, an use the latest technology.

Rigorous training and updation is given but human error is a big problem ... And without automation it will not rectify...

2nd point you have missed or you are only discussing as per report. So when you do a video your due diligence is required. The distance between Rangapani and chatterjat station is approx 2 Kms. Where the goods train has stopped at rangapani station then it's immediate speed is 47 kmh is next to impossible, further which is also clear evident that highest speed was 25 approx. So being a goods train it's technically not possible on that track & distance. Subsequently, heavy rain during the time of accident.
My colleague Paban Rabidas ,who joined our deptt just 7 months ago was badly injured & had to undergo orthopaedic surgery in his ankles in the incident who was returning to home after performing night shift.
Making 100 of documentations to solve 1 problem actually creates the problem. Indian Railway needs to ease down the processes and provide training on those.

Station Master is responsible for this accident. If station master had given the caution order (Speed 15 KMPH) to Good train locopilot, then this accident would be averted. Station Master should be punished. T/A 912 means authority to proceed at Normal speed, there is no any speed restriction.
Raiganj station master has not obtained line clear from station master of chorhat station. In case of paper line clear such line clear ought to have been obtained by the station master of Raiganj. This is the main reason of this accident apart from other reasons discussed in your video.
