I LUV you guys. Thai Cave is how I found and got into you. HAVE to say, Woody CLEARLY has an ongoing dehydration issue (and his paleness, gaunt, always cold). Covid/vaccine spikes dehydration in some. Thyroid is related in women. Supplements can do it in men. Alcohol. Marijuana. Chemicals/hormones in foods. I HOPE he gets to the root cause ASAP and makes whatever adjustments to his diet/supplements/meds/lifestyle that fit for him sap. Dehydration - when pushed past max - can then lead to a psiral of nasty, including emergencies with heart, eyes, teeth, breathing, lymph nodes, skin, fatigue.. I cannot event take OTC meds or drink much coffee or sleep meds without serious endangerment. Now I go for a iodized salty clear chicken or carrot broth vs any meds. I adjusted re what dehydrates me, I do'n't take any meds and haven't had an incident in 5 years and now know how to deal if I do . God Bless.

Yes, it was DCS but it was triggered by Woody's ongoing dehydration issue, IMO., which I hope he gets to the hottom of and adjusts his day to day routine accordingly and per what works for him. We all have unique chemistry. See my other comment. Btw, Woody is here because of BRIAN. Ftr.

It's possible a nice iodized salted water-based chicken broth and some apple juice and carbonated water could have set you straight. Dehydration is a core root cause.

I was a scuba diving instructor in Greece on an island in 2018 and got a severe neuro DCS in August with loss of consciousnesses under water. I had max level of DAN insurance. When I was transported back on shore and on O2, my fellow instructors and myself were trying to organize transport to a chamber. 1st thing we did is we called DAN. We spend more than 2 hours on the phone with them and got absolutely no help from them. They didn’t even know where was the closest chamber in Greece and wouldn’t acknowledge I had DCS. After getting angry with them, I finally organized everything on my own with my own money (boat, ambulance and chamber). 2 weeks later after I was fine (3 days of chamber, 5 days hospital) I called DAN again to get a reimbursement of expenses and again they didn’t helped at all, didn’t participate in anything (gear recovery, transport, medical bills). Since then I never ever recommended DAN to any student or divers.

Not a diver but I am a nurse and now work for one of those lovely recorded line nursing jobs and have been loving the channel for a few months.
This is absolutely baffling to me and insane. I did labor and delivery for years. We don’t say “when was the last time you ate” when someone is bleeding out or I can see the baby’s head, we can get the paperwork done later.
Half of those fields shouldn’t be mandatory. If they require them, that needs to be an onboarding form that they require upon paying for the service.
Also the fact that they offered you a seat at a table discussion with transcripts is insane. That shows they know they didn’t handle this appropriately.
I absolutely hate that this happened to you woody, and you too Gus because being the one thats level headed in a crisis is hard. I sincerely hope that change occurs after this and that you pursue it until it does. This may and hopefully will be a life impacting change for so many. Just by being aware but also potential policy changes.
So glad you’re doing better now and I would love more information once there is transcripts and meetings.
I’d also be super interested in talks about how dehydration on a long dive can cause this to occur. I knew the stops and blend of gas went into it but I didn’t realize hydration was also a factor.

    • @NoahBHS11
      @NoahBHS11 8 месяцев назад +7

Also a nurse as well. The other thing that sticks out to me (provided I interpreted the story right) is how underprepared it seemed the ETC team was to handle things. Changes in level of consciousness, profuse sweating, inability to regulate thermodynamic-wise… especially if he was getting hypotension. Start a line and get some maintenance fluids or boluses depending on vitals.
I also wonder, in addition to having DAN being able to act under the assessment of a physician in the future, if instead of filling out forms, could patients/representatives authorize charting information to be shared/faxed to DAN? For something that is a trauma or injury call, along with physician charting, these are a lot of documents that could help after the fact as well. Maybe there’s too much HIPAA/legal hurdles to follow for that.
All in all, DAN should, if anything needs “clearance”, develop a protocol where any physician can look at a checklist and say “Patient exhibits X Y Z”. They “score” the checklist and it gets referred directly back to DAN to determine fit to fly for inpatient settings. Takes all of 2 minutes. The provider is going to be your be-all-end-all assessment anyway.
Also a rapid protocol for calls from the field would be appropriate. Clinical presentation of DCS sounds like a standard that can be relatively simple to identify, and a quick “checklist” or set of questions to ask could be made by people who understand the science and could narrow down symptoms quickly - do you need to void, are you cold, etc.
It’s basically the ABCDEF trauma checklist but for DCS protocol. Give it to your phone operators at DAN or allow them to fax/send it to a provider if it’s inpatient and it will take them 2 minutes to determine if criteria is met with appropriate assessment and questioning. At maximum, immediate medivac; at minimum, more monitoring for additional symptoms with a checklist of things to look out for.

You can tell the paperwork was meant to deny care, not support it. It's written in the past tense as if it's follow up post-stabilization care. It has commercial flight and medical right next to each other.
It doesn't matter if you're "fit to fly" when in a medical jet - the fact that you're not fit is pretty much a given. Also cabin pressure is basically a hyperbolic chamber. There's going to be structural limits, but the pilots have manual control over when and how pressurized the cabin gets. You can fly at close to ground pressure pretty easily, and having a delared emergency gives you full discretion over and above any air traffic control wishes. Vectors, height, speed, and runway access. As a pilot, you just advise them what you need and are going to be doing. You can even land on military runways. I honestly would have tried going that route. A US Naval hospital specifically.

They clearly need to work on this or have a form that can be cleared for the treating physician to fill out and fax.
It’s kind of appearing to me that they’re advertising as “we will take care of you” implying in the moment but in reality they just cover costs. Which is fine for some people but that’s not what you need/expect in that moment

I'm a DAN member and have been for probably more than 2 decades. My dad grew up diving in Cat Island with his buddy and buddy's father. They always had an emergency plan to fly the plane at about 75 ft to get someone to the tank in Nassau if they got bent.
This video matters. These improvements you guys can empower the organization to make (I'm being VERY generous to DAN here) can and will save lives.
I'm shocked at how much red tape was on the emergency phone call and, as someone who relies on this network to ensure my safety, my family's safety, and my friend's safety - this must be fixed immediately.
I would put more significant faith in Amex Platinum concierge to get me to a deco chamber and pending your next video I will consider canceling my DAN membership.
Edit: finished the video. Wow... Amex did save you. Amex concierge got a water ambulance to extract my grandpa from a remote island to his doctors in Palm Beach when we thought he had a stroke. I'm not wasting time calling DAN if I have an emergency. I'm calling Amex until I hear some real improvements.

If it's so hard to get help when your a influencer with over 300k followers and knowing personally someone that is a consultant for their own insurance, just imagine how different this would of gone if you're a normal person with a emergency.

I'm beyond elated you're alive and well, Woody. Like many viewers, I'm not a diver, but I am an avid thru-hiker and mediocre mountaineer who does the vast majority of my trips solo and I have insurance through a GPS company in the event something goes wrong. Your story has me doing some more research, as I don't have a card like yours. The love of adventure and the desire to see the most difficult to reach places comes with many risks, if somebody promises to take care of you they should hold up their end of the bargain. Thank you, Gus, for being a brilliant, incredible friend for your willingness to face blood, vomit and lost dives to help your brother.
    It's neat you exist.

IMO hiking is not the same situation. I also have an sos device and plan through them for such situations but the main purpose of those plans is to rescue you when you do not have any other means to call for help (like being on the top of a mountain in the backcountry), hence the gps device uses satellite and not a cell network. The gps device will send out an sos signal so that the company will immediately form a plan and find a way to dispatch a rescue team to your location, whether or not they can make contact with you. They basically are working without information and without your cooperation, because you are likely unable to assist them in assisting you. When those people got killed by that grizzly in the Rockies, the parks rangers said they had received a call from the gps company alerting them to someone in need of rescue. The terrain is just so difficult, they couldn’t reach them in time. A credit card is useless in that situation. If you do have cell service, you can call 911 who will patch you over to search and rescue. In most jurisdictions, search and rescue do not charge to rescue. However, search and rescue will be limited by what equipment that local team has access to; some have more experience and resources than others. Where the credit card could come into play is once you are actually rescued and maybe you need medical attention, but you ultimately still need that gps insurance to get physically evacuated from the mountain to the hospital in the first place. I am also a diver and I see this diving situation closer to a travel insurance problem, where the insurance fails to come through in a medical emergency. For the credit card, you can just call them and ask them to increase your limit. You don’t need a platinum card, they will do it on a no annual fee card if you have a good credit history.

    Long post alert…
Boy, there’s a lot to unpack here. First off, I am so happy Woody made it and is on the mend! What a scary situation.
I am a newly certified diver (AOW with 28 dives). I am also a physician with some extensive training and experience in wilderness medicine, some of this pertaining to the subject matter of dive medicine. I think there is so much for new and experienced divers to learn and discuss about this situation. I am so glad that you are bringing some light to the DAN situation and hope that their processes can become easier for all. But there is so much more here.
I do have a question that appears to be aggressive and suggestive, but it is not meant to be. I am honestly interested in hearing the discussion around my question. Why was there a delay? As a new diver who is a physician, I would have taken you to definitive care the second you required Oxygen. Is it common for experienced divers who have DCS symptoms to take some oxygen and wait it out? I think a discussion regarding what are recommended practices and what happens culturally with experts like you all would benefit so many.
I can speak with authority and say the top priority is always the care of the patient. That goes for the DAN representatives, to the doctor is Abaco, and the public and private hospitals. Just because you can’t “see” the emergency, like trauma patients, doesn’t mean there isn’t one there. The “paperwork”, and even the payment, can be figured out later. The patient’s life comes first! I was so frustrated while listening to the story unfold
I would also love to have/hear a discussion regarding the appropriate planning, and contingency planing, for a dive trip. It should be more than just dive gear and I believe it would be different for different dives and scenarios. It think it is especially important when diving outside the US. Where is the closest chamber? How would we get there? Whose going to do what? Do we have the appropriate communication tools? Etc…Tons of teaching points here. (Disclaimer-I have not had time to watch the Planning for Abaco video yet.)
Last thing…I think the world dive agencies need to spend some time developing processes and educational materials for these well known dive destinations with regards to identifying, triaging, treating, and transporting dive injuries. It seems to me that the clinic in Abaco would have an SOP for when someone like Woody shows up. Now…I know it’s not the US in most cases, but these dive destination communities have a responsibility to to the divers who are bringing tourist dollars to the area. Additionally, I think the international dive agencies, who collect dues from everyone, should play a role.
Bottom line is that I am so happy Woody is ok and I am grateful that you have been so generous to share this story. Thank God for Gus and Brian who were able to watch over you and advocate for you! I think there is way more to do with this learning experience than simply getting the attention of DAN. I think there is 3-4 more episodes out of this!?!? I know that I am a newbie diver and there are certainly more experienced divers and physicians who can comment (uh Doug), but would welcome further communications/discussions if you so desire. Can’t wait to sit back and watch and learn. Keep healing Woody!

When they were describing the immediate symptoms upon returning to the surface it sounded a lot like an experience I had after a dive. I had also been diving while not drinking enough water and was abnormally tired and had a bad headache. It was nowhere near as bad as what happened to Woody though, I didn't need to breathe oxygen or anything like that. After re hydrating I was fine.
My guess is that as much as we try to categorize DCS as different levels its a sliding scale and most experienced divers have had similar experiences and he thought he was gonna be okay with a bit of rest this time as well. My experience level is similar to yours though, so take it for what it is; a guess.

Asks a lot of questions I also have. In addition, as there is no ILS at the airport, what other treatment would be possible if MEDEVAC was not possible due to weather? There is not even a one man chamber available in Abaco?
Then, Im a little concerned that Woody seems to intend to over hydrate in the future. There is some correlation between over hydration and Immersion Pulmonary Oedema (IPO) - have you considered this?
      Stay safe.

You're experienced divers with an ex Navy guy, why wait so long to seek medical attention? Crazy how life works. Nice work staying alive. I had a life flight last year that saved my life. It was a short helicopter ride, less than an hour, in Washinton State. $60,000. Thankfully, my insurance paid for it. My hospitals bills were over 2 million dollars. Thankfully, I just had to pay my $2000 yearly out of pocket deductible. I do pay $1900 a month for these benifts. Love your show. I did confined space rescue for many years in the fire service and I am a diver but never been interested in combining the two. Stay safe.

So glad Woody is alright. I actually went diving in the French Polynesia over Christmas break and some of them were over 100ft deep. I had recently gotten my advanced open water but had not run into feeling narcd.
I felt it for the first time in the middle of a dive with strong current. The only reason I was able to remain calm and take the proper safety precautions was thanks to you guys and your videos. I very clearly remember hearing you guys talk about feeling narcd and how to alleviate the symptoms and in that moment I had enough clarity to safely ascend and get myself under control and keep with my buddy and group. Thank you so much for all you do you! I’m sure hundreds if not thousands can point to you guys as a reason they remained calm in a stressful dive situation.

That being said, I have heard and read success stories and people’s rescued by DAN as well. So the scary thing now is that there will be some new divers who watch this video and decide to get regular travel insurance instead of DAN, then find out their dives aren’t covered when they need to do a claim. Or worse still, they don’t get insured at all.

Here are the list of DCS Symptoms from DAN's own website. This is just for those who are curious etc. I am just learning more about this too. I am no doctor nor am I a diver.
    Signs and Symptoms
    DCS
Unusual fatigue (I think this was possibly Woody's first symptom and it was extreme.)
Skin itch (I don't remember Woody mentioning an itch?)
Pain in joints or arm, leg or torso muscles (I don't remember Woody mentioning pain like this.)
Dizziness or vertigo (I remember Woody talking about feeling quite or very dizzy. )
Ringing in the ears (Yes I think Woody mentioned this! )
Numbness, tingling and/or paralysis (hmmm? Not sure sure? I think Woody mentioned he could not feel it legs or something? Marking this as not sure.)
Shortness of breath (Along with Unusual fatigue this was also basically Woody's first symptom and it was extreme by the sounds of it. )
A blotchy rash (I don't recall any mention of a rash.)
Muscle weakness or paralysis (Yes this is fair to say that a few moments I believe Woody has trouble walking? This could have been why.)
Difficulty urinating (Woody at least had no need to urinate. Could have have been Dehydration playing a role.)
Confusion, personality changes or bizarre behavior (Yes Woody was out of it for like 40-50% of this and did seem to act a little bizarre.)
Amnesia (He seems to have a little Amnesia about the whole thing. Getting days kind of wrong. Mentioning they kind of blur together. But I don't know if that counts. )
Tremors (I also don't recall this but this was a long event. )
Staggering (Yes Woody had problems walking etc. )
Coughing up bloody, frothy sputum (I do not think this happened to Woody. I don't recall it and am pretty sure I never heard them talking about it. )
Unconsciousness or collapse (It sounds like on 2-4 occasions Woody kind of collapsed and seemed to lose consciousness completely at least once! )
Like most conditions I don't think you have to have all or most symptoms to consider or confirm DCS.
I think that's possibly 9-10 or maybe more symptoms Woody had and a strong/severe indicator on a minimum of three. Out of 16.
Also on another website I read headaches were listed as a symptom. And that actually may have been the very first Symptom Woody felt but for some reason that's not even on DAN's list?

It's incredible how that plane operates and was operated. Truly fascinating story and I'm very excited to listen to everything with the transcripts.

Oh Woody, I am so glad you are okay now. What a scary experience to go through. Good on you Gus for being a good mate. DAN sound like a nightmare. I hope you get this resolved.
