This is not quite correct. You're meant to swallow after inflating the balloon whilst keeping the balloon inflated against your nostril and still holding the other nostril shut. It's after you swallow that you get the blowback that opens the Eustachian tube
I wonder why would they miss out this important step. Seems strange to me. It's written in the instructions that come with it. I tried to do it but feel uncomfortable with the swallowing, the air coming back seems to block my ear more. I have now stopped using it. I wonder why they did not mention the swallowing!
A trick that helps me a lot is to first keep a bit of water in my mouth ( 1 to 3 gulps/swallowing). Then just follow the instructions. I swallow the water slowly. It pops my ear every time. Without the water I find almost impossible to swallow while holding the damn balloon inflated!
Today my first day using this tool , I did all that in both ways for both ears but non opened, the air stays in the balloon and I feel nothing just pressure of the air trying to escape from my nose 😢 why?

It means you have negative pressure in your inner ear. I get the same after radiotherapy and getting so sick of it.
