I am 33 and my testosterone level is just above the baseline. I feel like i am dead inside, and my body is alive. I can't concentrate, i get tired quickly, started to hate people, explode quickly but have no willpower whatsoever, and can't sleep well. In my whole life i have never been ill. This is VERY frustrating. I don't know what to do. There is a long story behind all this, but i don't want to go into it because i am tired of talking about something nobody understands. Just want it to END.
Very good question! TRT with any of the testosterone products will diminish pituitary follicle stimulating hormone production and will thus decrease sperm count, adversely affecting fertility. Clomiphene citrate and Anastrozole will increase blood testosterone without reducing sperm counts. AS
Although it is a naturally-occurring hormone that mainly functions as an precursor to more potent androgens such as testosterone and DHT, it does have some degree of androgenic activity and might help the situation
@rifetf Chronic use of sustained action opiates lowers T by inhibiting the hormones in the hypothalamus that then decrease LH production in the pituitary gland that reduces T production by the testes. Opiates also have a direct negative effect on T production.
As you are aware, hormone replacement for most men is either undiagnosed by the MD/DO or a battle to get help. I have had to educate myself and then try to get proper treatment from my PC then I switched to a urologist. Neither had an invested interest in assisting me. Im 42, tt was 226 and free was 25. I felt horrible and when I figured this out I am now on test cyp self injections and feel great. Now Im fighting estrogen levels, started arimidex1 - 1.25mg week. Opinion on E control?
Sad that more clinicians are not aware or not concerned about low T and its many manifestations. T is metabolized to DHT or E, so if your T is too high, there is potential for an elevated E. I have never had to rx elevated E on the basis of TRT. You definitely need to check your T levels, particularly mid-cycle between injections.
In many cases, there is not a clear reason for the low T, although most of the time, the testes fail to produce a sufficient amount of T. The good news is that TRT should be able to restore your levels to normal and improve your symptoms.
@tomcat624u Anything under 300 is considered low, but free T determinations are important as well. With the IM injections, it should not be long before you feel salutary effects.
@aofnyc The real question is whether this is important. Men who go on T therapy are producing insufficient amounts of testosterone from their own testicles. It's possible to use T gels or other therapies intermittently, but I don't see the point. If a man wants to preserve fertility or testicular volume, his choices are to use clomiphene citrate or HCG injections, either alone or in combination with exogenous T. Hope this helps
@mario192586 I would not recommend using HGH. The T can be used indefinitely, but it certainly reasonable at some point to stop it for a while and recheck T levels. As an anabolic steroid, T can promote muscle growth and fat burning.
Hi, was wondering I have a lot of the symptoms for this, the one that is apperant to me the most is the fact that I have next to no facial hair, I have pubic hair, a bit of muscle, adams apple, nearly everything else but I look about 13 and am 25. It is very depressing and affects all parts of my life. I'm gonna ask my GP about it next week, but if I get shot down, which is likely the case then what do I do from there?
Hello Dr. Siegel I read and hear often about T levels. It seems that there is a contradiction of what a 'normal" T level is. So when my doctor gives me the T level number what should I be trying to figure out from this number. I am reading that the number could actually be low when it is considered acceptable. So what level is good for a 47 year old man 162lbs 5'10" Thanks
@emann43 You need to see your doc, check T levels and then you can be prescribe replacement T if appropriate...it is not difficult to get a prescription.
@heptixy Even though you are young, it is still possible to have hormonal issues and the best bet is to see a doc and be honest about the problem...nothing to be embarrassed about--it's not your fault. A simple blood test or two can help sort out the problem and treatment is available.
I got a blood test on Friday and the tests show I have a serum Testosterone level of 67. I don't know what it all means but I'm pretty bummed out about it. I'm 52. I lost 40 lbs last fall. I started to lose some hair so I asked the Dr about it and he sent me for a blood test. I have not yet seen the Dr but my level of 67 is super low. I was taking saw palmetto also and I wonder if this affected the reading. The thing that bothers me the most is how I collect fat in my chest. I've had man boobs for years and if higher t levels could help this to go away I'd be grateful. I've also had problems with anxiety and depression plus
Can 25mg DHEA oral daily have effects similar to testosterone supplementation? DHEA is readily prescribed over here (Europe). In my case it's hopeless to test for testosterone in the morning, I know it's high, while it become abysmal in the evening (sex-drive = zero in the evening, zippo).
question: should I skip applying Gel on blood test day? I got a result at 1600 one time but had put Testim/Androgel a 2 hours before blood test...my doc says to skip on blood test day as to not taint the result..not very accurate if you ask me!!
@ty99999returns Gels are more physiological, with AM peak and PM trough of testosterone, simulation typical biorhythm; shots cause acute super-high testosterone levels with slow decline thereafter to subnormal levels; also a bit easier to use and no need for nurse to have to do treatments.
Doctor, I am 56 years old and have been diagnosed with low testosterone of 1.98 on a scale of 4.5 pg/ml to 42 pg/ml. Isnt this extremely low? I am confused because all of the scales I see are different. I have also been diagnosed with insulin insensitity. Is this a cause or sympton of low T? thank you very much
If i stay with workouts like powerlifting or bodybuilding and focus on foods, do you think i can get same result that I am suppose to get from supplements?
@stratocaster1986able Certainly worthwhile to do a hormone check, but understand that many characteristics are under genetic control, including body and facial hair distribution and density. If your brothers, dad, grandfathers, etc have minimal facial hair, chances are that it is an inheritable trait. Your youthful looks that depress you now will probably make you happy later in life! If you don't get anywhere with your GP, you should see an endocrinologist for safe measure. Best of luck!
Doctor: I've noticed a sudden dramatic decline in sexual ability over the last 4 years, loss of libido and partial impotence. I'm now 54 and had absolutely no problems until I was about 50. Could testosterone levels dip that suddenly? Thank you.
hi doc ive been diagnosed with low T im 35 yrs old and i have a level of 9 nmol. i had alot of test done to find the cause, but they couldnt find anything! i think my levels used to be very high as 4 yrs ago i had it checked and it was 18 nmol and felt tired then to! before then i used to put on muscle mass easy and had a very high sex drive, both which have now gone. any ideas? also since 4/5 yrs ive had hairloss to. it started at the sametime. yet there is no hairloss in my family!
Doc, i tried a steroid for 10 weeks and now it should be out of my system. Around the day it was meant to come out of my system i have been feeling very emotional, i can't sleep nor eat and having anxiety attacks. Is this due to low testosterone?
That weight gain is excessive over a 2 month period. Being an anabolic steroid, you can expect some increase in lean body mass and increased energy, so something is awry and you ought to check in with your doc.
wish it was easier to find doctor's that aren't ignorant.. low tsh, low wbc, low rbc, borderline hematocrit and hemoglobin, borderline free test. chronic fatigue, weakness, 0 motivation or aggression.. insists SSRI's will help, even though they exacerbate all of my symptoms and leave me more bed ridden than usual.
@syke84 Yes I do...a well-rounded and balanced, healthy diet avoiding processed garbage, plenty of fruits, vegetables, whole grains, legumes and lean protein combined with bring it at the gym is the formula for results...forget the over-priced, over-hyped nonsense!

It is within the realm of possibility, although unusual.

Scales vary, but this is clearly low. Insulin resistance is clearly associated with low T and is related to abdominal obesity.
@ty99999returns Gels are more physiological, with AM peak and PM trough of testosterone, simulation typical biorhythm; shots cause acute super-high testosterone levels with slow decline thereafter to subnormal levels; also a bit easier to use and no need for nurse to have to do treatments.

hello Dr does opioid narcotic and THC lowers t levels?
If i stay with workouts like powerlifting or bodybuilding and focus on foods, do you think i can get same result that I am suppose to get from supplements?
Mine is 400 and my dr said normal but finding on line that this number will cause me to have symptoms. I have tons of body fatigue.
How can you cycle Testim so the testicles can still produce testosterone ? Example can you do 1 month on and one month off.
Yes, entirely possible...another possibility is gradual decline until critical "tipping point" reached.
Dr. I got rheumatoid arthritis 3 years ago can this disease lower my T levels ?
@stratocaster1986able Certainly worthwhile to do a hormone check, but understand that many characteristics are under genetic control, including body and facial hair distribution and density. It could be, although impossible to say.

@KravaLLT You need to see a doc and have your hormone levels checked.
Doc, can anyone lose T at any age? I'm 22 years old and feel like I should have 10x the T I currently have.
That weight gain is excessive over a 2 month period. Being an anabolic steroid, you can expect some increase in lean body mass and increased energy, so something is awry and you ought to check in with your doc.
@tomcat624u T replacement along with smart eating and an exercise program can reverse/improve metabolic syndrome.
@luthaa87 A vasectomy will bring sperm levels to zero, but has nothing to do with testosterone levels.
wish it was easier to find doctor's that aren't ignorant.. low tsh, low wbc, low rbc, borderline hematocrit and hemoglobin, borderline free test. chronic fatigue, weakness, 0 motivation or aggression.. insists SSRI's will help, even though they exacerbate all of my symptoms and leave me more bed ridden than usual.

@MousseTauChocolate I would see a urologist; if you already have, see another who specializes in andrology.
@tomcat624u Not a typical symptom of low T.
