Such events should go before a Coroners Court with Jury and if an individual of any description is deemed to be culpable then that individual is referred to the CPS. This way there is a public interest element to the decision from the start. Of course, the Coroners Court criteria will require amendment but it is the 'public' interest element that is essential and not an 'appointed' unelected body or group.
