@@VikVeerENTSurgeon Vik if you think that this is an isolated case then your absolutely wrong they fobbed me off as well with lies telling me that the operations that you do are only good for two years then you're back to square one , you may or may not remember me but I came to see you privately before going the NHS route , you assured me that their was an operation that could fix my lateral wall collapse , but when I went to the Royal ENT I begged them to refer me to yourself, but they just fobbed me off with lies and excuses.

Im in the US and getting my induced sleep study next week to see if i qualify for the Inspire implant. I wish this was available here now. I heard its on the way however i hold a CDL license and my job is on the line they wont pass my next medical exam until i get the sleep apnea managed. I have tryed so hard to adjust to cpap and other methods to no avail. Im desperate at this point but wish i could get this implant instead.
In New Zealand there is a huge problem with apnea, I have had it since I was in my forties, can't go to dentist I can't breathe, even struggle at the hair salon, you could travel the world with these devices, I'd be 1st in line, so desperate, we have to pay so much for simple devices, I had to stop using cpap, blew up stomach 😊

I was wondering when they would come up with something like this. The Inspire seemed unnecessarily invasive. I am also wondering if there are drugs in development that reduce the inflammation of the airway that causes it to narrow. Apparently, the narrowing causes an increase in breathing speed and intensity which pulls the tongue back into the airway.
inflammation accounts for a percentage point or two, I doubt that it would solve a sleep apnoea problem. I'm helping design a local anaesthetic implant that can be put in during a clinic appointment. it's probably years away from getting to market. but yes, this is slightly less invasive.
Hello Vik Can I ask about galvanic stimulus such as sports people get when they injure their foot or ankle? This is done by a physiotherapist putting the bare injured/recovering foot on a plate which passes current into the nerves controlling the muscles in the foot/ankle causing them to flex and thereby help to mobilise things and repair injuries. I think osteopaths use a similar technique? There is no surgery and I just wondered why an external electrical device couldn't do the same for the nerves that control the tongue?
Hi Doc. I have OSA and been using a CPAP for about 8 months. Apart from when I have insomnia, it doesn’t keep me awake especially now I have the humidity level so it doesn’t drip on my face and wake me. However, I don’t feel any better in the morning and am still exhausted during the day, needing to sleep for 16 hours at the weekend to make up for it. Also still losing my voice through tiredness, which was one of the main reasons I went on it as I am an amateur singer. Any advice on what to do next?

officially it would work. it depends on a few things, but yes, it should. I'm not keen on making a diagnosis without DISE, but they are licensed to go ahead without it.
I must say that I am shocked by the possibilities of dealing with sleep apnea. In Poland it's CPAP or just cut tonsils etc. It's like you need to treat yourself. Can you tell me in which hospital in UK there is a full treatment on sleep apnea?

@francesjones9384 I used Zeus and bought it asap when it was released (rip my lovely beard) it worked okay (I had nose problems then so every night stuffed it didnt matter since nose breather, but nose a bit better after surgery) but was to big also there is a bit of trial and error to find the best spot for it to had the tens/ems/similar to make the muscle move. On the plus side it was a continious burst instead of those made from Asia (no geographical pun intented more to let you know from where, without name drop) that use their kind of bad ai to feel when there should be a burst (Yes I tried them too until someone nicked my bag where I had it). So I think if they got the design/maintenance factor better and have some clever way how to make it stick more then possible adhesive (maybe a skimask or sth, no joke) I would gladly give i a try again, since I am on my 3rd cpap machine with my 5th mask and still cant where it..
If your fitness level changes it you get sick your breathing rate would change. So you'd have to manually change it's setting? There are many devices that seem to trigger the vagus nerve to help with sleep. Could tying to that frequency of events help to keep everything in sync? (Not sure if vagus nerve simulation actually helps, seems like none of the devices really have proof they do, not sure)

it works great for selected people. I did a trial on it during DISE, worked amazingly for some, not at all for others. honestly I couldn't work out why.
I tried inap with poor results. It seemed to help but was Hit or miss... i actually bit off the flange and it was hanging by a thread in my mouth which could have been really dangerous. I would avoid it unless you get the stronger flange for sure!
Any use if you have a deviated septum? That's what causes my breathing problems at night.... and the operation to correct it is too horrible a prospect (and doesn't, apparently, always work).
Hi vik, i had septoplasty and outfracture but after the outfracture my nose is dry and i have decreased sense of smell and air. I had radiofrequency ablation before too but problems begin after outfracture. Is it possible to make the turbinates back to their original position?
Hello doctor do u know if this works is people who for some reason have started to have sleep breathing disorder duw to a brain stem issue , can it still independently work for the full breathing process . I mean what if there is a central apnea events are involved here
it's for those people who have a life threatening and life disabiling condition and no other treatment option. it's a last resort. I hope that helps put it in context, most people who aren't suffering as much wouldn't understand, it certainly isn't for everyone. it's like cochlea implants, you would only consider it is there was no other option.

Consume water only the last 2 and a half hours before lying down to sleep. No late nibbling or other liquids. Then if you do snack late follow with gaviscon advance 10ml. Monitor progress with app snorelab
@@drk-ent i used to take gaviscon . but when i take it daily for a long period of time i do pauses. I can't without ppi`s though.. i m 2 Meters 120kg . I m trying not to abuse them , even ppi`s i try to take as few as possible since i take them for 18 years i think. i m 34 now

Would a magnetic attachment impact negatively?
I guess they were worried it would damage the skin sandwiched between or that it would fall off in the middle of the night if you gained weight.

Are the Inspire and Genio implants MRI-compatible?
1.5 Tesla only for Inspire whereas Genio goes up to 3 Tesla MRI which I believe is the modern standard.

it does not help central apnoeas, but it can help some problems in the brain stem.

I’ve had pretty good results with the tongue sleeve. Occasionally it loosens up and I snore but it has saved my marriage.
I still can't believe after so many thousands of years of human evolution that my tongue tries to kill me while I sleep....

@@drk-ent This works! I monitor with a Wellue O2 ring. I still need CPAP though for best results.
OSA causes reflux, it's not really the other way round. I would fix the OSA first. good luck!
@ i have gerd since before sleep apneea, OSA started for 2 years now. Think is the hiatal hernia and hyperacidty causing me all the trouble
