The flight crew of Helios 522 overlooked the incorrect pressurization system state, on 3 separate occasions- during the pre-flight procedure, the after-start check and the after take-off check. The aircraft took off with the pressurization system still turned to “manual” and the aft outflow valve still open (partially). (Air crafts are pressurized because the air outside is very thin at the altitude they fly. To survive these high altitudes, occupants need help breathing. The solution is to pump air into the plane so the interior pressure is higher than the atmospheric pressure, which is regulated with the outflow valve.
