What do you mean when you say that there is trouble with Continuity of Care with Managed Care? Do you mean that Nurse Case Managers aren't following up with members properly and in a timely manner? Is it effecting the health or treatment of at risk members who may be in the CCM or CCS category?
Reading the deck isn't a very good idea while presenting. I just wasted my 12 minutes. You could have provided a link to download the deck, and I would have saved my 5 minutes atleast(or anyone for that matter). Thanks anyways for the info. Wasn't very useful though !!
