For those who claim Jackie Chan should be number one, I disagree because Jackie Chan is basically a stuntman and an actor. He actually started in the industry as a stuntman for Bruce Lee. (He is a crazy nut that put his life on the line for every movies, but stuntmen were disqualified from this list...)

He started as a Stuntman but that was when he was very young. He has been a lead actor for a long time. He is not considered a stuntman anymore. He is an actor so should be on the list and he should be much higher as he was the actor in the movie where he nearly died. He was not a stuntman. He was an actor doing his own stunts.
Jackie is an actor who does his own stunts, he is not just a stuntman. That's like saying Tom Cruise who's somehow first on this list is also only a stuntman as he is also famous for doing his own stunts.

That kind of stupid scenario can literally be applied to anyone at anytime, it's redundant. If you hear almost died it's usually a severe accident that brings people near to death.

There was a level insided the tank that she could pull if she needed help but the handcuffs she had on got stuck at the bottom of the tank so she couldnt pull the lever nor swim to the top

In Lord of the rings when an orc threw a dagger at Aragorn (Viggo Mortensen) he was supposed to throw it off to the side and Aragon was to deflect it. But the dagger was actually thrown true. Viggo still deflected it

Buddy Ebsen (who later played Jed Clampett) also almost died filming The Wizard of Oz. He was the original Tin Man but the silver make up poisoned him and he spent months in the hospital after almost dying. Jack Haley replaced him and they used different makeup.

I have a question for anybody. The one for "Now You See Me," where the girl was momentarily trapped underwater....so when she bangs on the glass "for real" and everyone thought she was just acting, I'm curious, what do you suppose would've been her "safe word" to alert the crew that something had indeed gone wrong? I don't expect a literal answer, as there's no way to know, but this one bothered me for some reason. Because we're either led to believe that they didn't anticipate something like this happening and so had NO contingency plan (highly doubt that) , or there was some agreed upon "signal" she could give. What do you suppose that could've been? Only thing I can come up with is some kind of very obvious hand gesture? You obviously can't tap on the glass to alert anyone since that IS a natural reaction (and was in the script). Was it a matter of panic and the actress simply forgot what her signal to the crew was supposed to be?? This one really bothered me because it seems so flagrantly absent-minded by someone. I just need to know I'm not crazy by having some either agree with me, or give me some other theory. Thanks in advance.

im pretty sure chan has had many many injuries and "No insurance company will underwrite Chan's productions, in which he performs all his own stunts. After a number of stuntmen were injured during the making of Police Story, the star formed the Jackie Chan Stuntmen Association, training the stuntmen personally and paying for their medical bills out of his own pocket." according the Guinness book of records

Owen Wilson in Behind Enemy Lines - During a scene where a tank fires at him, he was pulled back by a cable tied around his waist to mimic being thrown from the explosion of the shell, but during one instance the cable almost was pulled in time and he was almost killed by the blast.

As Aragorn, Viggo evaded death twice. Once in the near drowning incident, and the other time was in the fight scene with Lurtz the Uruk Hai. Aragorn expertly deflects a knife thrown by Lurtz with his sword. The problem is, it was a real knife, and the attack was unscripted.

One that was missed was Scott Glen in "Backdraft" although it was not on camera, during the scene in which he and Kurt Russell are hanging 15 feet off the ground, and on fire, Scott's fireproof equipment once failed during a take, but the crew had the hoses on just as he gave the signal to indicate something was wrong

There's posthumous performances, but I don't think that was limited to on set deaths. They did do a posthumous performance list though.

As others have said, Bruce died in the midst of making Game of Death but DID NOT DIE on set. HOWEVER, his son Brandon was in the middle of filming a scene when accidentally shot by a prop gun.
Technically, I don't think Bruce Lee was filming Game of Death when he died. He postponed filming of Game of Death to work on Enter The Dragon. After filming of Enter The Dragon wrapped he died after taking some medicine for what he thought was a headache which was a more serious brain condition. (Being that it was in the hotel of a female friend added to controversy of infidelity, as well as if he was poisoned) At the time I don't think Game of Death's production had resumed thus the reason only 10 minutes of the original film exist.
Brandon Lee was shot by a gun which was used for two scenes, one where it was used close up so realistic revolver bullets were used, another with caps. Unbeknownst to them, during the first scene, the head of one of these bullets was lodged in the chamber, and when fired with blanks in the second scene created enough force to launch what was lodged in the chamber and mimic a real bullet.

Another diver gave him a regulator, but it was upside down, and caused him to suck in water. A cameraman finally helped him get the regulator in correctly. Later that night, Ed Harris broke down in tears due to the incident.

I remember that. Hexum thought blanks were harmless and put a gun with blanks to his temple and fired it. Blanks still have gunpowder in them and the gunpowder exploding is enough to kill when placed right at a location such as the temple. At the time an article I read said he was showing off for some girls between takes.
I think there was also an actor who died falling down a stairway while filming a scene but I don't remember who it was or what show or movie they were filming for. I seem to recall that, out of respect, they made sure the film of the fall was never seen by anyone except the police/court.
