Seems that despite the relief cold or elevation (from RICE) might give from pain or pressure, more blood flow to the area seems to help the area heal better. Would this apply to a hair transplant, too? Seems another important factor in recovery is sleep, so resting slightly more horizontal after a procedure might be conducive to better sleep and more blood juice to the head to nurture and heal the grafts. It's also totally possible the swelling around the head would just induce headaches and pressure to the point they can't sleep at all. What's the good Doctor Linkov's take on this. Finasteride does not “lower testosterone”, it lowers DHT. The medication actually RAISES systemic testosterone (by about 10-15%) as less of it is being 5 alpha-reduced and converted to DHT.
Yes, but DHT is a much more potent androgen than testosterone. So, any effects of higher testosterone would be more than offset by the lower DHT levels. This refers most commonly to sexual side effects and motivation and energy levels.

@jahstinful from what I have read and experienced during the short period when I tried Propecia, I gather that DHT is in fact a (sort of) supercharged version of T. That doesn't mean that it can't have different effects, too.
@@ordoabchao4202 DHT is very androgenic but it is ONLY effective in the areas it's produced (prostate, skin and hair follicles) as an adult man since the hormone is intracrine in the tissues it's produced rather than endocrine like testosterone. It doesn't matter how adrogenic it is when it can't be used beyond the tissues it's produced. Most other areas in your body deactivates DHT by 3a-HSD and turns it into a very weak androgen called 3α-diol. Saying it's some super charged testosterone is straight up BS. If you replaced all your testosterone with DHT it would be effectively the same as castrating yourself. The reason why you can get sexual side effects from finasteride is not because you lower DHT but because you raise testosterone by around 10% when you take it which means you will have a slightly higher conversion of testosterone to estrodiols and for men that are especially sensetive to estrodiols will experience problems with their libido.

My older brother started balding in his mid 20s while my hairline stayed strong, I used to poke gentle fun at him over it. I just kind of assumed that maybe I'd be one of the lucky few who get to keep their hair into old age, we have a few men in my family who have done.. then about a year ago at 31, my balding started making up for lost time. I went from a full head of very thick hair to the receding hair line and pretty intense thinning on the crown very quickly. Karma for my jokes at my brothers expense. Needless to say he's enjoying it very much.
I started losing my hair when I was really young. There was one guy I used to work with way back who had this thick impressive head of hair and always made fun of me. He would be the last guy I'd thought would lose his hair. Fair enough, I saw him on Facebook about 10 years ago. He's now bald as a coot. You just never know what's further down the path.

May I ask what would cause extremely rapid receding hairline? I always had a little bit as I am 36... but in the passed 2 months it has become BAD. If I continue at this rate I will be completely bald by the time i'm 40.
I have a question about finasteride and minoxidil , At the clinic they told me that the Topical solution that they produce that contains finasteride and minoxidil + other ingredients would be enough treatment without the pills and would also be reduced side effects of the medicine. My question: Is the topical solution as effective as taking the pills? if not do you recommend taking the pills (finasteride and minoxidil) and topical solution at the same time? Also would it be better if I got a pill cutter and micro dose finasteride and minoxidil with the topical solution? what do you think Doc. Also thanks in advance.
This was such a well documented video and thanks for your commentary. I'm a month since my FUE in Turkey and it's a long road. The biggest struggle first month is the washing of hair with the foam and shampoo, washing hair everyday and not being able to apply a cream just made my scalp feel so dry. I eventually used aloe Vera gel after week 3. The itchiness just stopped like a week ago, the donor area. It wasn't a bad itch but there. I didn't leave a feedback for my clinic when though they wanted a video of me while there, I prefer to wait the whole post op than leave a review on the day of the operation. They were nice, professional and hopefully 6 months time I'll be able to document my journey too but nothing before. I'm not going to send hopefuls to a clinic just because they were nice on the day of the OP. 20:28, if Finisteride is to prevent hair loss, and I've had HT of good follicles to transplant area, what's the logic behind using it? It's so depressing to think one would use a tablet for life at a risk of E.D, I'd rather be a bald head with a full hard one than Tarzan with head full of hair and got a floppy snake 🐍. Thanks for the video.
One note about the mentioning of lower testosterone: taking finasteride actually increases testosterone levels in the body, as the testosterone that would be turned into DHT is simply not converted.

Dr Gary's I am quite shocked many plastic surgery procedures, hair graphs, and as you and the gentleman who made the video mentioned dental procedures that I can assure should either be done with general anesthesia as opposed to needles stuck in an already super sensitive mouth for a root canal. Xanax or Valium wouldn't help much as I already am prescribe the afore 1mg 3 x daily. Not because I like them, I've been on them for 15 years due to panic attacks and night terrors. I have terrible panic attacks so my Dr and I choose this and I still suffer horrible night terrors. It is unbelievable I get stuck in dreams where I'm being tortured by very evil people. I can't recall if I feel pain but I don't wake until someone hears me scream and shakes me. I'm not quite sure who the people in those dreams terrorizing me are but thinking now likely my last dentist with his evil needles and chair he never warns when he will push me up or down. You are looking at wrong familly, thats why. Male baldness is transfered through X (female chromosome) so you need to look at your grandmothers side of the familly to see why your dad is bald.
Genes are still randomized. A mom and dad can have 5 boys. Let’s say they’re 6ft and 5’7. There’s a chance some are shorter than dad, some are taller than dad, some are even shorter than mom, etc. Dad just wasn’t lucky.
Doctor I have a question. Is it recommended to stop taking finesteride and minoxidil before your hair transplant? As someone who has been taking both for a few years now and have decided to go through with surgery, I've geen told to stop taking it about a month before the procedure and was just curious on your thoughts.
if you've already been taking it I think just continue, from what I've heard they suggest you don't start before, so that the transplant doctor can judge the donor area better... however taking it before means that you wont need as many grafts as you've stabilised more hair
@Dimitri: The continuous intake of Finasteride is very important to protect the transplanted hair from attacks of DHT, and to keep the hair you still have.Using minoxidil on freshly transplanted sites, I would be very careful, would even suspend it for some time. Unfortunately, minoxidil and finasteride have a synergistic effect. The quality of the hair that is affected could decrease.
Everything is more expensive in the US, but not necessarily of better quality. If choosing to do it overseas I would roll the dice in S.Korea or Japan 99 out of 100 times vs. certain other regions.

I am 6'4 and 290lbs of muscle, he is 5"6 145lbs and hairy like a gorilla. He thinks I am not his biological child. Lol. My Maternal Grandfather and his Great Grand father and his father had 100% of their hair at their death including a super low hairline and no body hair... Meanwhile, a few friends are very bald and never used AAS, ever. I guess I am lucky with good genes. I feel bad for bald men. It ruins them. I hope they find a cure for baldness. It totally destroyed my friend Marks life. He literally stares at my shaven that I shave to skin. Its strange that I hate hair on my body and head and shave it off 2 x a day.... . Mark has zero confidence and has tried every known baldness treatment to man, including transplants, that failed and left him with ugly scars and a few random hairs poking out of a lumpy scalp. He has used laser, red lights, massage, Minoxidil and Propecia for many years. Meanwhile I still use AAS steroids weekly and do not plan on stopping... hey Dr. Would you recommend taking 0.5mg finasteride Monday, Wednesday and Friday? I know the regular does is 1mg a day but I am concerned of side effects. Do you think the 0.5mg dose schedule is enough to help stop balding?
side effects won't happen, they're a myth and not real, just take 1mg a day, or get a 5mg finasteride pills and cut them into 4th's and take 1 a day, that is the cheapest way to take them. Watch haircafe's channel on youtube
They said I can’t get transplant due to my donor area being bad? I was in shock and showed two clinics and all rejected me….? Dr. Gary are you able to help me….my last resort would be SMP if I can’t get a transplant :(
I really like all Gary’s hair loss videos but I thought he could’ve elaborated when the guy said “you have lower testosterone on Finasteride” . As far as my understanding, You don’t have lower “testosterone” in your body you have lower “dihydrotesterone” which is the the byproduct of the testosterone, and the 5 alpha reductase reaction. DHT plays a massive role in masculinization during puberty, however post puberty it does not play much a role in sexual function which is why side effects are relatively rare
Why do people get surgery in their home country for more money instead of flying to turkey? My inclination is to pay more and stay home because it’s too important to try and save money…but is 10K that prohibitive?
Here is the problem with the reported numbers of side effects. They are only the reported ones. Many people stop taking it without reporting side effects.
why do we have to use medical treatment after hair transplant ? if there's side effect in those medicaments , why can't we use natural products like nigella oila or castor oil ? thx for the anser
Dr. Linkov, since you mentioned your Minoxidil video, I'm wondering whether one can use salicylic acid in a face cleanser, or whether that would interfere with the Minoxidil?
I went on Propecia/Finasteride many years ago and the side-effects were VERY intense for me. I flushed my 3 month supply I had and NEVER wanted to touch that horrific drug ever again. Thank god the side effects eventually went away after a few months. I then decided to accept my hairless & shaved my head ever since.
I tried hair systems, and I wonder if it's possible for a Dr. to restore my temples to have a better framing of the face, to have good sides while wearing my system
Address another area of hair lose. That is the issue isn't it. The hair continues to fall out behind where the transplant was done so you need continued transplants to address the hair lose further back or you end up having a fringe of hair and nothing behind it.

I’m over 50 and glad I still have a full head of hair. I’m Native American and guess it’s just genetics. But I had severe Covid 2 years ago and my hair really thinned out and thought it was permanent but it’s back to full thickness again 🙏
You may also try a collagen supplement, it might be beneficial for hair, skin and nails.
Your people deserve it
Got COVID in March of this year and experienced extreme hair shedding that started in May, and is still ongoing to this day... 5 months later. My scalp is a disaster area. I got a buzz cut last week and now I can see the full extent of the damage. My hair is very thick so nobody believed this was even happening until I shaved and took pictures. Devastating feeling. Glad to hear your hair grew back. I am hoping that this is just COVID induced telogen effluvium and not rapid onset androgenic alopecia. For awhile I had thought the shedding was being caused by creatine
You are lucky man sir im under 20s and loss lots of hair on my head im very depressed 😢
@@arggamer575 My Hair Loss started when I was 18-19 and I reached NW 4 by the age of 24. That's when I started Finasteride and Minoxidil. Safe to say my Hair Loss stopped, but I had to get a Hair Transplant to recover that which was lost. Now I am at NW 2 at the age of 28. If I didn't start those 2 drugs I would have been completely NW6-7 bald by now. So there is hope.
I'm currently 25 and have been taking minoxidil for 7 months and it definitely restores lost hair, I had a few bald spots that now have hair that had been completely bald
Seems to depend on the person. Some people report great results, but for others it seems to have no effect at all.

Could you also do a video about the current status of research of hair follicle cloning and also about the research on the use of Scube3 protein for hair loss?
How about a video discussing color of hair and it's relevance to transplantation? Are grey hairs viable? Do darker hairs do better than lighter?
We can do a short on that for sure
I lost my hair in my early 20s and never thought about it much until I found your videos a couple of months ago. I'm 47 now, and to be honest, too busy to dwell on it. What's your opinion of the recent Japanese research that grew hair in a lab ?

Doesn’t Finasteride INCREASE overall testosterone by decreasing the synthesis of DHT? Repeatedly in his video it’s mentioned that he experienced side effects related to decreased testosterone levels - of which is somewhat dubious if my above thought is correct.
Thanks for the great video!
You’re right, testosterone goes up, I didn’t make that correction in my comments.
Yeah finasteride has no negative impact on test levels, it's purely DHT reduction and therefore a slight increase in testosterone.
I thought this too and expected Gary to correct. Glad to see it followed up here
Since it increases T, does it mean gym goers will see some different results?
im curious too about this, seems logical that it will be beneficial
Just found this channel after my haurtransplant. Done in the Netherlands. Binge watching since yesterday. Great videos!
Enjoyed this video. Very informative. Also, your eyebrows are looking great
My older brother started balding in his mid 20s while my hairline stayed strong, I used to poke gentle fun at him over it.
As much as having AA for 3 years starting at 17 was a very intense life adjustment. 20 odd years later in this Instagram world we live in, I'm thankful I had to learn to ngaf. School, work, life, I had to just make people accept me because life didn't stop cos you think my hair looks janky.
Will you be doing a video in the future about balding in women or the pros and cons of medications such as spironolactone? Love your videos!
I am confused. Everything I have read says that finesteride actually raises testosterone levels by 10-15%?
Really? Never even heard of a raise in test.
Dear Dr Linkov, could you please talk about female balding, especially related to insulin resistance and thyroid problems, how the could be treated (selenium, etc), how long it takes to recover? Thank you. 🙂
I live in Chiapas Mexico and I've never seen a balding indigenous person. And they go gray around 80 years old.

Doctor, are there any studies looking at whether there is correlation between the way hair grows in as a baby and the way it is lost later in life? Is it a last in first out type of thing? Or is it nothing?
Possibly a video on dermititis on the SACLP and FACE and treatments moving forward for short term treatment and long term treatment and types of treatments available and trust worthy thanks 👍.

12:28 but aren't test levels not still the same (or slightly higher) on finasteride? If test is not getting converted into dht then wouldn't there be marginally more instead of less test?
Correct, fin does not decrease testosterone levels.
yeah this guy is very uninformed
Hey there! Could you share where you went? Also how many grafts and how much it cost you? Thanks!
So how did it go and did you keep using the medicine ? Just curious
One note about the mentioning of lower testosterone: taking finasteride actually increases testosterone levels in the body, as the testosterone that would be turned into DHT is simply not converted.
But then the negative feedback mechanism kicks in and it can be converted to oestrogen via aromatisation.
No use of testosterone if it's functional variant itself is not available.
4:30 Saw palmetto? (also biotin)
Dr. Linkov, have you spoken about its use along with finasteride?
hi doc! Big fan here! Can you please do a short video on FUE recovery? I.i., returning back to strenuous exercising, having your first shave, using normal shampoo, etc?
I wanna see examples of patients that are not suitable for hair transplant. I am Norwood 6-7 so I’m wondering where is the limit, when do you think it’s not worth going on that journey.
8:50- and the more harsh truth is for someone is was depressed before experiencing hairloss, then later get on finasteride, Starts having all the side effect with no gains, will worsen their depression
I remember the Internet forums saying 2020 was the year that hair loss would be “cured”😢
It's always 5-10 years away 😢
Dr.Gary if some one is using finasteride and minoxidil.....would you say don't use anything else like a store bought shampoo or conditioner that contains caster oil or DIY fruit blend hair rinse where u blend up strawberry and keep it on your head for a few minutes them rinse it off? Would you suggest only doing the finasteride and minoxidil only?
Does a hair transplant infection cause significant risks of graft survival? I am on month 3 and am currently applying the antibiotic I just got from my doctor
Yeah I do need general anesthesia and then a needle prior to waking for that pain!
"Some people use staples" why was that funny? Why did I laugh at that?

interesting that early transplant patients like Sinatra, Elton John and myself all had the operation and ended up wearing wigs since eventually the donor sites get exposed as you age and you have no choice. It is curious that people still think that Finasteride does not regrow hair given that we have numerous studies up to 10 years that categorically demonstrate that it regrows hair in the majority of people that take it. The data is there, why are these people making videos about baldness and finasteride without knowing this?
Doc, what do we think about rosemary oil as a substitute for Rogaine? What do we think about rosemary oil for hair regrowth in general?
I truly appreciate the thoughtfulness of these videos. Your comments are insightful and helpful. Were NY a realistic option for me, I'd certainly be interested in a consultation from City Facial Plastics. I live in the LA area. Can you recommend any doctors or clinics in the area? I've tried Googling the topic, but there are so many garbage results through which to sift. Any input would be sincerely appreciated.
Not sure where to ask this, but as far as the scientific literature i've read over the last few years indicates, swelling isn't the villain it used to be deemed as for recovery.
Cheers for all the insight,
My grandfather only had a little hairloss at the temples when he died at 89. My dad is only 68 and has it at the crown as well. How is this possible?
I know two brothers close in age, one is 48 one 42. 48 has head full of hair, no balding, no receding while the other has been losing his hair since 30 and 42 is pretty much bald. Some ppl are just lucky.
Eyebrows looking good!
Great video very informative. I just wanted to ask why didn’t you go over the prosthesis option
Finasteride, originally prescribed to counter prostate hyperplasia, turned out to heighten the risk of more agressive prostate cancer at a later age.
Don't stop finasteride but stopping Minoxidil a week before surgery is fairly normal
Everything is more expensive in the US, but not necessarily of better quality. If choosing to do it overseas I would roll the dice in S.Korea or Japan 99 out of 100 times vs. certain other regions.
It's sad that baldness is so common and causes mental issues and lost youthful looks. On the flip side, I am a 53 years old male. I have used Anabolic Androgenic Steroids AAS, since I was 20 years. I used AAS in ridiculous amounts of injectable amounts. 600mg a week. . I barely have any body hair and zero hairloss. My father is bald and has peach fuzz on his head. I pray for a cure for this terrible condition that ruins men and robs them of their youth and good looks. I'm praying for a cure soon .. 🙏🙏🙏
In the video he mentioned that "he did not do his hairline any favors when working late". What did he mean?

According to r/tressless, yeah.
They said I can’t get transplant due to my donor area being bad? I was in shock and showed two clinics and all rejected me….? Dr. Gary are you able to help me….my last resort would be SMP if I can’t get a transplant :(
What are your thoughts on verteporfin and how it might be used in practice one day?
I got my 2200 graf procedure there. Highly recommended. I am planning to go next year for a re touch.

Speak to your derm but i would not expect any problems with this combo (provided the SA is used on the face and min on the scalp)
Gentlemen, one of my biggest takeaways:
A hair transplant is not a replacement for medical therapy (taking pills like finasteride and minoxidil)
Doctor, why don't you move to a cheaper State so we can come and use your services?
Because my family is here. But good news is that we will be offering some FREE hair transplant surgeries soon. Stay tuned.
Will your hair transplant work if it’s your medication that is making your hair fall out?
Address another area of hair lose. That is the issue isn't it. The hair continues to fall out behind where the transplant was done so you need continued transplants to address the hair lose further back or you end up having a fringe of hair and nothing behind it.
I never want to lower my testeserone. Isn't Mens testesrone already greatly lowered than it was in the 50s.
Hey Dr. Linkov, first and foremost thank you so much for your videos and all the knowledge that you share. Would you ever consider doing a video on Seborrheic dermatitis? I suffer from this condition and this hear i had a 4700 graft transplant. My conditions comes in waves and it has affected me since 2016. I worry about it affecting my results of my hair transplants.
Hey, can you make a video (or comment response) sharing your thoughts on the effects of oral minoxidil and accelerated skin aging? This is something that is making me hesitant to take it as I have quite youthful skin and would ideally not prefer to have that side effect.
