I wouldn’t worry too much about your Sims or other premade townies dying to provide for the Reaper career. I have replayed GTW’s active careers not long ago and noticed two things:
1- In the detective career, only randomly generated Sims could be criminals and be imprisoned, meanwhile premade ones were always innocents and would be released if falsely accused.
2 - In the doctor career, premade Sims could show up pregnant and give birth, but the babies were never “canon” and they’d just disappear after the delivery. Like, I had Eliza Pancakes show up pregnant three times and the babies never appeared in her household or family tree afterwards.
I guess it’ll work in a similar manner for the Reaper career. Either it’ll be only random townies or the deaths will never be “canon”. Honestly, neither are great. The lack of consequences makes the careers feel kinda pointless, and it also makes the detective career way too easy. But I obviously don’t want my Sims ending up criminals, pregnant, or dead, out of my control. The ideal would be if it was a toggle in neighborhood stories, but I doubt EA would think of it.
Maybe there's also an option if you want a chance for existing households to be reaped or not. I wonder if maybe a GTW refresh is in the stars if they expanded the system for this expansion. It would be nice, I do like the GTW careers a lot but they could use a bit of brushing up.

If it works like the Get to Work careers, I'd guess you get randomly generated townies you have to track down similar to the detective career. Otherwise you'd pretty soon have a pretty dead map.
I’m sure you can edit the Area without cheats if it’s like the science lab or the police station and hospital from get to work.
