Nicky wasn't in the video but he is playing the piano on the song. Mick played the electric keys. Ronnie had just joined the band and wayne played the slide guitar on the song.
This is the studio version with Mick doing live vocals over it. That was common back then for videos before the MTV age. They did the same thing for Brown Sugar on Top of the Pops. Live vocals by Mick but the rest the studio recording. Charlie even had cardboard covers on his drum heads for that one.

"Black & Blue" was a strange period for The Stones. Their previous guitarist, Mick Taylor, left the band, and this album showed them trying out different guitarists on different songs. Despite these circumstances, this is, IMO, an underrated album. There are many GREAT songs on it---"Memory Motel", "Fool To Cry", "Hands Of Fate"---and is well worth checking out.
A fun antedate about "Fool to Cry" - when filming Star Wars: the Empire Strikes Back, in 1979, in film studios near London, Mark Hamill heard this song. Mark H' has said in an interview or more, that on the Degobagh set, he had to wear an ear-mic' to hear the off-stage dialogue of Yoda, or direction. Hamill, at one point, caught BBC Radio, not studio talk. He says in a SW's documentary that he paused, and exclaimed, "hey, I've got the Stones, Fool to Cry" and the voice from the studio returned with like "Mark, get back in character" 😀🤫
