So does "controlled airspace" only apply to airspace near and around airports? Even downtown areas not under airport airspace are not considered controlled and therefore do not require permission?

Yes, controlled airspace at low altitude is found near airports. It usually starts at 1200 feet above the ground in other parts of the country, but obviously that's higher than where drones are flying.
