Ohhh that hurt!!! Also a piece of advice don't get carried away buy the videos on the internet that the cncs plunge crazy fast on the work piece (they are just running a gcode that have tested 100 times and know that it will work) , always start with little feed when you are testing a new gcode...

The problem here is not that the wrong coordinate system was used it is that the operator failed to catch the problem before the collision. Never making a mistake is not a realistic strategy for preserving your safety and equipment. Establishing good habits that allow you a margin in which to detect mistakes before the crash is.

Before hitting cycle start cultivate the habit of
    1) hitting Memory (or MDI), Current Commands, Reset.
    2) Stoping to look at the code about to be run
    3) think about what motion you expect
    4) hit Cycle start and immediately cover feed hold in case the motion is not as expected.
the idea is to prepare your mind to work more quickly if something goes wrong.

Safety code can be added during post at the beginning and end of tools to ensure the machine can't do anything unpredicted such as:
    G91 G28Z0 G49 G80 M05
    G90 M09 G20 G40 G17 G64 F.001 D00

When running an unproven setup, rapid motion should never be above 25%. Feed hold before each tool touches the work and check the distance to go to see that it is as expected. Get a straight edge or a caliper in there if need be to verify the tool is where it should be before letting it touch the work.
This crash was preventable, not by being more careful to avoid programing or setup mistakes but by detecting them before the crash.
