My problem is usually the low quality voice machine the clinics and offices use . It is hard to understand and always confusing

There can be a difference between a level in an official test vs. speaking capabilities. We would say this is around B1/B2 level.

What is the salary of doctors there, can they do private practice? Whats their salary difference in comparison others. Can someone who’s employed in Germany, say a doctor, can he start his own startup, or business along with job after getting permission from current employer? I am doctor from India and want to move to germany, can you please guide me
To work as doctor in Germany you first need to obtain a medical license in Germany. All the other steps may follow afterwards m. Salary depends on soooo many factors.
