The ice formed on the pipe of the the engine is due to the expansion of the gases (either oxygen or methane), it is thermodynamic property of phase change of any substances from liquid to gas.

Which means the bell is slightly larger to compensate for the loss of atmosphere and is why you get those shock diamonds during static fire tests. This shows slight under expansion at sea level and also means these engines might throttle back during the last phase of flight to prevent overexpansion which also lowers ISP. I dont know how spaceX calculates their ISP but rest assured you're correct in that the above is the primary reason why sea-level engines have lower ISP.

Raptor needs to go Microwave. This will reduce L/Oxygen no need in increase L/O over 80% - 85%. Increasing CH4 Methane tank needed. This can put a loaded StarShip in HEO over LEO.
