    One of the other considerations of masseter injection as a practicing dentist with airway experience is those patients with undiagnosed UARS/Apnea. Nocturnal bruxism( grinding) in the untreated airway patient is protective. Patients unconsciously manipulate their mandibles in an effort to protrude the mandible and open the AP space of the airway to help move air. If you turn off this important ability you can potentially decrease the ability for the patient to open the airway and increase the risk of not waking up. Brusxism is NOT only about the occlusion. Just my 2 cents

      This is very important, and what a great explanation. I am an Aesthetic Clinician, and my husband is a Dentist. I only treat a patient's masseter muscles once they have seen their dentist as there are too many variables, all of which should be addressed before obtaining aesthetic results.

      Treating sleep apnea is very true for me to avoid night time bruxism. I record my sleep and sometimes I will take off my mask during the middle of the night. Sur enough, not only will I hear snoring in my recordings, I will also hear myself grinding my teeth ek. With my cpap on, neither the snoring nor the teeth grinding is happening.

      The action of the masseter is to close the mandible. The lateral pterygoid contributes the protective protrusive vector. (Which would open the airway). Can you elaborate on how relaxing the masseter would contribute to airway compromise? Or Message me directly please?

    This is good to know. I had severe severe TMJ to the point where grinding and clenching was so bad that I have 15 cavities (getting them fixed now). My face was swollen and now since Botox it’s no longer round and swollen. It’s perfect 😊

    The mental crease line is a nice tip, also letting the patient clench lets you visualise the insertion points better.

    Multiple times after getting my masseters done (for night time grinding done by dentists and cosmetic clinic) I have had horrifying side effects. Infection in my parotid gland, rounding of my face as well, as well as a prominent jowl. I have a very thin face and the complications were mocked up to that. Happy to share complication photos if interested as they are very significant. Parotid infection caused swelling from my eye down to my chest, impacted breathing and eating. Resolved after 3 weeks. There was a debate as to whether my injector was not using proper techniques as far as sanitizing or if the toxin somehow was injected into the gland not the muscle resulting in a blockage. I’ve never had issues with my P gland since it healed. I’m not educated enough to make a judgment on what could have caused it, but I can say it was terrifying as it did impact my breathing.

      I think ur jawline bone was not prominent, that’s why ur face got rounder. If ur jawline is well developed, it won’t get rounder, it actually will get more definition even for the cheekbones

    Such an advanced area to treat. Injectors should not be injecting in areas they don’t understand 🤦🏼‍♀️ I do this on myself as my medication causes me to grind my teeth 24/7 giving me headaches. Great content again, very informative

    I am canceling my appointment. At 47, wanting a slimmer face and the end to my teeth grinding - but realizing all the complications (a sinister smile) jowls, nerve complications -- I will ask for a TMJ guard. For the love of GOD. Thank you

    My face is completely asymmetric and I could not really see any major bone structure issue, but touching the left side of my face I felt my masseter was sore, bigger and stronger than the right side. I googled a place to see what could be done to fix this issue that has affecting my self-steem for ages. One session of masseter botox was more than $800, so dare. I myself bought the botox and did the procedure, now I can notice that my face is looking more symmetric and even and I am planning to do again in 4 months time.

      @@DrTimPearcehello, my jawline bone is well developed and not weak, but I noticed my left masseter is bigger. Will my face get rounder or get more definition? I noticed my face is getting wider for both masseters and temporal muscles, and I have a prominent cheekbone but it’s getting “smaller” bc of the hypertrophy of the muscles :(

    Thought only to do masseseter for tight jaw & clenching. She was way to young anyhow, another silly tik tok trend. Of course, we all avoid the risorus muscle. I would only do it for medical rather than aesthetic. Informative, thank you Dr P!

    I have several questions here
    - Does combining massetter botox with intradermal lifting / nefertiti lifting help prevent subzygomatic shadowing or jowls formation?, Is it more effective in sliming patients with round/oval shape face?
    - Why does paradoxical massetteric bulging happen in some case and how to prevent it?
    - I used to do massetter botox to a patient with rectangular / square shape face, in 2-3 weeks follow up, her massetter was very small but her face did not look slimmer, what’s the actual cause and how can I advice the patient?
    - Should I inject more units of botox to massetter if it still overcontracting after 2-3 weeks of first injection?

      Jowls will form because of the fat pad slippage in the midface but on the right person Nefertiti botox can smooth out the jawline but only if that band is pulling on that jowl area.
      The bulging - there are a few theories, it's possibly the superficial fibers contracting without stabilisation of the fibers underneath. Apparently superficial injection can cause herniation of the deep fibres eventually too.
      Massater botox takes weeks to months for the atrophy of muscle. At the three weeks review you can assess the strength and possibly retreat but it takes time to get the slimming affect.

      Hi Mary, my face shriveled up to a tiny wrinkled jowly mess in 1 month with 22.5 botox each side masseters. I had never had it before. Will they grow back? It's been almost 3 months now and there's been a tiny bit of improvement but not a ton. Why would I shrink so fast?
      My temporalis muscles are growing large. That may be lifting and my masseters may not actually be getting better. Actually one of my cheeks caved in and the side that didn't, the temporalis is growing bigger than the caved cheek side. Do you know if I'm ever going to look normal again?

    I had Jawtox , It’s really effective, but since I just tried it, and wasn’t able to get back for another, my lower jaw got jowls, and my face looks not balanced. I don’t wanna try getting botox again. Is there any way to get rid of big jaws wothout surgery?

    I got Botox a week ago and my face yesterday morning was numb on my right side and I couldn’t smile properly. I’m so stressed with high anxiety and I really hope it’s temporary.

    Great video, I was going to get these injections prior to a deep plane facelift. How many injection would you need to get a permamant result? Should I do one session, have the lift, then just keep injecting? I guess there is still a risk of jowling even after a deep plane if I inject too much afterwards right?

    If you’re drawing the line is it still from
    The Tragus to the mental crease? And how many points of injection would you do? I’ve seen 3, 4 and 5. How do you determine the injection points or does it not matter as long as you have the intended units? Thank you!

    I had massaters treated. I clinch 1 side but treated both. I assume there are rules cause I mentioned that I don’t clinch that side and I’m looking to balance it not maintain out of balance. So she insisted we inject both sides. I was right, the side that didn’t need it was so flat and sagging and the side that was stronger was still stronger and the other muscles around picked up the slack and it was painful and it was sticking out and I was still clinching. We tried again and she still didn’t listen and ended up the same. So I still clinch, still asymmetrical.

    Good info Tim! Thank you! Do you find that length of time of relaxation is dose dependent and do you think there is a difference between dysport and Botox in the masseter?

      Under treating will of course allow the patient to still clench / grind in their sleep so trying to the get the right dose can take a bit of tweaking.
      We tend to use botox in this area as azzalure can spread more and this area we don’t want the risk of infiltration of rissorius.

    What is your opinion on tox in the neck to prevent jowls? Good or a waste of time/money?

      I wouldn't recommend it as a preventive measure, or as the the soul solution- jowls are the end point of quite a lot of aging anatomy , and the pull of the platysma is a very small component.

    Teeth grinding has caused my masetter muscle to become very hard and sore. Life has been stressful...but my teeth are being worn down and I awake with a frightful headache each morning. I play the trumpet so have strong facial muscles due to my embouchure. Am I able to have botox to stop me from becoming a toothless crone or will it affect my ability to play my instrument?

    I did it mainly for medical reasons and now I have sagging on my skin. Also it made my TMJ way worse. I try to relearn to use all the muscles. It has been almost a year and even though I can use my masseters my face hasn't completely went back to normal. How long does it take? And I don't want to have any more cosmetic procedures done, should I just wait? I have done it twice in a year.

      The massater muscle is a large muscle and has a certain amount of surface area that keep a face shape. When that muscle atrophies yes you can notice a heaviness to the jowl and less skin tightness.
      Not treating the massaters will allow you to rebuild that muscle and in time your shape will change again.
      Liaise with your Dentist regarding your treatment for TMJ.

      My face has collapsed and I have a huge amount of sagging which I had just had successfully filled by my practitioner. I asked for the masseter injections to help my migraines - which it has but God I look awful - I can’t smile or my lip curls up either 😮

They should tell everyone the side effects before the procedure but unfortunately they dont and then we are trying to fix things with further cosmetic procedures such as fillers.     SOMEONE PLEASE HELP
    My face used to be sculpted, high cheekbones, strong masseters, sharp jaw and it went down into a flat chin. My face was longer, strong and sculpted. I had a square face shape, slightly enlarged by clenching but beautiful nonetheless.
    I then had masseter Botox, it's been six months and the masseters are coming back but they're not as strong. My face now looks wider and shorter than before and not sculpted. There's no definition. No prominent cheekbones. Wide squished shape.
    Will I ever recover? No one told me about any permanent changes. My face has shrunk to 1/3 of its original size.
    My face went extremely narrow and I had next to no masseters at one point like a true egg. All my natural beauty is gone, my face has 0 structure now and it’s been horrible looking in the mirror.
    I am really truly worried I’ll never ever look the same again. Please help will it ever go back to normal?
    My face is a weird off square shape now but it looks ever more square given the structures gone and the jaw is gone so it’s incredibly squished and short. No chin left either now. All volume gone.

    I was stupid too, now I have my jaw cracking every time I swallow or change position of my jaw.. never happened before I did masseter botox

      I did just for slim my jawline but ended with big cracking sensation with little pain.. last about 5 months!I learned my lesson 😅

    Thx Dr. Tim- I suspect I have gotten part of the risorius a time or too. One lady said when she smiled her husband said she looked like a squirrel:(.
    I will adjust the diagram to the mental crease in my mind from this point forward!

    Does this doctor hav a clinic and if so where? Ive developed buldges after botox and need to go see this doctor!! Anybody know if the buldges will dissappear??

      If you are experiencing bulges in the massater muscle it could be that not all the muscle fibres are treated. It may require more toxin but if it’s early days it may be just a little more time to take.

      @@DrTimPearce i no longer trust the practictioner i went too as they did not even inform me that this could be an issue! As well as that when i informed them they called me back in the next day to add a bit more botox in to rectify, it hasnt worked and i feel my face shap is looking squarish almost too.
      Ill wait 2 weeks but would it be possible for me to come in to your clinic to see if somebody at the clinic can help if it still hasnt been solved?

    Yep, I’ve had pain every day since my injections and smaller jawline w jowling. I regret it! I’ve more tension than ever in my massetter muscles and the pain radiates down my neck. I hope this wears off. It’s been 3 months🥲
