I wouldn't use the lord of blood's exultation or the kindred of rot's exultation, those require you to be very close to the enemy, and because of the size of the fire giant, they rarely trigger even when you're fighting in melee range.
I'd also recommend using the jar cannon and a great bow to deal extra damage between poison/rot applications. Also, Black Knife Tiche for the second phase will shred him.
