  • Addressing the prevailing myth that ACL surgery takes 6 months and completing an at-home "return to sport test" to assess progress so far.

Keep at it! You’re doing all the right things. It just takes time, lots of dedicated time. 6months is a long time to be going all out, and it’s hard to hear that there’s even more to do.
I’m currently at 7.5 months and did my second RTS testing (shared the video on my channel yesterday). I’ve had to accept that now I’m finally *ready* to train be prepared to ski again. Everything so far was just to get me to this point where I can even do the relevant exercises.

Thanks a lot for keep posting your timelines! It really helps to prepare to what to expect. There should be enough snow now in NW, hope you could get to enjoy it this season👍

Before my injury I had never really stepped into the gym and had to work so hard to pass the return to sport test! I failed once and it was because of those deadly side to side jumps. You’ve got this!

I'm not sure I could pass this test and I've never had ACL surgery...😬

Do you have an updated target timeline on when you'll get back on skis based on where you are now? Also totally agree that 6 months for RTS seems wildly inaccurate :o I wonder if they mean in 6 months...you can return to some sports, like cycling?? or hiking? But yeah the skiing at 6 months thing seems crazy! I have my RTS test this Friday, fingers crossed I've closed on some of the asymmetries and can ski at the one year mark in Jan!

My target timeline is constantly shifting 😵‍💫 I am now kinda aiming for making real turns on skis ~8 months post op (this is apparently when US Ski Team athletes are allowed to return to skiing so that makes me feel kind of good about that target date haha). But there is no snow yet in the Pacific Northwest so that is keeping me off skis for now anyways! Note at this point I have "passed" two different RTS tests with two different PTs but that is not enough for me to want to put myself back on skis for making real turns - I want my knee functionality to feel effectively even. But I will say I returned to skiing ~11 months post-op last year and I had waaaayyyy more asymmetries than I do now and it was just fine for mellow lower impact skiing :) Hope that helps!

I think for the triple jump test you are supposed to do the 3 jumps consecutively, and only stick the landing on the last jump. At least that's how I have seen most return to sport testing do it; otherwise it's not really a triple jump, right? Just three successive single jumps.

Yep you are absolutely right haha... Just did an official RTS test with a PT and it was a much faster paced triple jump indeed. At the time I filmed this (just last week) I had a hard time firing my jumps any faster than this - which right there is a strong indication not to return to sport haha

Hi Delilah, my name is Wesley and I'm a PT that specializes in ACL rehab. I was wondering if your PT had any discussion with you on isolated strength testing?

I am actually in the midst of "trying out" 3 different new PTs as my first two are not qualified to help me with programming for my next stage of training before returning to skiing. The one I am actively meeting with this week does not believe in isolated strength testing as an objective measure of RTS qualification (which may be a red flag...?)..... The first two did bimonthly isometric strength testing using a handheld dynamometer which I did find very useful (my last results showed 92% LSI in quads and 100% in hamstrings).

had my surgery on june 12th this year. so exactly 6 months. I must say I am way behind than you. But putting in the workouts everyday. When did you get your full hyperextension ?

I know its hard not to compare but keep in mind rehabbing my knee is literally my job as a professional athlete - there is no way I could have made this much progress if I couldn't prioritize rehab and strength training 100% for the last 7 months. And this is my second ACL surgery in a row! I was no where near this strong and functional at 7 months on my first surgery side. Keep progressing and being critical about the guidance you are given - not all PTs are created equal and even I have had to change/add PTs multiple times in my rehab progression

I’ve just done my acl surgery last week. Hyperextension really seem to be dependent on where your graft have been taken from, for my surgery with BPTB graft I was at all full extension right after surgery. But it really hurts to activate quads. It seems to be little different if your graft is from hamstring.
Recovery might be highly dependent on your state before surgery. For me, I’ve not realized that I tore my acl and meniscus and kept playing soccer, snowboarding and exercising in gym for 4 month. Recovery is going pretty good for me, I’m already walking without crutches and mostly pain free.
I’ve also done HBOT therapy, but not sure how much it helps.
Wishing everyone a speedy recovery and stay positive😄
