Keep the water spray controlled - often it is way too high. Also, try moving the mirror a bit further a way. A dental assistant helps but this isn't always possible. Cut a little, the look, then cut a little then look. After a while, "you will see through the spray" like a Jedi Knight!

Today I had very my first patient in operative dentistry today as a dentistry student and I had a very hard time on the distolingual surfice of tooth 24 on indirect vision. The tooth was also rotated so I felt very bad for having hard time trying to actually see and operate at the same time. This video made me feel much better, I now know this can be improved. Thank you!

Great content doc! I have a question to ask. 3 years ago i chose dentistry over medicine. That day I'd had a neck pain and pain in the fingers of my right hand for a week. I thought, i didn't have any major problems with my neck before and I've been feeling this pain for a week so i don't wanna decide against dentistry because of it. But unfortunately the pain hasn't gone away ever since. Doctor said it was probably a bad idea for me to choose dentistry because there should be a problem with my neck (herniated disc) and i went for MRI and doctors said it's fine and i need physical therapy and it helped but after six months the pain came back. My therapist said it's not a big deal just stiffness and trigger points. It's not severe but even walking or standing triggers it. Both stiffness and pain in neck and pain in the fingers of my right hand. Now I have 3 more years to finish dental school and I'm not even sure if i can become a dentist or not. Or even if i should quit or not. I don't even know if it's too late for me to get into med school. It's really hard (if possible) to do that now. I feel like i made a huge mistake. I'm 21. How serious do you think it is? Should i quit dental school? Some people say dental radiology is a good choice for me but in order to make enough money to own my own dental radiography clinic i need to work as a general dentist for some time. It means a lot if you answer.
Dentistry has many options which don't require as much neck bending...like orofacial pain, public health, research, etc. Medicine has similar issues with surgeons and of course many non-stressing ergonomic options. I'd seek additional opinions before throwing in the towel. Best wishes.

Each operator develops their own style. I use a modified pen grasp with subtle movements initiated with my upper and lower arm and sometimes my wrist. I rarely need to use my fingers. If you slow down the handpiece, by lifting off the rheostat, you'll be able to sense more resistance and know how to push, pull, slide lift or drop the bur as needed...

I usually try to position the mirror distal to the tooth and angle as needed. Smaller mirrors can be very helpful, like #2 and even #0. Second molars are challenging. Sometimes we must simply rely on our finger rest and bur position and then "cut a little, look a little" to get the prep completed.

Usually 7 for setting up the line of draw, then back to 11-12 to gain access.

The assistant can blow air on the mirror, but if you are by yourself, move the mirror farther away.

Both are true - line the mirror, handpiece and tooth and your line of site in one plane, which might be a line or a plane.

Wipe the mirror on patients buccal mucosa
fix the water on your handpiece, in stead of sparing water make it drip water...controle it from the chair unite

11 for most in the mirror and 9 for some aspects like proximals with direct vision

I'd use direct vision for the A plane at 12 O'clock and at 9 O'clock for the B plane.
