    I’m a cyclist and a runner .Most of the time I’m ok and I’m so glad to have found your channel, because you’ve confirmed everything I believe about my patellar pain.
    I nearly got a knee replacement but so pleased I didn’t.
    I would like to be able to walk downstairs without looking like a 90 year old which I find very depressing, seeing as I’m a competitive athlete 😂

      How are you feeling 3 weeks later? I’ve had this problem for over 6 months, and my doctor and PT say my quads are stronger, yet my pain is worse. I’ve also been doing some glute work, and my balance is better, but I’m thinking I need to do more and of course, stop working the quads as much.

    Another great video. I spent about a week doing your glute exercises.. It took a bit to become used to isolating that muscle. WOW, did those wipe me out once I figured them out. SO, I feeling pretty good, got
    on my spinning,,,QUADS....after 4 days of spinning,,,Yep, my knee gave out while just walking up stairs and I feel like I'm back to ice and heat before I can get going again. It's been 48 hours and hopefully by tomorrow I can get back to listening to your recommendation. Do you have any ideas on how to turn back

    Hello Dr.David,
    first of all, thank you for all your content and ideas that are more progressive than the classic medicine approach (holds true for Germany as well).
    I (33y old, hobby sports guy, did a lot of running, marathon and volleyball) injured my already not super healthy knee during a turnament, where I hit my foot and knee. According to the MRIs there is more liquid in the knee, but only a minor damage to the cartilage due to overusage and maybe the accident, however I lived in significant pain the last two years, even if not moving the knee at all.
    After adopting your "glute vs quad muscle imbalance ide" as well as feet/ankle training into my worout and finally feel like Im on the right path. Now, after two years of pain following an sport accident (knee and foot) I get to a level, where, when I use kineso tape, I do not have significant pain anymore during my daily life.
    Now my question: would you suggest that I could start playing a little volleyball again with kineso tape as long as the pain is not significantly higher afterwards (same day or days after), or would you suggest that I first need to reach a pain free level without the tape/sleeves before slowly returing to jumping/runnning sports?

      Regarding your question about returning to volleyball, it sounds like you've been making significant progress, especially with the kinesio tape helping to manage the pain during your daily activities. While we understand the desire to get back into sports, it's crucial to prioritize your recovery and avoid exacerbating any discomfort.
      We would recommend proceeding cautiously and gradually reintroducing volleyball into your routine. Starting with kinesio tape as support is a good idea, but it's essential to pay attention to how your knee feels during and after playing. If you find that the pain remains manageable and doesn't worsen significantly, it could be a positive sign that you're on the right track.
      However, we would also advise striving for a pain-free level without the tape or sleeves before fully engaging in jumping and running sports. This may require some patience and continued dedication to your rehabilitation exercises, but it's crucial for ensuring long-term joint health and preventing further injury.
      Ultimately, listen to your body and trust your instincts. If you experience any significant discomfort or pain, it may be a sign to take a step back and focus on strengthening and stabilizing your knee further before returning to more strenuous activities.

    great video! I was curious.....when I went to PT, the guy recommended these inserts that that actually forced my knee the opposite direction because I was having pain on the inside of my knee. Your video mentions to get the arch supports to get your knees inline more. Looking at my x-rays..I am or have lost a lot of cartilage..more so inside my knee. should I get arch supports instead of the other inserts?

    Thanks for the video. I did ask this in another related video, but you again mention 'healing' the chondromalacia patella here, and I wonder exactly what you mean by healing, since that rough/bumpy uneven cartilage (especially if you've had it for a long time) supposedly doesn't actually go away without surgical intervention.

    Is there any research out there on peptide injections for this problem ? I’ve been suffering with front of knee pain from this for around 6 years. 3 months ago I started extensive physical therapy to help the muscle balance / tracking issues. Made great progress but I feel like exploring adding in injection based therapies.

    Correcting the imbalance can get more complicated depending on the muscle imbalance a person has. If it's more caused by movememts in the transverse plane you will have too much femoral external rotation on one side and internal on the other. So the focus will be on different exercises and stretches to balance things out.
    An example of sports that could cause this are surfing, golfing, tennis, snowboarding etc. All acruvities that require rotation predominately in one direction.
    Often times in rotational sports the primary imalance causing issues is going to be imbalances of the anterior and exterior obliques muscles. Research the anterior and posterior obliques slings to get a bettet understanding.

    I appreciate you are busy.
    I have Patellar Chondromalacia and there is also an intra-articular loose body, a small structure around four millimeters in size, which may be a detached fragment of the articular cartilage. The articular cartilage injury is graded as four, The ligaments and menisci appear normal, and I have a Patella Alta, which is a slight elevation of the kneecap that may or may not contribute to the altered biomechanics and erosion of the articular cartilage. The report also mentions minimal fluid within the joint space, which is not of much concern. I see online Dr, You are to be the best around when it comes to Chondromalacia so any advice how to best treat it will be much appreciated as I’m fit 44yrs old and I’ve just got a HA injection but don’t think it’s helped. Again, I appreciate you are busy

      Hey Colin, sorry to hear that you have chondromalacia patella :-( especially at such a young age! This is likely due to overusing your quads for a long period of time. There is a muscle at your knee that not many people know about called the articularis genu, which pulls the patella up causing Patella Alta! This muscle is part of the quads. So, the best advice is to massage your quads very well and get them to relax a bit more while simultaneously working on improving the use of your glutes. Your glutes will commonly be relatively weak when you are overusing your quads and it will help restore muscular balance to your knee. This can help chondromalacia patella by allowing any damage that has been done to slowly heal over time. This needs to become a daily practice for you!     I have grade 3 kneecap arthritis. My IT band is painful and lumpy and so are my quads and adductors all the way to my hips. It hurts to walk. This all became symptomatic after falling on my kneecap in September 2023. I’m not sure where to start my rehab. I have a torn hip labrum on the same side.     Thank you so much, so glad I found your videos. Just got diagnosed with patella alta and chondromalacia (grade 2) caused by it. Every doctor that I went to suggested strengthening my quads is a way to solve the issue...which, I see now, will only worsen the problem by dragging patella up even more.. Will definitely start doing glute exercises and relaxing my quads right away since I don't really feel any pain right now just some tightness in the knee zone. Racket sports is a huge part of my life, can't really imagine not being able to play them. Hoping I will find some relief and eventually solve the issue. Thank you for sharing your knowledge!

    Hello. I have a 1cm x 0.5 cm cartilage defect in the central part of the patella. Chondromolation stage 3 to 4. I play football, I am 28 years old. I haven't played football for 6 months. In 2 months I should have knee surgery (microfracture). My condition is improving now, I can slowly run. I'm interested when you say that quads should not be trained. When can you start training your quads and hamstrings, because I can't play soccer without it.. I want to be a fully active athlete, I also play tennis. That's why I'm wondering when it's time to start training other muscles as well?? Do you think surgery is necessary or not?? I also currently have hyaluronic acid in my knee.. thanks for the reply :)

      Hey Ervin, sorry to hear about your cartilage defect behind your patella :-(
      You can start to train your quads after your muscles are in better balance. You know your muscles are in balance when you're confident that you use your glutes very well in nearly all physical activity. If you're afraid you're going to hurt your knee more because it's already starting to hurt a bit or swelling, then your quads are too strong and your muscles are out of balance. More glute muscle strength is needed.
      Most cases like yours can get by without surgery, but its always a personal decision that you have to weigh based on your needs and belief of how well you can heal. It's great that you got hyaluronic acid, hopefully it's helping you.
      It often takes 3 to 12+ months of training the glutes to get the imbalance properly fixed to take pressure off the knee cap and allow you to confidently play sports. Without seeing you in the clinic to accurately diagnose and treat you, the best advice would be to hold on surgery and go through fixing the muscle imbalance to see how much you can improve. Microfracture surgery has mixed results and you'll need to fix the imbalance anyway or you'll end up injuring the cartilage again even after you've healed from the surgery and return to sports.

    I have patella degeneration and I don’t feel pain, but I swelled up once. I’m trying to learn how to keep it from swelling up. I walk around 13 miles a day for my job. I’m trying to change jobs, but that could take a month. In the meantime, I’m trying to learn how to keep my knee from swelling during that time.

    This info is helpful.. however for me it's too late already. I am only 36 and already have a small narrowing of my knee joint space and a lot of pain to come with it. Not seeing a bright future for myself 😢

      Hey Paula! This method of exercise has shown benefits for some people. However, what we have seen a lot of is overuse of the quads. If this is the case, then transferring the muscular dominance to the glutes may be a more effective method of dealing with chondromalacia patella. Hope this helps!
