Steps to submit a PRISM Case
Download and completely fill out the prescription from the PRISM webpage:

What to send:
• Completed PRISM Prescription
• Verified Soft-Tissue Master Cast
• Verification Jig
• Complete Wax Up or Some sort of duplicate of provisional
• Verification Jig
What is not needed:
• Articulator
• Opposing Arch
Once your case is received:
• We will contact you to go over the details
• Design & manufacturing timeline will be established
