Hello, just a quick question, on the matter of restoring my ex wifes name; Does question #12 on FL-170 have to match question 4F in form FL-180? What i mean is that on form FL-180, I AM asking the court to allow for my ex to change her name back to her maiden name. Where as on form FL-170 IF I mark that the "respondent" box, would it seem that the respondent submitted an answer? Or should I mark the "petitioner's" box since I did the whole divorce on my own? Or should I just leave those questions blank?
