This almost sounds like the image has been inserted in some weird format. Try selecting the image, then pressing CTRL+X to cut it out and then press CTRL+V to paste it back in. Often this already solves the issue. In case it doesn't, try using a different image to see if it's due to the image or a general problem on your end.
