    there was a super simple sollution for even my ultra bad achilles tendon problem that I struggel with for 6 years, after running in barefoot shoes:
    .
    - before every run take collagen, glycine and vitamin c
    .
    - relative "heavy" loaded stretching of gastro and soleus in a calf machine or legpress machine about 2 to 3 times per week in 3 to 5 sets of slow deep end range reps of 12 to 15
    .
    - relative medium loaded stretching the gastro and soleus before and after every run

    Could you tell me what this means, here is my diagnosis…
    Impression:
    Right calcaneus Haglund deformity (mild) with minimal fluid in the retrocalcaneal bursa. Mild low signal right calcaneal apophysis potentially chronic sequela of apophysitis. Consider
    correlation with calcaneus x-rays.

      Sound like you may have irritation where the Achilles tendon inserts into the heel bone as well as a bit of irritation of the bursa. You may find the following videos useful:

    I don’t understand the difference between a flare up in someone with chronic pain and just the chronic pain you have been in for over 3 months. I have had terrible Achilles pain for at least a year and a half, and it’s been off an on plaguing me for about 15 years. For me this flare up started at least a year and a half ago. But you said they usually don’t last more than 2 weeks. Are you talking about a flare up where my chronic pain gets suddenly worse? I generally have pain all the time and I’m going to physical therapy and it’s kind of stabilized things but not getting much better (on the right), but occasionally the pain gets much worse. Is that what you mean by a flare up?

      Yes, it is that occasional worsening of the pain when everything has actually been stable or slowly improving until this point. In your case, your pain system will be on super high alert after all this time. So I would imagine that even simple things can cause quite a lot of pain even though they aren't making the injury itself worse on a tissue damage level.

      So sorry to hear of your situation. Yes, alas, you currently have chronic pain, and as Maryke says below, a flare-up is when it gets suddenly worse. Great that you are getting physical therapy support, but understanding pain and specifically the potentially many beliefs, thoughts, emotions and actions that are causing your pain would be a helpful way to move forward.
