I wish to write something about the word Folate, the etymology of this word.
Folliage means leaves, folate is a vitamin derived from leafy vegetables like Spinach, the word follicle is used to mean "a little bag", in olden days little bags would be made out of leaves.

1000 daily for 1 week?? here in holland the doctors do 2 times for 5 weeks, when really necesarry. what would be better you think?? i thinks it s all about looking how the patient feels. and i feel 0.0 improvement after 8 injections now

If it is due to vitamin B12 or folate deficiency, life long vitamin B12 or folic acid supplement is required according to the cause. Please visit your physician for more information. Hello Sir, I was searching for congenital dyserythropoietic anemia diagnosis, and found your videos. My nephew is suffering from congenital dyserythropoietic anemia(CDA). He is 2 year old. Local Doctor say for blood transfusion every two week.
Would be very helpful if you highlight some suggestion or any help..

Narayan Jalan Hello, congenital dyserythropoietic anaemia is a rare disorder where there is ineffective production of Red Blood Cells ( RBC) and less than normal quantity of hemoglobin in the blood. As suggested by your nephew's physician - such disease do need frequent blood transfusion in frequent interval ( e.g. 2 to 4 weeks interval ) because the patient does not produce sufficient RBC by himself to meet his body's demand. At the same time chelation therapy is also given to remove excess iron as a result of frequent blood transfusion. Sometimes Bone Marrow transplant is considered but it has it's risk to consider as well. That is as far as I know, since I am not a haematologist - I will recommend you to follow your physician's advice and follow their treatment guideline strictly and ask for their advice and opinion. Wish you all the best and apologies for late reply ( I live in Rajshahi 6 days as a non resident post graduate trainee of MD Pathology Course and net connection is not good here always, I stay in Dhaka (the capital) for one day and that is when I get high speed net for youtube uploads)

Fixed acids are produce due to metabolism. They need to be kept in constant check to enable the body to keep a constant environment. These acids are regulated by bicarbonate via a reaction that produces carbon dioxide and water, they are regulated indirectly as once the fixed acids enter the blood stream they dissociate into their respective hydrogen ions, it's these hydrogen ions that combine with the bicarbonate to produce carbonic acid that will disassociate to form carbon dioxide and water thecarbon dioxide gets excreted via the lungs, this reaction uses up the bicarbonate and returns the levels to the normal levels. Iftheirs is an exces left the kidneys will excrete the hydrogen ions.

first ;a comment you did not mentioned the iatrogenic causes of folic acid deficiency such as methotrexate and sulphamethoxyzole
second a question about the difference between folic acid and folate and why the size of RBC becomes bigger in megaloblastic anemia and what are the steps from the megaloblasts to macrcytes

Thank you for your question. The short answer is "nuclear-to-cytoplasmic asynchrony". It means in megaloblastic anaemia the nuclear maturation is delayed while cytoplasmic maturation and haemoglobin accumulation proceed at a normal pace. This leads to hypersegmented neutrophils, even megakaryocytes may have bizarre multilobate nuclei due to same reason.

sir i have doubt.... is megaloblastic anemia and macrocytic anaemia same means when asked to write in exam any one of them we can write same???
Thank you for your question. In macrocytic anaemia, red blood cells are abnormally large. MCV > 98fL. It can be divided into megaloblastic and non-megaloblastic. So macrocytic anaemia is a broader term. It can be either megaloblastic or non-megaloblastic based on appearance of eryhtroblasts. Take care and stay safe.

Why hypersegmented neutrophils are seen in megaloblastic anemia sir?
Issues with the DNA synthesis results in Uradine insertion rather than Thyamine, resulting in hypersegmentation of the nucleus. This is because Vit B12 is required as a co-enzyme to convert homocysteine to methionine necessary for the conversion of folate to thyamine. Without either of these, DNA synthesis does not properly occur.
