Nothing feels more bad of a game design that shoving QTEs in that the player can literally fail at every one and it affects nothing... or do all of them and affects nothing. Look at Mortal Kombat X as an example of shoved in mechanics and designs that mean fucking nothing. That's failure of the game design. Having bad controls, controls that don't work, mechanics that are executed poorly, mechanics that contradict with other mechanics where in it makes a mess of a game, either really repetitive and boring gameplay or extremely unfair and bullshit gameplay, is failure of the game design.
