We will see a patient with end stage liver failure and a very exciting procedure to stop esophageal bleeding (TIPS procedure). Come with me to the ICU, emergency department and even my call room!

      Monika S If I’m not mistaken, internal medicine is 3 years and then rheumatology is 2-3 years (depending on the program), 5-6 total. So I assume she’s got 1 or 2 years left if she’s a 4th year right now.

      haha there's no doubt that is a serious inconvenience. At this hospital we still write notes and orders by hand, so the call rooms don't have computers... although if/when there's a new EMR i'm sure we will get computers in the call rooms. I also wish they were there already! haha

      @@gingerlynoise many hospitals in Canada still don’t have emr or online charting system. During nights the nurses often write verbal orders after calling doctors

    Right now I am a EMT. Yesterday I worked with a paramedic for the first time and I put on a 12 lead ecg on a patient and I helped him spike an IV it was so exciting it makes me want to become a doctor. Thanks for posting these really inspiring!

      Where are you from? I'm a RN in PA, USA & my boyfriend is an EMT & it's standard practice for him to do EKGs & prime tubing for the medics, he can even do finger sticks!

    My grandma dealt with liver failure for the past 4 years and eventually passed in October of 2020. She had multiple procedures and a TIPS as well. It was nice to see the other side of it and reminded me of everything she went through

    the whole road to residency really overwhelms me sometimes and makes me second guess the path I've chosen to strive on, but your videos make the destination seem so great. I really wanna do this!! Amidst all the match week frenzy, I felt so down and scared of failure but this was a pick me up

    I wish there was a dr like you when my mom was in the icu with septic shock. The one we had was good medically wise, but had zero bedside manner, and one of the first nurses was so rude. My cousin was an icu nurse in a different state at the time and he called me that night to check in. I was in tears because of that nurse and Dr. The treatment towards us, and the refusal to answer our questions was insane. We had put him on the list to get info on my mom so he ended up speaking to the head nurse, and an administrator about the nurse. My mom's general surgeon who worked with a completely different hospital would call in nightly, and then call me. She actually also reported the nurse. She spoke to the Dr we didn't like due to her coldness and agreed that Dr needed some bedside manners since she was in the icu. Between my mom's general surgeon and my cousin we didn't have to ask questions, they knew what to ask, what needed to be done and ect and then would call me at night and relay everything.

    When I worked at the hospital in my town as a transporter, moving patients to and from the ICU was always so nerve wracking
    So to know that you personally helped take the patient back is amazing! ❤️

    My uncle just passed away yesterday due to oesophageal varices from portal hypertension due to chronic liver disease. It's just eerie to me that after living through hell for past 24 hours, this is the topic of your video.

    I’m in school to be a cardiac sonographer and my test Monday is on heart murmurs. What a coincidence. Love when heart things pop up and I fully understand what’s going on ! Love your videos!

    I have so much respect for you and all the hard work you and every icu staff members around the world do. I myself have been home for a few days after two excruciating weeks in hospital and i have major respect for healthcare workers more than ever. Your videos are amazing

    As someone who is post transplant liver failure is something that scares me every day!!! But at the same time I am so grateful for my Donors and their families as I have unfortunately have had to have 2 transplants now, the last being in January. (I was born with a rare liver and kidney disease called hyperoxalouria causing me to need my first transplant in 2015) unfortunately over the 6 years i had many complications mainly a blood clot in the main hepatic artery to my liver causing causing me to need my second transplant. This transplant is going really well along with my recovery 🙂

    These video's remind me that NO ONE is immune to getting ill or sick or injured and the fact that there are so many wonderful Doctors and Nurse's and training interns in the hospitals like Siobhan and Alisa who are intent on helping to get their patients get well leaves me feeling confident that we ALL will be okay!     Love your explanations! When you speak about infection of the heart ❤ makes me see 👀 that cleanliness is important on a variety of levels even if giving blood or receiving shots (new and a reminder all rolled into one). Thank you for all that you do!

    That feeling when you’re on call and having a quiet night with no admissions or any trouble in the ward is UNRATED! 🥺😪 I got a solid 6hours of sleep during my last call and I couldn’t believe it🤣😂

    Infective endocarditis- I had that. One replacement valve and a year and a half later I can go "Wooo"! at home just like it was a stand up set and the comic was asking if anyone was from Edmonton.

    Thank you for being so kind with your words when talking about the patient with the heart murmur. I know many who would not give drug users any respect. These are just people who are struggling for whatever reason. My brother was an addict and so I appreciated the respect you showed the patient. Love your vlogs 💖

      My mom always says, "Everybody has a hard time. Are you gonna be the person to make it worse, or help." She just got her B.S. in applied psychology. She always wanted to be the person to help others when they need a friend, now she can do it every day.

    Portal hypertension is no joke. Had bleeding varices that almost killed me (BP 65 and uncontrolled internal bleeding)...and ascites of 7-8 liters drained each week. Spent 9 days just in the ICU. Once I was stable enough had a TIPS procedure and even though it took three or four months to really take hold, my last EGD showed only a couple minor small varices. And only a liter or so of ascites too small to really drain.
    Credit to the ICU docs but especially the ICU nurses for taking such good care of me.

    Thank you so much for taking us with you - I almost feel like I met these patients myself. You have a gift of explaining situations and procedures so an everyday person gets an idea of what's going on. I bet you're an awesome teacher to your med students.
    Question (I'm Swiss, so apologies if that's a dumb one): does every Canadian have health insurance? I was wondering who paid for the drug user's expensive tests and treatment.

      Great question Tamara - Canadian health care covers all expenses in the hospital including procedures, medications, physician fees. Outside the hospital, visits to the doctor and (most) procedures are covered, but not all medications... that can be a challenge

      @Lateisha M I am from Canada, and yes you’re right. Everyone has the right to health care in a hospital, but you need insurance for medications (or things like insulin) and some specialist (physio, chiropractor, massage) or non-emergency procedures (e.g. cosmetic surgery).

      @@foxcrow Not just the right, but it covers the cost as well. Here in the US, they have to treat you in the emergency room even if you have no insurance, but you can/will become bankrupt afterwards, so for most people, that still isn't an option. Have to choose between dying or bankrupting yourself. (And possibly even your family left behind)

    I was just curious how you handle withdrawal issues from the guy that did IV drugs? I can only imagine what his body would go through unable to “abuse” it as he was doing. I have been watching you since you were a 2nd year resident. You are so amazing and I have learned so much from you. I also have RA so really thrilled you picked rheumatology.

    Hi Siobhan, thank you for uploading these videos and giving us an insight into what it's like before the COVID pandemic, during and I'm sure after the pandemic. i had my 2nd dose of the Pfizer vaccine today . Very painful arm right now but it will be so worth it to know I will soon be so much better protected from COVID. I believe we will beat it

    One side of being a doctor that I've always been curious about is the friendship between coworkers. I'm on the spectrum and in premed. The only thing that I'm really affected by is my ability to form friendships with my peers. Do those who struggle with friend making have a hard time as doctors? It's the one thing I worry about, becoming a doctor.

      As a med student, I've noticed making friends is easy since everybody is in the same boat, but the quality is what matters the most so that takes some time.

      I don't think making friends is critical to being a good doctor. It is very important to be able to work with others and function in teams, but I think those are different skills than making friends. I think you will be pleasantly surprised!

      Hi, I'm am autistic second year med student. I'm still a baby and haven't been in clinic too much yet so I can't give you that much insight, but from what i've experienced so far, it's kind of the same as the rest of life. I've always been able to work in teams and i'm very empathetic, so in my pre-clerk rotations i've done well and felt confident. I get along with my peers, but at the end of the day I'm very tired socially and I make few lasting friendships. Basically in my experience it's no better or worse than the rest of autistic life!

      @@icecrystal34 That is really helpful insight! I'm the same, I function fine in groups, I'm very empathetic, and I really love being around and helping people. Like you, though, I feel socially drained by the end of the day and while I enjoy the company of my peers, I dont really make any sort of lasting friendship or bond that exists outside of the school/work interaction. I dont mind that I struggle with this, it is what it is, but I'm glad it doesn't have a huge effect on being a doctor. Thank you!

    I got my first Covid vaccine a few weeks ago. I only had a bit of a fever a few hours after and a bit of fatigue for the rest of the day. I feel great now and I am happy to have this security blanket while working in a hospital so close to many people everyday. Anxious to get my second dose!

    I am very young I'll be 29 in February been watching you since the start very proud of you, unfortunately I have a lot of medical issues and was recently diagnosed 5 months ago with moderate to severe liver cirrhosis I go to Vanderbilt this week keep me in your thoughts and prayers

    I'm almost 4 years clean from a heroin addiction, and I have a good friend who unfortunately still uses. He recently got infected endocarditis like the person in this video. I wonder how often this happens to heroin addicts because I haven't even heard about this until recently. And hopefully it doesn't cause permanent damage?

    My husband had endocarditis 2 years ago at 34 years. It was terrifying as it took them 12 hours to do an angiogram. Thankfully he recovered well
