If u Make a follower who has a follower form you can only get once (Sozo,All 4 bishops and TOWW) mate with another follower u get those things,this probably exists to prevent multiple Leshy or Narinders running about in ur cult.
you need 5 silk and 20 gold bars to make it if you’re talking about the dlc it’s not in it, it came with the update I bought it because I wanted a hoard of ladybug followers

not always when you first have a baby it plays but it turns back into the regular music when you have other :(
