Some Sega Genesis/Sega Mega Drive Rom's (like Streets of Rage 1) are Multi-Region: The same Rom released for USA, JAPAN and EUROPE for example. The difference is that on USA/EUROPE System the game will display the language in English. On a JAPAN system the game will display the language in Japanese. Sega Genesis/Sega Mega Drive Emulators (such as: Genesis Plus GX) has on option to set the system to desired region.
In order to play the Japanese version of Streets of Rage 1 (known as Bare Knuckle in Japan), you have to set the REGION to JAPAN first.
