Wouldn’t they have wanted to see their son in person before deciding to pull him off life support? Even so, the hospital did a horrible thing.

1989 raises a good point-especially coupled with the fact that the people involved seem to be elderly and it sounds like a couple of them did ask to see the body at least. THIS is weird though, for sure.

Mike is 60. His parents are almost certainly 80+. Maybe they don't have the mobility.
You'd be surprised how many things you have to give up when your body gets old. Saying goodbye to your almost-retirement-age son in person is just one of them.

The family interviewed were his sister and brother-in-law who were clearly not in their 80s.
I worked at this hospital as an RN and there was a similar incident where 2 patients were mixed up and one almost died. I was on the Sentinal event committee on this. I can't believe it happened the same hospital again..
Lawyers will be looking for you to assist them on trial if it comes to that,...just make sure u get a hefty cash deal before engaging....so stick with the truth n get paid....!!

I had lab results mixed up at the hospital once but a nasty nurse had made comments about my sample…and luckily that’s how I was able to advocate and verify the results that came back must not have been mine! I’m glad that nurse was rude or else I’d have had no idea and just accepted the wrong results as truth!!
When I started working at this hospital I thought all of the stories about mixing things up were told to scare us and very rare but they are not rare and it's becoming worse not only here but in other places as well. Its a very Capitalistic business and I stopped working as a nurse a few years ago because there is no Q.A., only bullet points to be sure never happen so insurance will not withhold payment.

This family needs to check with SS Administration to find out if he has been declared Dead in their records. He won't be able to collect any benefits in the future if not fixed NOW!

@jimfrazier8611 Canadians don't give their SSN for medical purposes, we give our Services Number in BC. SSN is strictly for government programs and benefits purposes.

When I worked in a hospital there were multiple checks in place to prevent the "wrong" patient from getting medication. I find it unbelievable they wouldn't do multiple cross checks before "pulling the plug".
I was in the hospital in the UK and they placed a mark on my leg for identification and wrote it in the documents, every time they gave me a shot, they looked at the documents, then looked at the mark (a circle on my left leg) then gave the shot.
If he did'nt have good heath insurance, they don't care.

Well, obviously it wasn’t Mike who called the ambulance for himself because he had passed out while choking.
His roommate was the one that called.
In my experiences with calling for help like this, the operator always asks for the relationship the caller has to the one needing help or ask for the callers name.
So the operator would have had information that there were more than one person living at that residence.
The paramedics would’ve had to have been let in by somebody so obviously it was the roommate that let them in.
Therefore, the paramedics could note that there were two men at that house at that time… The patient and the person that called for help.
Paramedics share information/write notes for doctors to read about the patient that comes in.
With all the information available to establish that there were two people in that residence and two possible identities to go by, this should never have happened.
With proper oversight, this would not have happened.

My husband was in that hospital after he coded at a nursing facility not far from there. After he was stabilized the Portland Veterans Hospital contacted me and told me that they needed to get him out of there as fast as possible because PeaceHealth had a very bad reputation and was not a good hospital at all . They told us that if they didn't get him out of that hospital that the hospital would probably kill him. This was back in 2010.

PeaceHealth dismissed a future patient, with hospice and 3 months to live. He was my patient for 14 years thereafter, dying 2 months after I retired, and he returned to the heroic, brutal and. Yes he was terminal in the incompetant hands of PeaceHealth. Hence my retirement from a broken health system 8 years ago.
I worked for Peace Health and witnessed some extremely terrible events,I wrote about one similar to this in comments a few minutes ago.I have 30 yrs experience in nursing.
You would have been frustrated if you had stayed. Now doctors cannot even treat patients off label. A sad situation.
I have heard negative things about Peacehealth. Keizer Permanente is a negative one as well. Humanity is a number and the elderly are pushed through like livestock to be butchered. The abuse that happens to the loved ones that intrust the institution is a death sentence.
So if the -good- -concerned- uncomfortable people who work there just quietly step aside then what should you expect? Would a good person just step aside quietly?

What the frick they need to investigate this place and get answers,l pray that they do 🙏 and that the families get some peace and get compensated for their suffering.
My family were called and told my little sister had passed. We were told by a medical examiner and a detective that they had positively identified the person as my sister. For 12hrs we believed that. Until she randomly called, which was how she communicated normally, months and months would go by without hearing from her. The ME tried to say my mom was in denial when she called. We had already made arrangements to go out of state and claim her body. Turned out another girl had taken her id. The id verification was only a visual comparison.

Which yeah, okay, it was during COVID… but not even a video call… Also most hospitals even with strong restrictions allowed visitors if someone was imminently dying, and you could always wear higher levels of PPE for protection if you were particularly worried.

@dawnjackson2861 losing someone is personal , there is no wrong way to do it , you do it the way it works for you and others do it the way it works for them . Some people dont want to see their loved one like that , to have that be their last memory , they want to remember them the way they were . Some people are also not emotionally strong enough to watch a loved one die in front of them .

So, the man is in the hospital for 3 days, and the Beehler family didn't even go to the hospital to see him before he died? The room mate of this guy didn't go to the hospital to check on him? He didn't bother to contact the room mate's family to let them know what happened, or to check on him? Don't they have pictures of the patient in medical records in Canada? And they don't ask about organ donation till they get him to the funeral home? That's way too late for that to happen. Sounds to me like there's not a lot of common sense going on there.
This didn't happen in Canada. It happened in Washington State, USA.

And since Washington State is run by a very progressive governor, everything was locked down here three years ago. I had a friend with a parent who died in a different hospital, and my friend was not allowed in to see her dad at all. It was awful.
They ALL seem brain dead.
There was very active Covid and no one was allowed to visit people in the hospital.
I was in the hospital in a room with an elderly lady who had hip surgery. They almost gave her medication that was meant for me. Thankfully I had woken up to hear the nurse saying my name to her if she's ready for her medication. I quickly and loudly questioned the nurse, scolding her. I also let her family know, when they came to visit, about the mix up.
It could have been prevented if they check the patient's wristband with a reader that goes on file what medication was prescribed to the patient.
Stupid human error.

They mixed up my identity several years ago. I was taken there in an ambulance. The nurses and doctor treated me horribly. I found out weeks later that my paperwork got mixed up with a drug addict.
That's horrible. The way drug addicts, suspected drug addicts, and former drug addicts are treated is disgusting. I understand compassion fatigue and medical staff need to do their due diligence and keep the Hippocratic oath and do no harm. A friend of mine was very recently ignored at 2 different ERs because he's A former addict and ended up with an antibiotic resistant infection in his hand that was so bad that the were considering amputation. His dominant hand is useless now and he's still dealing with other complications. He's not even 30 years old and his hand will never work properly again and will probably continue to be useless. The infection made it into his bones before he was taken seriously and he was still not treated very well by some of the staff. I've read his chart notes. It very clearly states they suspected he was seeking drugs. Its criminal negligence but I'm not a lawyer so what do I know? Still...
A hospital did the same thing to one of my friends. They treated her terribly, & she was so sick. Her family took her to a hospital in the next county over, & they took her straight to surgery. I’m sure she would’ve died if she had stayed in the first hospital.

And its gotten beyond ridiculous the conflagration of appropriate use of appropriate prescription pain relief targeted while street fentanyl and heroine pour into our country being unaddressed because the only data they can obtain and present to prove action is from the legally regulated prescription meds- leaving legitimate use being targeted, vilified, blamed inordinately.

If it wasn’t stated that the deciding family members were in the same area, maybe they lived out of the area and couldn’t afford to travel last minute. But also, COVID protocols at the time need to be considered. And that being said, they are not young, they also had to watch their own health likely, as well.

@@shecat1964 I thought the same thing but I guess, let’s be real; some people don’t keep in contact with their family. It doesn’t necessarily indicate a lack of love; there are some people who just don’t like to be contacted. That doesn’t mean you wouldn’t be sad to find out someone you love has died and especially if they had died in this way.
Years ago in Medford Oregon I was a volunteer for the medical records. I reported a mistake that there were two different men with the same name one was young and the other was old, their middle name were different, so both age and middle name were a red flag. They removed me from the record filing department soon after.
Wow I'm sorry they didn't do the right thing but thanks for doing your part. I had two patients with the same first and last names. They just moved me to a different unit when I notified them that mix ups were happening with their meds. One had heart conditions, the other was only there for pain... imagine the side effects that could happen later.
It's a common problem. The EPIC electronic medical system makes it worse. If they register a previous patient as a new patient for a new admission, it is impossible to see the old information. And EPIC doesn't allow the merging of old and new charts during an admission. This is a known issue with EPIC, but they decided not to implement changes.

@@nolabae True. All software systems have bugs, that's a given. The problem with EPIC is that they intentionally made it so you can't merge records while a patient is admitted to the hospital.
I'd love to be the malpractice attorney who, through discovery, finds out that they would rather pay claims than fix the software.
I'd be angry with my family if they decided to pull the plug and I ended up surviving and found out what they've done.

No, Mike Beehler was not in the hospital at that time. They had a patient on life support that they somehow thought was Mike. The hospital had record of Mike as a patient previously, so I'm guessing they saw the address where the EMTs picked up the current patient matched Mike's address, so they assumed it was Mike. However, the patient was Mike's roommate, David Wells. I'm guessing David didn't have any ID on him when he was transported to the hospital. So, I can see how the mistake was made, but the hospital should've asked a family member to positively identify the patient before pulling the plug.

Yeah some years ago I went in there, Hella sick and in a lot of pain. The doctor came in and told me we’re not giving you any pain medication there is nothing wrong with you treating me like I was some kind of junkie I passed out in the car on the way home burning up with 105 fever anyway the next morning I went to my primary urgent care and found out I had Pilonephritis Horribly bad and end up getting admitted to OSHA for nine days! My kidneys were shutting down! BS Hospital I’ll never go back
It isn't just there, healthcare providers are not pushing back and advocating for patients with legitimate needs and taking the easy, cowardly way out of the pressure placed on them due to the opioid crisis conflating legitimate use of prescription meds with illegal use and substances. Easier for them to adopt the stereotyping and mindset than to negotiate their duty to be advocates in the care of patients and push back on the scapegoating and pressure put on them.

Writing this from a hospital bed, I found the decision this poor family had to make just gut wrenching.

Many families are not notified if their loved one is sick! Some live across the country and have lost contact. Not all families choose to be estranged. Sometimes the patient has walked away from the family, not realizing there will be no one for them if they are incapacitated. It is still the responsibility of any treating provider to contact family of the hospital admission. And a professional hospital would give regular updates as well. But in this case, the Peace Health admissions mixed up these two patients. So they should have verified each person before attaching a name band to them. Verification is confirmed through ID /drivers license with personal belongings, and social security numbers. OR contacts that are recent, ie employers, landlords, roommates, or neighbors. Yes, this called due diligence, and that’s why social workers have a job!
Who knows if the ventilated patient would have survived if not for early extubation?

I think they were notified he died, but were unaware of the mixup and that there had been a decision to take him off life support made by someone else.

Not all families are close or in regular contact. There are a variety of possible reasons. Maybe the dead guy was a reclusive alcoholic. Who knows. That does not have any bearing on the family's grief in finding out he is dead and someone else was given his end-of-life decision- making power.
They know he died back then, they’re just now finding out about the mix up and that someone took him off life support.
@roberts6703 First, you should try comprehending the facts in the video. Second, getting accountability out of the medical bureaucracy is next to impossible. I went through similar circumstances with the death of my father here in BC and the hospital/government covered everything up- and since it happened during lockdowns, and it probably happened BECAUSE of the things they were doing back then behind closed doors, they just hid behind those magical rules that they made up to protect themselves. In fact, the RCMP even threatened to interfere with my livelihood by taking licensing away from me if I kept asking questions.
A friend of mine coined this phrase, "Incompetence is the new normal." A day does not go by with out this being confirmed several times per day.

@@Aochic I don't mean this as an insult, but some people aren't functioning mentally as highly as most other people, so wouldn't think about checking up on his roommate.
This was during Covid, they could not visit in person. The family whose relative did die, the hospital told them he died shortly after his death, once they properly identified him. It wasn't until three years later that the reporter told them he died because he had been taken off of life support mistakenly by strangers. The hospital didn't disclose their mistake.

This was 3 years ago during covid restrictions. The family was probably not allowed in the hospital. The right person was taken off life support. He was listed under the wrong name, and the wrong family was asked.

The news report is really unclear, but that is not what happened. Mike has a roommate. Mike goes out of town, and the roommate chokes on food. It is still not clear, but someone, the police, the ambulance, or the hospital, decides that the roommate is Mike. Within a few days Mike's family is asked to decide to pull the plug. A few days later the mistake is discovered and the roommate's family is notified of the death from choking. Three years later the reporter tells them the details about the mixup that no one else did. There was no one in the hospital for them to visit. They buried or cremated their relative 3 years ago.
That horrible event is called a “sentinel event”, which is required by law to be investigated. I’m sure the event was investigated and the investigation paperwork lost or misplaced.
Wait ..what? his family was going to have him taken off life support without even going to see him for one last time ? that is just crazy , I guess if you were very poor and could not raise any money to get there would be a reason why but they said they went to the funeral home so they must live close by and if they were in another area they had money for a funeral service etc. which costs alot so they certainly had enough money it seems .....what ? no way in hell do I not go be with my child as he passes on

If you are going to turn off life support for a family member, wouldn’t you actually go tho the hospital to visit the patient to say your goodbyes BEFORE the plug was pulled! If they had done that, they would have seen that the patient was not Mike! Personally I’d be pissed off that my family would decide to just flip the switch without visiting me first. Hospitals and health care professionals are too well protected by law. They need to be held accountable for screw ups like this!

I still don't understand making end of life decision without seeing the person first hand. If the hospital calls with news like that, first thing out of my mouth is don't do anything until I get there. The entire story is beyond shocking.
It happened during all the Covid lockdowns. Washington’s governed kept the lockdowns going as long as possible-we were one of the last states to open up.
Why didn't Mike's family visit him in the hospital? They make a life and death decision without physically seeing him in person? That's crazy to me. If they had seen him, they would have alerted the hospital staff about the misidentified man.

Our dad was on the hospital. I saw him in the evening? He was recovering nicely from pneumonia. At about 2 am I received a call my dad passed away. I asked the doctor what happened. Dr. simply stated , “he stopped breathing. He was a DNR”. I said No he was a full code. Dr. then hung up on me. A nurse then called back and said my father had not died, his roommate had. Policy changes were immediately put in place that only an attending MD could call family. NEVER a resident! Absolutely horrifying!
I can’t imagine what Mike must be feeling knowing his family pulled the plug on him, over the phone no less, and yet here he is alive and well. I hope my family is more willing to fight for me and give me time and actually show up for me than this poor guy.
As for the hospital, this is nothing extraordinary. Stop trusting them with your loved ones!
Not surprising. I had a knock down drag out encounter with a Nurse will being prep for a right knee replacement... she mark my left knee with a black marker after I told her no...its my right. I finally grabbed the paperwork from her hands and showed here the obvious notes. Had a dozen plus "oops" with Medical Providers, Drs. Nurses, Pharmacists, Accounting and biling over the years.
I’m in Vancouver, B.C. and have had surgery a few times. Each time I was asked at many times what I was in for. The nurse in the room, the person pushing the gurney to the operating room, the guy administering a block and the surgeon all asked me. For my arm I was asked which side I was to be operated on, so it blows my mind your nurse could mark the wrong knee.
A nurse is not the one who is allowed to mark the surgical site. It has to be a doctor

This is as crazy as the cleaner that unplugged someone life support many many years ago to polish the floors. Nurses called it the death bed until they realised what was going on, nobody knew not even the cleaner.

The real victims here are the true beavered family. Mike’s family should have requested to go and see the patient before making such a decision. When my sister was ready to be unplugged, everyone was in the room with her, including her kids, husband, and siblings, except for our dad, who, on health conditions, could not make the trip. This is egregious on all involved except for the deceased’s family.

Considering what has happened during the last 3 - 4 years during a so-called pandemic, with a total lack of "inform consent," why doesn't an issue like this surprise me. How much of modern medicine is based on science today, as opposed to money?

To me the craziest part is that they just got a call saying thier son was braindead and they said ya ok just pull the plug , they didnt want to got to the hospital and see gim before he passed . Maybe i missed something and there was more to it
My personal experience is that, if they can get away with not telling you they did something that harmed you, they won't say a word. Sadly I paid a price for it, not aware of what took place during my surgery.

Ummmmmmm........ why didn't they go to the hospital TO SEE THEIR LOVED ONE? They would have recognized if it was their loved one or not! Seems like it was the fault of family who took him off life support!!! They CLEARLY DID NOT EVEN GO TO THE HOSPITAL TO EVEN SEE THEIR "LOVED ONE".

There are all kinds of questions here. Why weren’t you at the bedside of your son as he was dying? All of this may have been avoided. Why is the other family acting so upset? They hadn’t reached out to their son in 3 years? Doesn’t sound like they cared a whole lot anyway. I may be mistaken, but were these men possibly possibly special needs individuals? I work with such, and it’s not unusual to see the family distance themselves.

@@millahnnaAug 2021 was not peak covid restriction. The peak was during the year 2020. There were still restrictions in 2021, but they were more relaxed, people had to wear masks at all times when coming to the hospital. People weren’t banned from coming to the hospital
This was in Washington state. Governor Enslee still thought it was peak Covid. He had the state locked down until later that year.

Don't those who love the person attend at the bedside to say goodbye and respect their relative especially when they have agreed to "pull the plug? Getting nothing more than a phone call about finishing a person's life and then saying, OK go ahead, and leaving it at that seems to be a very cold way to treat your son. This is a very bizarre story from the POV of the guy who rang the following week only to be told he was supposed to be dead. I truly believed that some form of signature and verifiable paperwork would be necessary when taking a life. Strange.
Who called the 911? and gave the EMS wrong name...??? Did they have different "health insurance"...

I went to that Peace Health for stabbing pains associated with my periods (possibly a rupturing cyst, but I never found out for sure). Doctor gave me a prescription for laxatives. Mind you this was after he said I was lying when I told him it couldn't be appendicitis because I had mine out a decade before.
That sounds like a doctor just dismissing a woman. Didn't they do any tests on you. Hope you're are doing better now.
@@tansywelsh1636 THey did xrays, bloodwork, and internal ultrasound. COuldn't find evidence of ovarian cysts (which I have a history of but it was a long time ago). Said I was clearly constipated based on exrays (I was not and specified such at intake). He didn't seem to even look at my scans. I'm finally done with some nice early menopause and the stabbing pains finally stopped. I'll probably never know what caused them.

@@tansywelsh1636 THanks to some nice early menopause, it's a problem I never have to worry about again. He did run tests but barely seemed to look at them. Said it was clear I was constipated on my xrays but my intake questions made it clear that was not my problem. I had a history with cysts many years prior but I'll honestly probably never know what caused the pain every cycle.

REASON: the reason it happened was that the medical system had records of a person from the same address treated by that same hospital, so when the steak choking victim was transported to hospital, he went without any ID on him, so the hospital looked up the address where the EMF picked him up, then assumed there was only ONE person living at that address, and according to their previous hospital records, they had only ever treated ONE person at that address, so they assumed it was the same person.

Had to check, this happened in Vancouver Washington, which looks like it should be part of Portland. A US hospital, not Canadian.
Vancouver is across the Columbia River from Portland and is in Washington state.

Just a lesson how family should be involved with their critical care loved ones in a hospital. Doesn't sound like the man had anyone until lawsuit time.
The people need to tell us this happens, the hospital only wants to cover it up. This happened 4 years ago and we are learning it today. Call this hospital and complain till they cant handle all the calls.

If I was them I would be suing the hospital and the mortuary with both families as the complainants since they now have to live with this egregious mistake. What a horrible burden to bear for all of them.
The statute of limitations is probably up on this case. I don't know what it is in that state, but here it is two years.
I don't know if funeral homes independently confirm the identity of the deceased but I imagine if a body arrives and the hospital tells you it's John Smith, unless it's a woman on the gurney or otherwise obviously not a match with the death certificate, they pretty much assume yup, that's John Smith.
Tragic. And all because of one mistake: wrong identity. This tells us that anytime we or a loved one is hospitalized someone needs to make sure the patient is clearly identified.
I can’t make sense of this story. You get a phone call from a hospital telling you your son is brain dead did you want to pull the plug and you just say “yes” over the phone ? Did they never visit him in the hospital? 😬

What I don’t understand is the fact that 911 was called. 911 asks the caller for important information, such as the Injured or affected individuals name, approximate age, Circumstances, address… Was that not done here?
If that wasn’t done, the first on the scene could have identified the individual by visualizing their identification on their drivers license or ID. Were any roommates home? It seems odd that 911 and/or first responders on the scene never obtained the individuals identifiers prior to arrival at the hospital. It sure makes you wonder how common this may be?!
I was a paramedic and the mistake would have started with the First Responders who provided the hospital with the wrong name, or the hospital ER, mixing up the names. It's routine to get some ID or a wallet. The 911 dispatcher's log has all the relevant information. First to arrive is an engine company, E61, then a delay for a medic unit. This was during Covid where ambulances were in short supply.
In the 911 dispatch log, it shows that Mike Beehler never gave the dispatcher the roommate's age or gender, which is odd. Mike does sound impaired. Maybe he had some head trauma in the past, a stroke, or just low functioning.
It would be unusual that Mike Beehler never went to the hospital to see how his roommate, David Wells, was doing.
He would have discovered that there was no David Wells admitted, then Mike would have contacted the ambulance/fire department to see where the patient was transported to and dug deeper. But hospitals are very good are covering up and burying their mistakes.
There is another story here.
Different areas kept the covid restrictions for different times. For a while people couldn't visit someone in the hospital.

So many comments think Michael (the man that did not die) was the woman and her husband's child, but he was the lady's brother. Michael never was in the hospital. It was the roommate alone that was in the hospital. They are the ones that made the mistake and called his sister to pull the plug. The man that ended up dying (michael's roommate) had a son who speaks around the 4 minute mark.

For instance, several people commented that the woman and man (from the beginning of the video) didn't go visit their SON in the hospital before pulling the plug. But the man in the hospital was mistakenly identified as the woman's brother, not her son. Also, I read.comments that said how did they not know Michael was in the hospital (why didn't they visit him). He was never in the hospital, the roommate was the one actually in the hospital. He is the one that the woman and man pulled the plug on.
I am really sorry if this is still confusing, I was trying to clarify some of the details that got misunderstood.

That was not the case the roommate of their relative was in the hospital? They said pull the plug on their mistaken relative who was really the roommate by accident? So their relative was never in the hospital at all? He just happened to call a week later which confused the couple and realized they called the plug on the wrong person? Do I understand this right?

So they pulled the plug on a guy who could of lived, an the guy who lived was suppose to die, but ended up living. So both patients were not actually at the point of no return in terms of living. Is that correct? Im confused

The son was told his dad died. The only thing he didn't know for 3 years was that strangers were asked to make a decision to pull him off life support.
My God! That's not the kind of mistake that can ever be reconciled or explained. This hospital should be forced to pay dearly for what happened, and both families should be compensated.

Funeral Director here. This is why I bother the hospital nurses to personally take me to the deceased and literally point out the bed.
Its possible the hospital had the deceased in a 'cremation pouch' and mislabeled from the start. However its up the the funeral home to check the mandated hospital I.D. tags on a patient before taking and cremating them.
The issue was he was misidentified while he was first brought into the hospital and still living, his I.D./paperwork was all under the wrong name.
Who agrees to have their child removed from life support without even as much as a trip to the hospital to see them?
It was in 2020 if my math is right, back when visitors weren’t allowed due to the pandemic. And they were his brother & sister-in-law not his parents.
