    Definition of record
    Record is an account of something, written to preserve the knowledge of events. Records and reports are crucial aid to those who are responsible for providing best services to individuals, families, and community. Good reports are time-savers as they prevent duplication of work, reduce errors, and show the efficiency of the staff.
    Definition of report
    Reports are written when the contained information is to be used by many or is of permanent value. A written report should show an awareness of time and thinking, and should focus on the past, present and future state of the patient or family or the event. Also the reports should include description and conclusion of action that influence further planning and decision-making.
