Recently got a copy of my C&P exam for TBI, after a 35 minute exam by a Family Medical Doctor, which covered my heart condition, hypertension, hyperthyroidism my heart bypass scar, incision hernia, Parkinson’s Disease, Chronic Pancreatitis, diabetes and my TBI, (no neurological or psychiatric tests) I have no residual effects on any of those AND my “TBI has been resolved”, imagine my surprise hallelujah I have been cured, my doctors are going to be so excited and surprised, not to mention my priest for a miracle has occurred 😮
My C&P exam was a telehealth and was over within 40 mins with constant interrupts when all said and done got denied with a decision of "not new and relevant evidence",but have been seeing someone for months at a VHA,and that info was in my eVet files, now they either didn't look or just took the examiners decision.
Well, brother, you and I are walking through the woods together because I try to fill out some of these forms and they take me as much as three days to do it because I get overwhelmed and start checking wrong boxes or putting wrong information and I get overwhelmed and have to walk away from it and come back in an hour or a day or whatever. And when I’m working on it, I have to have absolutely zero distractions of anybody around me or a television or radio even my dogs.
Great Video! Question on the DBQ, you used the "Review" form for PTSD. I am filing my INITIAL claim for PTSD and my Psyc has agreed to do my DBQ for me. I have found another form that says it is for "Initial" PTSD but it is dated from 2010 with NO expiration date on it. That form is 21-0960P-4, it is also NOT on the VA Forms website, only the "REVIEW" version is. My question is which form should I have my Psyc Dr fill out so I have a FDC (along with her nexus, Dx and notes of treatment for my CR stressor)?

Can this ptsd form or the mental health form be used as a secondary for tinnitus depression /hopelessness ?
If I hired you, are there physicians that would assist us in filling this out ? Or are we on our own ?
Yall be honest reporting substance abuse. They do not automatically assume it's what causes problems.
What happened to 38 CFR with regard to C&P exams NOT being the primary source for rating disabilities? (Totality of Evidence)
