- Thai Air 311, crashed on July 1992 on the way to Kathmandu from Bangkok. It slammed into the mountain north of Tribhuwan International Airport. It is the second deadliest crash in the history of Aviation of Nepal. Here's a detailed video on how it crashed.

One major problem that happens inside Copkit is the Pilot-Attitute in Captain .
In many flights , first officers or assistant pilots are scared to ask if Captain made any mistakes . Similarly , Captains take things in Ego and think themselves highly intellectual as compared to assistant pilot sitting in side . This creates a unhealthy environment in Copkit and even Though first officer or assistant pilots may be true , sometime deu to this problem , Captain doesn't trust first officers and tens to do everything by their own leading in distraction and much higher workload .
There are more than 100 crashes due to Captain Ego in Pilot causing several deaths . This is also one of many reasons of this plane to crash .
