Tourette syndrome is a neurodevelopmental disorder and can be classified as a disability. Dave wasn't talking about people who are not conventionally beautiful. He was talking about an obvious physical impairment/disability. You can't just disregard this microagression as the same thing as "attacking people for their appearance". Just like you don't have to be black to call out racist behaviour, you don't have to be the recipient of ableist discrimination to call out this disgusting way of thinking.
