Can anyone explain why they laugh so hard at the grandmother bike joke?
I get how it's funny for us italians to hear that phrase translated like that, but not why it got such a reaction from the english people lol

We don't have that as a pre-existing phrase in English so it's very random

I think it's just the comparison.
Sort of like any thing can be something different if you mess it up, a carbonara with ham becomes a pasta with ham but that doesn't make it a carbonara. My grandmother with wheels becomes a vehicle but it doesn't make her a bike

Because calling a woman; 'a bike's means she sleeps around (promiscuous)!

Not a common joke here, and unexpected stuff is really the crux of comedy. A lot of British humor is in the delivery though, and his delivery was about as perfect as it could be, very dry with a straight face and serious voice - which us brits love for some reason. Professional british comedians are generally excellent at timing and that unexpected dry delivery.
Bob Mortimer & Lee Mack are excellent examples of that british timing and delivery which can be make things funny when they aren't even "jokes". As a contrast comedian Joe Lycett is more like "American style" comedy, his jokes are very much in the punchline normally. They still get laughs, but there is something about the very unexpected dry delivery that catches you off guard sometimes and makes things way more funny. Thanks for attending my ted talk

      - it was random
      - the absurd imagery of a sweet grandma with wheels for arms
      - deadpan delivery
      - timing of the joke, nobody was prepared for that
