    This brought back memories of my Dad who had a mastoid operation when he was about 19. He was so scared of hospitals he never went back for it to be cleaned out until he was over 65! Now that must have been a challenge!

    I was just hospitalized for a week for a mastoid infection. Turned out to be staph. I had to get sent home with an intravenous iv to continue antibiotics. Can’t begin to tell you how painful it was, even the healing process. But let me tell you, my ent surgeon was so impressed with how much I knew and it was because of you! Love your Videos and thanks for teaching me so much! (Even though I’d rather not use that knowledge for personal experience lol)

    This patient is a gold star winner for being able to sit and take all of that tugging on that skin in his bad ear. I hope you will film him again when he comes back to finish what was started.

      I wonder if it helps that he has so little hearing in that ear, because he might not find the suction as concerning as other patients?
      But considering how long it's been since anyone got in that ear, he certainly is a champ for sitting that long.

      @@MsZoedog66 oh yes, but I think Rhys has mentioned before that aside from the physical sensation, some people find the suction really loud and that's bothersome to them.

      @@qp2136 The skin is dead but that skin can also be attached to hair and that can twinge some when having ribbons pulled...speaking from experience.

      It has now been 7 months since Mr B. did this extraction which means that he has either done it or is about to do the follow up appointment. I REALLY HOPE WE GET TO SEE A VIDEO OF IT!! 🌹

    I can't imagine what my right ear looks like by now.
    I had a mastoidectomy done in 1995 in an attempt at treating Meniere's. They NEVER told me the area would need to be kept an eye on or cleaned. I have not had a cleaning done on it ever.
    Now, it's actually painful to touch anywhere around the ear, and I can feel that it's badly impacted.
    I am so thankful you could give this man relief. Amazing job! I wish you were my doctor.

    I had a mastoidectomy on my left ear, and I have to get it cleaned out 3 or 4 times a year. I've always been afraid to watch them do it (on the video screen), and it really set my mind at ease to watch this!

      Why do you have to get these? And why do you accumulate so much wax? Is it painful? Does it smell bad??

      @@ashleyc8373 It's not wax, it's debris and discharge. I don't have an eardrum or ossicles, just an open hole to the mastoid cavity, so it sheds skin cells and what not, and sometimes has a mild infection. Regular cleanouts keep it in decent shape. Not usually painful (often feels like scratching a big itch!). Sometimes the discharge is a little smelly.

    I think Rhys has used the term _”lovely”_ to describe every single patient he’s ever had. Can’t recall 1 video where he uses a different adjective or refers to them as anything other than: _Lovely gent. Lovely lady._ or _Lovely patient._ 😆 hahah I just absolutely adore it!

    I know I've told you before, but it's been awhile. But I really appreciate how you explain everything. & Your enthusiasm for your job is so contagious for your fans! I know I just love to listen to your program! & I appreciate Taylor helping out too! Thank you for all you do for these people! I would bring my elderly Mother in if we were in 2 hours apart (more or less). But we live in America. So we'll have to find someone a bit closer.

      It's a rare thing, but it made me feel a bit ill, it was 'Icky.' I did have a big pasta meal earlier on. I'd prefer to see His next visit, when it has dried out a bit. Mastoid Cavity removals are the longer videos that I really enjoy. Thank You Rhys, have a Good week, Take Care.

    When the mastoid bone is removed is it possible to replace it with some type of artificial bone so the ear canal can retain a bit of its normal shape?
    I’m very curious to hear what you have to say. 🤔

    I love these. This is pretty much what I dealt with on my mastoidectomy. Except mine became infected and leaked lots of nasty stuff. Took a number of years to find the right specialist but now it's clear and dry. Get minor skins issues every 12 months but nothing the ENT guy can't solve with some microsuction.

    Rhys, thank you and this patient so much for sharing this very informative video. I have a small suggestion, if you wouldn't mind. When this patient comes back in, please edit in at the beginning of the next video a little footage from the end of this one so that we can remember what it looked like now and see the difference and the value of your advice. I actually wouldn't mind seeing the whole video again, with the followup after it, but that's because this channel is my absolute favorite to watch. As others have mentioned, your professionalism, your genuine care for your patients, and your enthusiasm for your work shines as a tremendous example to not only your colleagues but to all of us who serve in the health care industry in some manner.     I had a radical mastoidectomy due to an advanced cholesteatoma when I was about 12 years old. I'm 40 now and have to get my mastoid cavity and middle ear cleared of debris, dead skin and wax once yearly now. It used to be once every 6 months and before that once every 3 months.
    It's been a crazy experience and there's been some painful and bloody cleanings as that skin really sticks to the flesh and bone. I've found that cortisporin for a week before my appointment really helps soften it up and makes things easy to simply suction out. Being that most of my eardrum is gone it makes for a dizzying experience sometimes. Thanks for the awesome video!

      @@SonOfZeuz I do have some noticeable hearing loss but I do have some strange side effects as well. If I hold a small speaker up to certain parts of my sinuses and press them into my face or by my nostril the sound travels into that ear and it sounds like I have a headphone in that ear. Certain low frequency sounds also seem to vibrate in that ear strangely and sometimes loudly and it can sometimes make me feel off a little. Also, I won't put my head underwater because of water reaching my middle ear and causing vertigo.

      I had a radical mastoidectomy and once I had a piece of wax hitting my semicircular canal through my eardrum so the world jumped everytime I swallowed. It was very weird.

    Oh my goodness! Over 8 centimeters! Wow! I thought you were never going to finish! You need to record when this patient comes back because I HAVE to see how it heals. I can’t believe how much was in there! How in the world did this patient live like this, with his ear plugged up like this? This is one of the hardest ones you’ve done, since I’ve been watching your channel anyway! Wow!

      I wonder if they'll have the patient use bicarbonate drops for a few days before the return visit, or if it's better to keep the ear as dry as possible right up until his appointment.

    WOW!! Epic is right. Trying to remove the wax in the second ear was like herding cats, that stuff was dodgy. I can’t wait to see his 6th month checkup. Mr B, you are so protective of your patients. You said it was through no fault of his own & he was a lovely gent, you blessed him. You are very nurturing to all of the patients.     I’ve had mastoiditis and indeed, it hurts worse than I ever imagined!! Somehow I’ve gotten away from watching this channel as religiously as before & I’m reminded again why I still love it!     I wish I had you when I was going through repeated mastoid infections and having my my incredible pain not only ignored, but I was also accused of making things up for drugs when the infection and swelling were SO obvious, even to non-medical people. I suffered for years! Granted, I have brown skin and a uterus, so I was not entirely surprised so much as incredibly disappointed and angry at having to suffer active disrespect and neglect on top of the misery of pain and infection at the VA. Thank you for acknowledging how painful this can be. I wasn't crazy.
    Also, I couldn't stop watching! It was enthralling!

    I know this was two years ago, but this so far is one of my favorites. I can't imagine the frustration of being an audiologist is when it comes to this, but the satisfaction of both the audiologist and patient at the end seems so rewarding! Who would have thought that a person could carry so much debris in just one canal!!! Great job!!!!

    I had two mastoidectomies in 2012 as I had a cholesteotoma in my right ear so this was really interesting to watch

      I had my "mastoidectomy" in 2003. My cholesteotoma had eaten my entire mastoid, so there wasn't one to remove.

      Had my mastoidectomy in 93. I've had the inside painted many times with permanganate (bright purple) and a couple times silver nitrate. Luckily i don't seem to have a lot of skin slough or wax in there so it's easy to clean. I have had a couple of 5 year stretches with no issues at all. And I can still hear out of it amazing given the cholesteatoma was about an inch or 24mm across and ate into the bone.

    That's earwax/skin removal Porn right there! I've watched Dr Zhao do hundreds of these and they're my favourite. It's great to see yourself do a cavity Rhys, as much as I love Dr Zhao I haven't a clue what he's saying so it was great to have English commentary.

    My son used to get so many ear infections when he was 3-6. My pediatrician at the time was older and he put him on antibiotics for a month. 10 day 2x and the 1x a day for 20 days. He never had another infection. He said that the normal round doesn’t always get the part stuck in the valley so the extra 20 days helped it to get out.

    I was told at the age of 22 that I’d need a double mastoidectomy but it was and still is a major surgery and I asked for a second opinion because I know the posterior auricular nerve is on top or goes over the mastoid bone and can cause the face muscles to droop if disturbed. Thankfully my second opinion didn’t think it was necessary. Did your patient have any facial distortion bc of his surgery?

    Awesome awesome video!! I’m so glad the patient was able to get in and see you. I hope he was able to feel some relief in both ears.
    As much as I wanted to see more removed, the patient comfort and health comes first.

    I am so glad to see this video! I had a cholesteatoma and had have a mastoidectomy 25 yrs ago. I have this problem sometimes but not always. Love your content!!!

      Said as much to my 40 year old daughter who was watching with me and then had to explain what that was.Tripe grossed her out but the ear didn’t.

    Could you not use 0.5-1mm bigger tube on big white skin debri? Getting that bit more grip or even just purely better at sucking it straight up? Yes I understand its tight in there...

    OMG Dr. Barber that was a great Mastoid clean out!! Dr Zhoas usually end with so much more in the canal and you can’t see the ear drum they are so filled with dead skin and blood!
