A player by the name of Don Gallinger was suspended for life for gambling on games when he played for the Boston Bruins. Another player was given the same suspension but I can't recall his name.
That would have been Billy "The Kid" Taylor. He had played for a few different teams, but he had only been on the Rangers roster briefly at the time of his suspension in1948. Both bans weren't lifted until 1970.
I suppose you could sort of make an argument for Marty McSorely being banned for life. He was suspended for the last 23 games of his season, had the suspension extended for a full year, and never played again. Not technically a full ban, but I imagine the NHL probably enacted that kind of suspension thinking that might end his career.
To hit Eddie Shore - one of the most intimidating players in NHL history - to such an extent that one of his ears was in danger of getting amputated is going to get you targeted. To hit him while he's your team mate isn't going to endear yourself to your new employers. To hit him during a practice session - you're lucky not to leave Boston inside a can of what Boston's most famous for.

If he payed $8,000 in three years, and assuming his contract was for $5,000 each season, a good living for professional athletes at the time, he would’ve missed out on nearly 60% of his pay for those three seasons

The Wild Beaver Billy Coutu hall incident with the Ref, sounds like a scene out of Shorsey. Must’ve received his knick name “ Wild Beaver” from his high sticking incidents. The Ear incident was during Billy’s first practice with Boston Bruins, Coutu’s forehead hit Eddie Shore's skull, severing Shore's ear. Shore visited several doctors who wanted to amputate the ear, but finally found one who sewed it back on. After refusing anesthetic, Shore used a mirror to watch the doctor sew the ear back on. Shore claimed Coutu used his hockey stick to cut off the ear, and Coutu was fined $50; Shore later recanted and Coutu's money was refunded. Eddie Shore, was Boston’s Super star in those days. Billy Coutu had some redeeming qualities since he head coached for 4 years after player career on teams making the playoffs and Won Championship 1933-34 season with Providence Reds.

I would've said Bertuzzi too but I think his suspension was artificially inflated by the lockout the next year
