I live in a city that's actually one of the top 20 targets if shit hits the fan, and the population density here is less than 500k.

Six per MIRV before the treaties that limited how many warheads each missile would be allowed to use. Each Minuteman III carries (iirc) 3 MIRVs. So that's theoretically a capacity of 18 warheads. Nowadays it's probably something along the lines of 6-9 warheads, with the rest being decoys, but honestly there's no telling how many would be on an actually armed ICBM if someone goes doomsday on the rest of the world.
It should be launched as-is. It shouldn't be disassembled and re-assembled. Although low probability, issues that would have prevented a successful launch could be corrected during the disassembly/ reassembly process. I understand that it is more probable that issues would be caused as opposed to resolved, it is still good practice to test old equipment that has been sitting in an as-is condition.
Reasons I can see for the disassembly/reassembly: the launches tend to wreck the silos, so they likely didn't want to use a dedicated and "primed" silo for the job. Therefore it had to be moved, and you do NOT want to transport a fully-assembled ICBM anywhere for multiple safety and logistical reasons. Disassembling and reassembling it on location was the best way to guarantee it made it to the test site in "one piece" and didn't end up as a national liability because some nutjob cased the transport.
