Mat this is not the complete use for the smudge tool if you have a brush with certain amout of scattering...and there are more tweaks but I have seen people that use it very differently in portraits and it works very well I used to have a special brush I made myself by accident and it was great for blending, it was actually easier to blend with that than doing it by hand.
The right side pic is essentially from an earlier video showing "on-screen" blending. It's present in this video to show the difference between smudge tool and standard brush tool to blend. Although I don't think he mentions this, I doubt he was actually trying to mislead anyone. I think he just forgot to explain it. The video you're most likely looking for is a later video in the course called "Smudge Tool For Blending". This then moves onto "Best Blending Tools" parts 1 to 3 videos, which includes comparisons of the mixer brush as well as smudge and standard brush.

I blend with the smudge tool by working the colors back and forth, massaging them into place across a gradient.
