The lore currently seems to hint that the warden came from another dimension. The only other potential theory in my mind, founded on the fact that it's name is "warden", implying it protects or otherwise watches over the city, is that the warden was created in response to whatever came out of the portal, possibly made from it. This could be explained as; builders create portal>portal creates too many bad mobs and builders never get to enter it anyway>builders are able to fight the mobs back for a time, but will fall soon if they don't figure something out>builders figure out how to build a golem using part from the new creatures and the souls of their fallen fellow builders, but their numbers are still dwindling rapidly>warden/golem begins defending builders, but it's too late, they perish, but not before managing to close the portal>the warden continues the work of eradicating the otherworldly mobs, after which is goes dormant until the player enters>warden recognizes the potential of the player to reopen the portal and reemerges, hostile, to prevent that. This would also be a lore friendly means for Mojang to refuse to create a deep dark dimension. It's simply too dangerous from a lore perspective. You have to overcome the insurmountable warden only to have your world flooded with otherworldly creatures it took an entire civilization to combat. A civilization that ultimately fell. Compared to them, the player is miniscule.

Fun Fact: iron golems did actually turn on villagers at one point, and it wasn't a creepy pasta. In the snapshots for 1.8 the developers were trying to make iron golems attack creepers, at low health so they wouldn't explode, and I guess they coded something wrong and they started attacking everything unprovoked. Goes without saying they fixed that quickly. They never managed to get the coding right so they reverted it to prevent creepers from destroying villages. They also tried to have mobs run from creepers that are exploding but that caused too much lag.

The voices of the villagers and players could be heard from the soul trapper on its chest and the golem can catch the souls from its victims. But every iron golem cannot fit through two blocks high areas, which means every Minecraft player have to build a two block high base/bunker to stop them from entering. So here's my advice for you, Minecraft player. If you hear voices at a village, build a base immediately.
