- Paciente de 35 años que llega a consulta sin medicar con patrón de alopecia MBP IV difuso, y pelo muy corto. Nos avisa que su intención es seguir llevándolo muy corto, por lo que pretende naturalidad y homogeneización tanto en donante como receptora. Donante con densidad normal pero con pelo extremadamente fino, 40 micras.
Se medica con finasteride y se plantea una estrategia en dos pasos y posible retoque. Se realiza una primera intervención a los 10 meses con 2501 injertos para el área frontal y las entradas (5955 pelos; 2,38 pelos/graft; 0.7 mm punch; 201G1; 1068G2; 932G3; 178G4; 22G5).
Al año se realiza una segunda intervención de 2101 grafts para la zona media y coronilla con ligero refuerzo de frontal (5391 pelos; 2,57 pelos/graft; 0.7 mm punch; 133G1; 921G2; 798G3; 223G4; 26G5).
La dificultad técnica en estos casos radica en injertar entre su pelo nativo sin dañarlo. Contento con el resultado al año, pudiendo mantener un pelo corto, sin white dots visibles y cobertura adecuada, el paciente nos solicita aumentar en lo posible la densidad en la zona frontal, por lo que se realiza un último retoque de 801 injertos (1832 pelos; 2,29 pelos/graft; 0.7 mm punch; 77G1/442G2/257G3/25G4).
Se presenta el video y resultado de la segunda operación después de 4602 grafts.
This 35 yo patients come to consultation without medical treatment and with a diffuse MBP NW IV and very short hairstyle. He aware us that his intention is to continue wearing very short so he asks for naturalness and homogeneization of both areas: donor and recipient. About his hair characteristics, he has normal density but very thin caliber, 40 microns.
We prescribed finasteride and planned a 2 steps strategy with the possibility of a touch-up at the end. The first procedure was after 10 months of med’s and we extracted 2501 grafts for the frontal area and corners (5955 hairs; 2,38 hairs/graft; 0.7 mm punch; 201G1; 1068G2; 932G3; 178G4; 22G5).
After one year we did the second procedure of 2101 grafts for the crown and mid area with a bit in the frontal area (5391 hairs; 2,57 hairs/graft; 0.7 mm punch; 133G1; 921G2; 798G3; 223G4; 26G5). Very difficult placement, due the small gaps in between native hair with the possibility of trauma and shock loss.
Happy with the result after one year, wearing a short hair style, without visible white dots and adecuate coverage, the patient told us to do a last procedure to augment the coverage in the frontal area, reason why we did a small touch-up of 801 grafts (1832 hairs; 2,29 hairs/graft; 0.7 mm punch; 77G1/442G2/257G3/25G4).
