Yoshi-P said more character customization options are coming now that Female Hroth are done. That said, I have a feeling that they won't come until the expansion after Dawntrail. They'll add more hairstyles throughout the expansion as they always do, but anything else I think is better to bet on for the next expansion.
Regarding hairstyles, I do believe female Hrothgar will have more available once they're actually properly available in-game. At the bare minimum they'll have all the unlockable hairstyles available to them, but they might get more base options too that just aren't implemented yet because the expansion still isn't actually finished/available yet.

What you'd like as customization options for FFXIV is unfortunately never going to be implemented due to legacy spaghetti code that they've had to deal with since they worked on 2.0 making it impossible unless they rewrite completely the way character models are handled. Which would be an incredibly tedious task as it would affect everything character related. At best they can offer more options for each category, like more face, more hairstyle, more facial paints, etc, but that's probably it.
You’re quite on the money regarding the world and zones, and the increasing size and quality - back in ARR this was due in no small part to console technical debt: the game had to run on the PS3. I believe both the NoClip and Speakers of Hydaelyn documentaries on the recovery from 1.0, comment on this, but essentially they made a promise to fans that the game would be playable on PS3 with the reboot, and stuck to it. This is why many ARR zones are in multiple parts (loading issues) vs long poorly optimised areas in 1.0. PS3 support ended before the launch of Stormblood, shifting to PS4 only, and Yoshi P has confirmed there’s still room to optimise for that platform, especially as the PS5 has been notably harder to obtain over the last few years comparatively.

Yeah, and Skyen touched on why MMO's in general run into this limitation in that everyone needs to be able to wear nearly every piece of gear (and we saw how people reacted to trying to even slightly move away from that with Hroth/Viera hats). And that itself leads to limitations where most clothes are relatively simply shaped fabrics with texture sort of glued on. That design stipulation helps, but also limits your PC body types because those textures can only be stretched so far and still look good. My favorite example is from WoW. Skyen mentioned a lack of fat character options in MMOs, the male Pandaren in WoW are fat, and as a result a common complaint from people who play them is huge swathes of gear, chest armor especially, looks absolutely horrid on them because the texture on the gear gets stretched into an unintelligible blob.

Which is why I would like for them to replace a story expansion with an immense QoL update for the game that tackles long-term player issue's. But that's incredibly unlikely

I also play a Hrothgar, and yeah, the improvements are subtle, but very much appreciated. The aliasing on the hair especially has always bothered me (although I appreciate how they use that effect to look like embroidery.) I love my character too much to change anything more than the hairstyle, but I'm definitely making a Hrothgal as an alt for an MSQ replay.

It should be noted that no female Hrothgar were a trade-off back in Shadowbringers. They wanted to add new races and had the resources for one of two options: either both genders for one race, or one gender for two new races. They chose the latter to create male Hrothgar and female Viera. Male Viera only were added in Endwalker and female Hrothgar now in Dawntrail
About the customisation limitations, I seem to remember they explained the characters appearence is stored as a series of values. If you add more options, it increases thedata you need to send and receive. It probably wouldn't be a huge problem now, but the base code of the game was built 10 years ago (some parts may be even older from 1.0) when that was a real limitation, so it would probably take a lot of work to add more stuff like this. I wouldn't be surprised if this was the next thing they shoot for now that they don't have plans for more races, but I wouldn't expect anything too drastic either.
I was so thrilled that fem!Hroths had *literally any amount of muscle* that I completely failed to notice the fact that they still have EXTREME sexual dimorphism... MMOs have worn me down more than I thought. Thank heavens for the thriving... underground customization community... I suppose.

Regarding fat people, I believe that there really isn't any models outside of 2 specific once that come to mind. Both of them are part of the Shadowbringers expansion so no spoiler, but one of them is 10/10. People have been asking for customization like that since Shadowbringers.
Although this is a graphical update it isn't a retooling of the entire system. ARR was for all intents a rush job and everything is bound by that 10 yr old version of the game. The models clearly weren't made to work with sliders beyond what they do. It's more just snap into place pieces. They would likely have to rebuild how their models interact with the program entirely. Something like that would end up being ffxiv-2 if the intent is to rebuild systems. Armor is tailor made for each model type and not second skin on the model like some other games. Ronso is the furthest they'll go for bestial races (arguably further than they wanted to go) as the warrior of light is of the races of man and not beastmen. Which brings you to how much the lore restricts certain creative freedoms we get. Not to mention the dev team's own (albeit changing) bias on male/female/hero fantasy. As older platforms continue to be cut off from service the more they can add in options at least .
I am sorry. I end up just stopping and shaking my head to not yell a recording of someone who has not spent ages in the character creation menu. I want to just say this: hrothgar hair styles, tattoos and other face features are locked to each of the 4 faces they have. Both subspecies/color set options (hellion vs Lost ones) have the same 4 faces to pick from
The problem with sexualdimorphismus is I think they did it right with midlander and highlander differences. If all the races had an option to have twink or bear versions everyone would be happy
