The temporomandibular joint - the TMJ - is the joint between the lower jawbone - the mandible - and the temporal bone of the skull. The TMJ is responsible for jaw movement and enables chewing, talking and yawning. Temporomandibular disorders, or TMD, refer to a group of conditions characterized by pain in the jaw area and limited movement of the mandible. TMD may be caused by problems in the joint itself or in the muscles surrounding the joint. Problems in the joint include: arthritis, inflammation and internal derangements. When the problem is in the muscles, the condition is called myofascial pain syndrome.
Myofascial pain syndrome is very common and can occur in patients with a normal temporomandibular joint. The syndrome is characterized by presence of hyperirritable spots located in skeletal muscles called trigger points. A trigger point can be felt as a nodule of muscle with harder than normal consistency. Palpation of trigger points may elicit pain in a different location. This is called referred pain.
Trigger points are developed as a result of muscle overuse. Commonly, the muscles of chewing, or mastication, are overworked when patients excessively clench or grind their teeth unconsciously during sleep. The medical term for this condition is “nocturnal bruxism”. A trigger point is composed of many contraction knots where individual muscle fibers contract and cannot relax. The sustained contraction of muscle sarcomeres compresses local blood supply, resulting in energy shortage of the area. This metabolic crisis activates pain receptors, generating a regional pain pattern that follows a specific nerve passage. The pain patterns are therefore consistent and are well documented for various muscles.
Trigger points in the masseter refer pain to the cheeks, lower jaw, upper and lower molar teeth, eyebrow, inside the ear and around the TMJ area. Trigger points in the temporalis are also associated with headache and toothache from upper teeth. The main culprits of myofascial pain in the TMJ area are the pterygoid muscles. Trigger points in medial pterygoid refer pain to the TMJ region in front of the ear, inside the mouth and upper outside of the neck. They may also manifest as sore throat and difficulty swallowing. Pain from lateral pterygoid trigger points can be felt in front of the ear and on the upper jaw.
Treatments aim to address bruxism, to relieve muscle spasm and release trigger points. Treatment options include:
- Therapies: stress management, behavior therapy, biofeedback - to encourage relaxation.
- Dental night guards: Splints and mouth guards - to protect the teeth from damage.
- Medication: pain relievers, muscle relaxants, botox injections.
- Trigger points release techniques such as needling and “spray and stretch”.

I suffer with the same thing, but I have been seeing a very good chiropractor who does neck and mandible adjustments as well as myofascial & muscle release therapy. After my first treatment I was in tears because I couldn't believe how quickly the pain had dissipated. At first I saw him once a week for a month, now it's once every 2-3 weeks, I can't believe the difference.

The problem can also be bad dental work. If you had a dental work involving implant, crown work, filling can cause it too. Usually bad bite is the most common factor to TMJ TMD

Can it be due to braces too? Ive let my dentist knew i had these type of pain and she did an xtra and said there was nothing… shld i consult another dentist?
I have been gerring Botox injections about every six weeks or so for 17 years. Even with most of my teeth now gone. Without them I think I would nuts from the head pain.
How does botox work with this

Tmj can also be a direct result from, wisdom teeth, cyst, tinnitus...the body tends to clench under stress and or pain...hope this helps all of us out there...for instance I'm getting my bottom 2 wisdom teeth removed next month month as well as a cyst removal...I may have permanent numbness and it sucks....but the problems and paint its been causing me for the last 10 years has taken its toll....

Omg that's exactly my experience!! The most intense pressure in my head. I don't get the jaw pain like everyone talks about, I do have ear aches and neck pain at times, but mostly it's the daily head pressure and no medication will touch it! I thought I was the only one. My anxiety also gets so bad that I cry, can't sleep, or eat. I know this comment was from a year ago and I hate that you were or are still having to deal with this too, but it makes me feel like I'm not crazy knowing someone else feels that god awful head pressure!! I hope you have or will find relief soon.

@@srinivasadireddi Yes, I get that dizziness too! However the doctors say it's not dizziness but my head feels off you know and it's frustrating because I feel like they don't get it? It's kinda hard to explain. I'm so sorry you experience this too, especially with OCD as well ☹. I know my anxiety makes it ten times worse so having to deal with all three must be so rough...I wouldn't wish it on anyone.
I take advil, but been taking them so much, went through 4 bottles of extra strength pills in 1 month.

I have had debilitating headaches and intense pressure specifically on the left side of my face, in the T zone, (above eyebrows, down the nose), my jaw, and eyes. This is been occurring for 3 1/2 months. I suspected it at first to be sinus pressure. I am so glad I found this video and finally understand what I’ve been enduring. Thanks for clarification.

Finally got something which i can connect with my problem.
I only just realised i clench my jaw when i sleep, for over 10 years. I must be doing it pretty bad, because the symptoms i get mess with my mind. I have it right now. My usual symptoms are migraine aura, blindness, hearing is echoed. If i use my jaw all day, like i have today, i get, pressure around my head when i relax my jaw, the pressure moves my eyes a bit, i also get glowy vision, facial pain, dizziness, random thoughts, depression, confusion because of the vision and hearing problem. At first, 10 years ago, it scared the hell out of me, it was a year of panic attacks. These days it's just another day. I get my night guard next week, hopefully that can release some pressure.
It's been a rough ten years. At first it was all panic and fear. When my vision was glowy and hearing was echoed + fibro nerve pain + IBS + blindness + fatigue. It never stopped, doctor had no idea, everybody thought i was crazy., went through severe suicide thoughts. I have been through so much it's too much to tell. After a few years i have 3 pages of symptoms. I figured it out on my own, no specialist or doctor told me, or even mentioned jaw clenching. Everyday for 10 years, depression, pain killer overdose, gastritis cause by painkillers. I guess it wasn't all caused by the clenching. I doubt fibromyalgia is caused by jaw clenching.
Thank you for making this video, ppl need to know how severely bad this jaw clenching can get, and how it can change their lives.
Do you have ear ringing..(tinnitus)?..I've had similar symptoms myself since 2009...I'm having my wisdom teeth removed next month...and my dentist said it I grind my teeth and it may be causing the tinnitus..thanks hope you feel better..
@@80sfreak42 Yep. Been pretty bad the last couple of months. I only just realised it gets worse after eating, talking for a long time, and weather changes. It's 100% TMJ. I can hear it click when moving my neck. I can't believe it's been ten long years. It affects my vision and hearing, i can hardly see and hear. If your ears are ringing, that's a big sign its TMJ.
@@Sixeye_ yes I too have very similar symptoms, especially here in California with these weird weather/climate changes ..I wake up with plugged ears, dry mouth, and jaw pain. I told my I clench my teeth when I sleep and he said after my impacted wisdom teeth removal he's gonna prescribe me a fitted mouth guard to help my tmj...I'm hoping I don't have a wisdom tooth infection because I show most to all the symptoms. It feels like I have a sore throat all the time, a metallic taste all the time, jaw pain, ear pain, ear popping clicking when I swallow water or food. @@80sfreak42 Metallic taste is GERD. I get the same thing with swallowing. ATM i'm trying not to chew so fast. It's gets really bad. Pressure from the TMJ puts pressure on the brain causing thinking problems. My eyes will move to the right by themselves because of the pressure. I used a ice pack on my left TMJ, while laying on the bed, it helps. I think because my injury when i was 16, impact to the face, i think it may be irreversible. Glowy vision, blindness, shadows more visible. I seen the world differently. I hope there's some kinda surgery, i can't live like this. If not, i will going on to smoking weed. Weed is my last resort. Been a hell 10 years. The first years was anxiety and panic attacks. 8 months ago i had kidney stone, which was easy for me. That's how bad it was. I had no idea TMJ could be so severe. I nearly lost my life.
@@Sixeye_ I feel you bro hang in there, I know exactly how you feel. I also have a herniated disc and a hernia in my lower abdomen that have been making my life miserable. The pain and ear ringing wont let me sleep as well. I have to resort to buying xanax from my friends to help me sleep. Sucks cause doctors wont prescribe me anything. Weed does help me a bit but makes the tinnitus a bit louder since it heightens my perception. I hate how all this is psychologically messing with my mind.

I was getting intermittent aching pain in one of my top right teeth (was not a tooth cavity), turns out that there was a huge knot at the trigger point in the temporalis muscle (2:35). When I even lightly massage that spot, the tooth pain instantly goes away -- thanks for the vid

For me I push and massage on the spot in front of my ear or even in my ear and it helps.

I had pain in my ear, head nose and teeth on the right side, i watched some video explaining some exercise which i put both hands in a fist, place then on top cheecks and draw it down with some pressure until your lips squint up like a fish. I did this a few times and was shocked it damn worked! My pain was gone! ❤ painkillers didnt help but with this technique it drew out all tensions.

I can’t tell if I have TMJ, occipital neuralgia, trigeminal neuralgia, intracranial hypertension or giant cell arteritis. It’s been a nightmare and the healthcare system takes too long to figure things out.

I ended up having my right TMJ replaced with a plastic socket screwed onto the back of my cheekbone and a titanium plate attached to my upper and lower jaw. Despite not being able to move my jaw from side to side or back and forth, this was the best thing for me. I had the surgery 13 years ago and its still doing fine to date.

Gua Sha and self massage has helped me a lot with this! I grind my teeth at night probably due to anxiety disorder but am also on SSRI to treat it-but this may cause teeth grinding as well lol 😭 also magnesium supplements can help relax tense muscles. I’ve had Botox in my jaw once and I think it helped but it’s too expensive for me to get often :/

my mouth feels inverted , i feel like i have trigeminal neuralgia but i don’t want to self diagnose myself but it’s been 2 years and it started in my right cheek it felt like it genuinely exploded and it’s continued to get worse everyday , and is now moving to my other cheek as well and my doctor told me “nerves grow back over time” which is so frustrating because no one takes me serious when i say my mouth feels inverted or any of the symptoms i have

I have been having sooo much anxiety because my ear has been hurting/feeling full but i have no ear infection. Ive been under the most stress of my life and anxiety has been through the roof. This explains my ear pain along with the dull pain i feel behind ear on head and down to neck. I also have been getting a stiff sore jaw throughout these past 1.5 months of extreme stress. tell you in 10mins that it's TMJ from clenching in your sleep.

🙏Whenever there is a problem with TMJ or mouth opening, chewing food, you would like to tell you from experience that while eating food, keep equal amount of food on both sides of the tooth inside the mouth and chew it slowly or eat soft food, hard things like Do not use copra, gram, brackish etc., exercise the mouth daily before going to sleep at night and after waking up in the morning and keep taking protein and calcium in the diet and be happy.🙂

I have all the time pain on touch on temporal bone above tmj. I feel nose/forehead/top of head pressure without pain, without congestion. My eyes are watery and I have no sinus infections nor alergy. I have no clue where this pain originates from. My jaw don't hurt, sometimes I feel click sound when I move my jaw from right to left and almost everytime when I go to dentist I have burning pain sensation when I open my mouth longer than 2minutes. I don't feel seek, I breathe normal through nose.

I had TMJ issues all my life. I ended up having 3 major jaw surgeries. I had a total jaw and partial mandible replacement. I also had a double jaw joint derma graft. I still have episodes of severe pain. It's the worse pain ever.
Did that make the pain go away?
I've neen told I have trigeminal neuralgia but I don't believe it. I have constant tooth and jaw pain with a burning sensation. I also have ringing in the ears. I will be starting dtr therapy next week.

I've been suffering from neck & jaw stiffness. first, jaw stiffness started at 2019 or before & neck stiffness at 2020. when jaw stiffness first appeared, i had the feeling of moving it alot to feel comfortable, i cant laugh while my mouth is wide open all the time, and it keeps producing popping & cracking sounds. Then, neck stiffness appeared after a year or so and its near impossible to live a day without moving my neck alot to get relief.
I've tried every most ways to relieve the stiffnesses and trigger points such as: chiropractor, physiotherapists, stretches, exercises, bought multiple neck support pillows and still looking for solutions but i got no results. all doctors told me i have to be fit and carefully exercise alot but at the same time im pretty sure its a problem of my jaw. I also recently developed trigger points in my upper side head & i dont know the reason. ( now i sleep on a normal pillow as neck support pillows are not that comfortable)
has anyone experienced anything like me? and found a solution by any chance?
All sound very similar if not word for word my experience too, very frustrating at times. I'm eager to get sugery I think it is a very good option along with braces to align the bite. Good luck you're certainly not alone x

I have this exact jaw pain and neck pain. Jaw pain started in 2019 and neck pain in 2021. Kindly share the solution you found..also, do you constantly open your mouth wide while talking and it relieves some pain in the jaw?
@@snehamurari6959 I find it hard to talk with my mouth wide open and I talk with my mouth normally open or a bit less, and the jaw pain/stiffness isn't felt all the time.

Loosen the ligaments in your neck.
It will allow your facial muscles to relax. Then from there determine whether or not your facial muscles are swollen or just puffy.. if you need to release muscles.. basically do this.
Pinch the muscle / ligament that feel too tight.. until you feel the area loosen off, then with the fingers you used, separate up and down in the direction of the muscle until you feel a relaxing sensation.. it will feel like the muscles just "let go" and loosen off. You might even feel a cool sensation to the affected muscle.

I can't belive I have gone twice for SOS service ..n they still couldn't figure it out...just giving me. Pain killers...which just don't seem to work!! They didn't even relate closely to these video..n its name of symptoms.....so am.home waiting for a head x-ray appointment 😡😡am pissed off...not.to mention..how.much am.pay for all visits...usually so costly.....How don't they know these symptoms n what causes them..(my dkts that is)pains so bad...you want to scream /bang head/all jaws hurting . I feel like my eustachian tubes are clogged sometimes and I noticed my jaw hurts and the muscles are stiff. I still can't believe this may be due to TMJ syndrome, but I was really worried about having an ear condition, but it all points to this. My dentist told me I clench at night, and my eye teeth are pretty much flat from clenching too. It sucks.
i have very similar symptoms....with ear ringing and i just had my wisdom teeth removed thinking that would help but now my jaw clicks and makes a popping sound...and i still have the ear ringing sounds (tinnitus)...how do you feel now?
I feel the same,it's like I have a magnet pulling things inside my head together such that sometimes I feel the head being heavy,ears ache and it's hard to hear,eye pain after watching or browsing for sometime....it's makes it hard to concentrate

Same, and it get 10 fold worse during cold weather, especially when I get caught up in the rain, I would feel like taking my jaw off.
I’ve been suffering from jaw discomfort for almost a year now! Dentist don’t help at all.. got a custom made guard and it feels like it’s making my jaw crack even more. I’m depressed over this.

I can totally relate to you. I had quite a few teeth removed 3 months ago & I’ve been dealing with this ever since. I’ve also been feeling depressed and I feel like I’m going crazy. Not sleeping very well. I have the tightness & pain in the jaw on both sides & my ears hurt. Dentist didn’t help and had zero recommendations. I’m about to start going to see a chiropractor.
I grind my teeth at night badly. I’ve had a terrible jaw pain on one side for weeks and it hurts to chew. I thought it was a toothache but none of my teeth hurt and I’ve never had a toothache like that before. It’s a muscle pain in my jaw. This explains it perfectly.
