- Dr. Anurag Agarwal demonstrates how the subnasal lip lift is performed to increase red lip show and reduce the length of the upper lip. This is an office procedure performed with local anesthesia. It is also commonly knows as a bullhorn lip lift, or an upper lip lift.
I had this surgery done two years ago, and it was the best money ever spent. It takes 10 years off of you. Don’t freak out when your lip looks lopsided when you go home from surgery. It does even out when the swelling goes down. This is not a very common procedure, but makes the most difference and I feel like it’s a secret procedure that a lot of people don’t talk about that really improved their appearance. Best money spent.

Yes I think it looks way better that injections that can give a ducky upper lip this instead makes the lip show more volume and for people who don't show teeth when smiling or talking it's a great choice 😀
In my opinion there shouldn't be an "one fit all" incision cut shape for every patien out there. It should be a personalized modified incision depending of the patient's lips shape and what is desired. If surgeons do the same incision cut on every single patient, chances are, some are going to get an unpleasant looking result. Remember, it isn't only about shortening the philtrum BUT also making the upper lip look as best as possible. I've seen many lip lift procedures and yes, the philtrum get shorter but the patient's lips endup looking awful and weird. So, in conclusion, this procedure needs to be mastered to the point of respecting the bottom of the nose,(don't distort it), shortening the philtrum the right amount of length and play with the incision to make the upper lip sit on a beautiful shape. It's like juggling with 3 oranges and keep them in motion without failing.

I'm recovering from my lip lift (day 3) and was a lil concerned but watching a few recovery videos feel more at ease.

I'm not as happy as I should be my doctor's sutures weren't done well and have go back for revision thank goodness he was too aggressive with the amount of skin he removed. He gave me pucker and I asked for more of a pout if you know what I mean.

Hello doctor and thank you for answering all those questions posted here. I have a question too. I'm having this procedure done back home in Czech Republic in 2 weeks and it's going to be combined with a QuickLift facelift (vertical lift where the SMAS will also be adjusted). I'm guessing it's like mini facelift here, in the US. I have an option to go for a facial glue instead of stitches for both procedures. What are your thoughts on that? From what I'm understanding, the scar heals better and faster. Thank you for your response :)

Hello. What's most important in my opinion, is eversion of the wound edges when the incisions are closed. This is because scars tend to sink slightly over time. It they are everted, they tend to heal flat. If they are not everted upon closure, they can become depressed incision lines. In my hands I use sutures to maximally evert the wound edges with deep sutures, then close the skin with superficial sutures. As long as the wound edges are everted with deep sutures, it should be ok to use glue just for the superficial skin approximation. I would suggest looking at your surgeon's before and afters to assess their results with glue closure...if they look good to you, then go for it!

I won't be there long enough to have stitches removed, or it would be cutting it very close, but main reason was not having to look like a Frankenstein's bride for 10 days😁I'm having a consultation a day before, so we'll go through everything, I'm pretty sure, as I tend to ask a lot of questions. @@AestheticSurgeryCenterNaples Looks like I misunderstood the use of a surgical glue for my procedures. The doctor will use 2 different kinds of stitches, just like you do and the glue will be used to hold the tissue in place better and help healing, as it contains something to avoid an infection to set it. Something along those lines. But I do feel better (although the doctor that's going to perform my surgeries has excellent reviews and lots and lots of practice), but it's still good to hear the same thing from a different doctor. The scarring from this particular procedure is significant and it never improves with time. You can spot this kind of surgery from across the room.

Hello, If the nose is overprotected and tenting the lip up, then sometimes the lip lowers during a rhinoplasty. But otherwise, no I don't have a procedure for lowering the lip surgically. Other option would be Botox for a gummy smile, which restricts the lip elevator muscles on a temporary basis, making the upper lip seem lower in position.

I think that if you have true keloids or even hypertrophic scar formation, I would stay away from this procedure. It's too front and central on the face to risk having a hypertrophic or keloid scar in that area

Hello Martin, that is a fairly short distance. It's not completely dependent on one number. The height of your upper lip relative to lower lip height matters. The vertical height of the lower third of your face as it compares to the upper two thirds also matters. So I can't give you a clear cut yes or no, but in general, I strive to leave at least 11-12mm of upper lip height.

Does your nasal or nose shape changes with this procedure doctor
Hi, thank you for your question. Generally, no your nose or nasal shape does not change. I think that if one was to remove a large amount of skin, then there is a potential for lowering or widening the nostrils due to tension on the closure. This is avoidable by not removing a great deal of tissue, and also performing a layered deep closure of the SMAS layer and pyriform ligament.
I worry about the bottom lip like will the proportions be off. And will the patient have to get filled indefinitely to keep up with the proportions. You can see at the end the bottom lip is much smaller than the top. Is there a bottom lip lift? Or would it be a pull
Her upper lip is quite swollen at the end due to injection of numbing medicine and the surgical trauma. It all depends on one's starting proportions as to where it will end up. Some people start with similar vertical height to the upper and lower lips, some with less vertical height to the upper lip, etc... There isn't a bottom lip lift per se, as there's no good place to hide the scar. Although uncommon, a V to Y advancement inside the lower lip can sometimes be performed to push the lower lip border lower.

Filler can make the upper lip look fuller but it won't shorten a long upper lip. Also, sometimes too much filler can make the lip protrude forward unnaturally. Lip filler can also reduce dental show. I see many patients who have had too much filler in their upper lip. I then have to dissolve it before performing an upper lip lift. However, if you are just seeking more fullness to the red part of the upper lip, filler is a great option.

Hi Eliza Jo, no I have never done it that way. I like to elevate as much of the horizontal width of the lip as possible. As it is, the subnasal lip lift is great for lifting the central 2/3 of the lip but not as good for the outer corners of the upper lip. I don't think I would like the appearance of solely elevating the very center of the lip as you are suggesting by keeping the incision just under the nasal tip. However, if someone only wanted the very center of the upper lip elevated (based on one's desire or perhaps one's baseline anatomy allowing for this), then it would be possible. I would ask your surgeon to demonstrate exactly which portion of the red lip would be elevated so you won't be disappointed. Hope that helps, Anurag Agarwal, M.D., F.A.C.S.

@@elizajo4265 Hi, I disagree with a blanket statement like that. There is always a fine white scar line that is unavoidable. However, I think it heals well. I have done it on younger patients and they have been pleased. It's all about what is more important to the individual. Does the reduction in upper lip length, increased red lip show, and dental show make it worth having the fine white scar line at the base of the nose? One patient (the blond) is 3 months out so her incision will still mature and improve. The redhead with the nasal septal piercing is a long term follow up.

The blonde has a pretty visible scar and i can't evaluate her either since the before and after pictures are not the same. Before she has lips closed and less makeup and after open mouth. If you get it done make sure to use surgical glue or stitches on the inside. If you use stitches on the outside they will scar on most people

Do you have to cut under the nose ? I understand it hides the scar... also how long for it usually take to heal?
Hello, in order to lift the central part of the upper lip, you have to make the incision under the nose. Stitches in place for one week, then there is redness along the incision line for the first 1-3 months (depends on the person and their skin type). Then eventually, it heals as a fine scar line.
